Brain-computer interface research can inspire closed-loop neuromodulation therapies, promising spatiotemporal precision for the treatment of brain disorders. Decoding dynamic patient states from brain signals with machine learning is required to leverage this precision, but a standardized framework for invasive brain signal decoding from neural implants does not exist. Here we develop a platform that integrates brain signal decoding with magnetic resonance imaging connectomics and demonstrate its use across 123 h of invasively recorded brain data from 73 neurosurgical patients treated with bra

INTRODUCTION: In everyday clinical neurophysiology practice, mononeuropathies are evaluated primarily by traditional electrodiagnostic testing. We sought to assess the additional benefit of neuromuscular ultrasound (US) in this scenario.
METHODS: All consecutive mononeuropathies undergoing combined US and electrodiagnostic evaluation over a 23-mo period at a single neurophysiology practice were reviewed. Three independent examiners assessed how often US was: (a) "contributory" - enabling a definite diagnosis not made by electrophysiology alone and/or impacting on the therapeutic decision, (b)…

Methylphenidate (MPH), commonly known as Ritalin, is the most widely prescribed drug worldwide to treat patients with attention deficit disorders. Although MPH is thought to modulate catecholamine neurotransmission in the brain, it remains unclear how these neurochemical effects influence neuronal activity and lead to attentional enhancements. Studies in rodents overwhelmingly point to the lateral prefrontal cortex (LPFC) as a main site of action of MPH. To understand the mechanism of action of MPH in a primate brain, we recorded the responses of neuronal populations using chronic multielectro

BACKGROUND: The purpose of this paper is to underline that the association of physical examination, electrophysiology, and ultrasound was crucial in identifying the correct disease and atypical characteristics of its presentation, in turn, providing proper information for both surgical treatment and a rehabilitation program. Fibular nerve compression is a common finding in the adult population, and it is more frequent at the fibular head. The clinical picture may be characterized by motor and/or sensory deficits. The same symptoms may sometimes be suggestive of root impairment.

Structural and functional magnetic resonance imaging modalities have been critical in advancing our understanding of the neuroanatomical and pathophysiological changes that emerge during the premanifest and symptomatic stages of Huntington's disease (HD). However, the relationship between underlying neuropathology and the motor, cognitive and behavioural changes associated with the disorder still remain poorly understood. Less conventional technologies, such as transcranial magnetic stimulation (TMS) and electroencephalography (EEG), provide a unique opportunity to further investigate the caus

Historians of neurophysiology remember Marie Jean Pierre Flourens (1794-1867) for his experimental approach to nineteenth-century debates on cortical localization and, in particular, for his successful attacks on Frantz Joseph Gall's (1758-1828) phrenology (Gall and Spurzheim, 1810-19). Whereas Gall and his colleague, Johann Gaspar Spurzheim (1776-1832), posited correlations between features of the skull and brain development and claimed to have localized character traits, competencies and temperaments in specific cortical regions, Flourens advocated cerebral equipotentiality and provided empi…

The well-accepted biopsychosocial model proposes that the experience of pain and responses to it result from a complex interaction of biological, psychological, and social factors. However, the separation of these constructs is substantially artificial, and we presume that psychological processes have biological effects, that biological processes affect an individual's psychosocial environment, and so on. Considerable research has demonstrated that pain-coping strategies influence perceived pain intensity and physical functioning, and individual differences in styles of pain coping even shape

Microneurography is a method using metal microelectrodes to investigate directly identified neural traffic in myelinated as well as unmyelinated efferent and afferent nerves leading to and coming from muscle and skin in human peripheral nerves in situ. The present paper reviews how this technique has been used in clinical neurophysiology to elucidate the neural mechanisms of autonomic regulation, motor control and sensory functions in humans under physiological and pathological conditions. Microneurography is particularly important to investigate efferent and afferent neural traffic in unmyeli…

Limb lengthening is used to correct leg length discrepancy and to increase stature. The reported frequency of peripheral nerve complications varies from 5% to 30%, but is probably underestimated. Damage may be direct or be caused by overstretching of the nerves. We have used electrophysiological tests to evaluate five patients during bilateral tibial lengthening by the Ilizarov method. Results after 24 to 107 days of lengthening showed electromyographic evidence of partial muscle denervation in all 10 limbs, with reduced motor conduction velocities in two tibial nerves and three common peronea

Recent technological developments facilitated the recording and quantitative analysis of eye movements, to the point where these movements are now widely used to probe diverse kinds of cognition (spatial, temporal, mathematical, social, etc.). This article provides a comprehensive review of the basic neurophysiological aspects underlying gaze direction changes during saccadic and pursuit eye movements, with a focus on primate studies that highlights overlooked but critical complexities. The primary analysis of eye movement kinematics reveals that in response to a moving target, the tracking is

Purinergic signaling represents an ancient intercellular communication system. Operating at the interface between bioenergetic metabolism and damage detection, extracellular purines such as adenosine 5'-triphosphate (ATP) and adenosine (ADO) are recruited into the nervous system for neurotransmission and homeostatic control. Here, we discuss how purine-dependent mechanisms modulate excitation-inhibition balance, neuroplasticity, and neurodevelopment. In the hippocampus, ADO exerts inhibitory tonic functions, while ATP is released during high neuronal activity, being considered a destabilizing

Exercise in neurological rehabilitation is often prescribed to improve mobility, yet its influence extends to cognition through interacting peripheral, central, and behavioral pathways. This narrative review synthesizes evidence on how skeletal muscle activity can engage peripheral biomarkers and system-level interactions that become clinically meaningful only when translated into behaviorally relevant cognitive-motor integration (CMI). We examine neurotrophic, metabolic, immune, vascular, and endocrine relays, then consider central translation through synaptic plasticity, metabolic adaptation

BACKGROUND: Home-based neurophysiological monitoring is improving the assessment and management of neurological conditions such as epilepsy. Technologies such as electroencephalography (EEG), electromyography (EMG), and accelerometry are increasingly integrated into wearable systems for at-home use. Due to an increasing amount of data from long-term monitoring, machine learning algorithms assist in automated data analysis. However, ensuring device accuracy, signal quality, and user compliance remains crucial for clinical useability.
OBJECTIVE: This chapter explores advances and challenges in

Depression, characterized by sustained low moods and even suicidal tendencies, has been intimately linked with mitochondrial dysfunction. This dysfunction is significantly connected with various psychiatric disorders, suggesting its potential role in the pathogenesis and progression of depression. Sirtuin 3 (SIRT3), a potent deacetylase enzyme primarily located within mitochondria, orchestrates mitochondrial function and mitigates various dysfunctions, e.g., insufficient cellular energy supply and oxidative stress. Insufficient cellular energy supply and oxidative stress disrupt normal neuropl

There are very few reports on the effects of benzodiazepines such as midazolam and diazepam on intraoperative visual-evoked potential (VEP), and there is no report on the effect of remimazolam at all. Five patients underwent neurosurgery using VEP monitoring for avoiding surgical injury to the optic nerve. In all cases, drug administration was based on actual body weight. General anesthesia was induced with propofol and remifentanil, and then maintained with propofol at target concentrations of 2.7-3.5 µg/ml for maintaining bispectral index (BIS) between 40 and 60.
