PURPOSE: Peripheral nerve compression syndromes are a common cause of pain, weakness, and functional limitations, yet they often remain underdiagnosed due to the limitations of traditional diagnostic methods such as electromyography and imaging. This article describes the clinical triad-manual muscle testing (MMT), sensory-collapse testing (SCT), and pain evaluation-as a structured, integrative approach to improving the diagnosis of nerve compressions.
METHODS: This narrative review examines the anatomical basis and diagnostic application of the clinical triad across common peripheral nerve c

INTRODUCTION/AIMS: The aim of this study was to describe a new method for studying the supraclavicular nerve (SCN) conduction and to report four cases with SCN lesions.
METHODS: The SCN was antidromically recorded with a pair of self-adhesive electrodes located in the middle of the clavicle. Stimulation (<5 mA) was delivered 7 cm proximally with a bar electrode. To facilitate recording, it was explained to the participant that they would feel a very faint electrical sensation locally and an electrical tingle upward (ear) or downward (shoulder/clavicle). Each participant was asked to say when

Prognostication following cardiorespiratory arrest relies on the neurological examination, which is supported by neuroimaging and neurophysiological testing. Acute posthypoxic myoclonus (PHM) is a clinical entity that has prognostic significance and historically has been considered an indicator of poor outcome, but this is not invariably the case. "Malignant" and more "benign" forms of acute PHM have been described and differentiating them is key in understanding their meaning in prognosis. Neurophysiological tests, electroencephalogram in particular, and clinical phenotyping are crucial in de

BACKGROUND: Orthostatic tremor is a rare disease characterized by difficulty with standing still. Patients often undergo a long diagnostic search and often feel underrecognized, partly due to unfamiliarity of physicians with the disease.
CASE DESCRIPTION: We report a 72-year-old male experiencing difficulty in standing still, accompanied by a tingling, trembling and painful sensation in the legs, over the last ten years. Several orthopedic and neurological causes had been suspected until neurological examination revealed a 'helicopter sign' upon auscultation of the leg muscles. Tremor registr

BACKGROUND: Useless hand syndrome (UHS) refers to clumsiness of complex finger movements and loss of manual dexterity due to cervical cord lesions, often associated with multiple sclerosis (MS). This condition may represent the first demyelinating event in MS or may occur in a patient with pre-existing MS.
METHODS: We report 3 patients with UHS, 2 of whom had not been previously diagnosed with MS. The history and physical examinations, EMG/NCV studies of the arms, and radiological findings are presented.
RESULTS: Presenting symptoms in all 3 cases included paresthesia and difficulty with man

Diagnosis of Carpal Tunnel Syndrome: Value of Ultrasound Compared to Nerve Conduction Studies Abstract. Carpal tunnel syndrome is the most common compression syndrome of the peripheral nerves. The patient's history with nocturnal brachialgia, daytime brachialgia, nocturnal paraesthesia and daytime paraesthesia (part of the 6-item CTS symptom scale) and a specific clinical exam take part while making a diagnosis. Additional diagnostics include electrophysiological testings. A high-resolution ultrasound examination for the evaluation of the morphology of the median nerve has gained importance in

A thorough neurological examination in emergency situations requires the evaluation of meningeal signs. Even though in most settings, evaluation of meningism is technically not very demanding, the interpretation of findings may prove difficult. As opposed to a widely held belief, clinical signs of meningism are neither specific nor highly sensitive for detection of meningitis or subarachnoid hemorrhage. A meaningful evaluation of meningeal signs, therefore, requires careful consideration of both clinical findings and other accessory symptoms.

Non-systemic vasculitic neuropathy (NSVN) is a vasculitis confined to the peripheral nervous system. It is one of the most common causes of vasculitic neuropathy, along with microscopic polyangiitis and eosinophilic granulomatosis with polyangiitis (Churg-Strauss syndrome). It remains unclear whether this disease is truly an isolated nosological entity, or part of the spectrum of systemic vasculitides. Anti-neutrophil cytoplasmic antibodies (ANCA) are negative in NSVN even though the size of affected vessels in this disease is similar to that in ANCA-associated vasculitis. Although the concept

Ocular pain is most commonly associated with redness and inflammation; however, eye pain can also occur in the absence of grossly visible pathology. Pain in the quiet eye can be the first sign of a number of threatening conditions. Many of these conditions such as intermittent angle closure glaucoma, carotid artery dissection, idiopathic intracranial hypertension, and giant cell arteritis can lead to permanent vision loss or blindness. In this review, ocular history and examination techniques are summarized. The article also reviews pertinent ocular, orbital, referred, and other causes of eye

OBJECTIVE: Parkinson's disease (PD) is a progressive neurological disorder characterised by a large number of motor and non-motor features that can impact on function to a variable degree. This review describes the clinical characteristics of PD with emphasis on those features that differentiate the disease from other parkinsonian disorders.
METHODS: A MedLine search was performed to identify studies that assess the clinical characteristics of PD. Search terms included "Parkinson's disease", "diagnosis" and "signs and symptoms".
RESULTS: Because there is no definitive test for the diagnosis

The life time prevalence of headache is more than 90% in the general population. The majority of patients presenting to physicians suffer from migraine. A simple clinical predictive score based on five questions will allow clinicians to confirm this diagnosis and will prevent further investigations. In all other circumstances, evidence is not sufficient to develop prediction rules to exclude secondary headache. However, neuroimaging should be performed in patients with a unexplained abnormal finding on the neurological examination.

Dizziness is a term which is used to describe a variety of sensations. It is possible to group these complaints into four types: a rotational sensation (Type I dizziness), impending faint (Type II dizziness), dysequilibrium (Type III dizziness) and vague lightheadness (Type IV dizziness). Type I dizziness or vertigo is due to disease of the vestibular system--peripheral or central, and is characterized by a feeling of movement relative to one's surrounding. The majority of dizzy patients, however, belong to Types II, III and IV, collectively called the non-vestibular system disorders. The dist

HTLV-1-associated myelopathy (HAM), also known as tropical spastic paraparesis (TSP), is a chronic progressive demyelinating disease that affects the spinal cord and white matter of the central nervous system. The lifetime incidence of HAM in HTLV-1 carriers is estimated to be less than 5%. Typical time of onset is in the fourth decade of life, with a female-to-male rate of 2:1. Gait disturbance and weakness and stiffness of the lower limbs are common presenting signs and symptoms of HAM. Lower extremities are affected to a much greater degree than upper extremities. Spasticity may be moderate

A case of a girl affected by Landau-Kleffner syndrome (acquired aphasia with convulsive disorder) is reported. The girl showed epileptic seizures and EEG abnormalities. At the age of 7 years 10 months they were followed by onset of aphasic symptoms accompanied with behavioural disturbances. By the age of 9 years she developed a severe verbal auditory agnosia and loss of spoken language expression, but was able to use reading and writing to communicate. This unusual dissociation suggests that spoken or written language can be affected selectively. The girl was followed up to the age of 14 years

Following minor occipital head-trauma, a six-year-old boy developed bilateral internuclear ophthalmoplegia (INO) as the only neurological sign. After having excluded all other possible aetiological causes by careful examination, including magnetic resonance imaging, the symptoms eventually were attributed to the trauma. The course of the INO was documented by electro-oculography. A saccades-training programme specially adapted to the boy's age was performed, and complete remission of the oculomotor symptoms occurred after one year.

Carpal tunnel syndrome is the term given to compression neuropathy of the median nerve in the carpal tunnel. It is the most common pressure-induced peripheral nerve lesion. It is typically associated with numbness of the index, long and ring fingers, and nocturnal brachialgia. The diagnosis is supported by the typical clinical symptomatology and the positive Hoffmann-Tinel sign, and is confirmed by the electroneurographic examination (ENG) of the median nerve, which can also be used for follow-up monitoring. During surgery, careful inspection and palpation of the carpal tunnel must be performe
