[Severe peripartum acidosis of the mature newborn infant (umbilical artery pH less than or equal to 7.00)--early morbidity and early growth].

Early morbidity and neurological development in 40 term infants with UApH less than or equal to 7,00 (6,60-7,00) were investigated. Group 1: 18 infants (45%) responded well to the measures of primary resuscitation--they never revealed any neurological symptoms and their growth and development were normal. Group 2: 15 infants (37,5%) who responded less well to primary resuscitation initially had mild to moderate neurological symptoms subsiding during the first few days of life. There were 2 cases of necrotizing enterocolitis and 4 cases of moderate meconium aspiration syndrome. Apgar scores, pH

[MRSA sepsis of premature infants].

During the 12 year period from 1978 to 1990, 112 mature newborn and premature infants were diagnosed as sepsis in our nursery. The first case of MRSA sepsis was found in 1985. Since then, cases abruptly increased in number and 31 cases were found in total. Seven cases died and 24 were cured. Antibiotics such as AMK, MINO, IPM were effective. As the sensitivity of these drugs has been gradually dropping, we believe that VCM will be selected as the first choice. Early diagnosis and therapy are most important. Daily measurement of low level CRP (0.1 to 1.0 mg/dl) is useful and careful management

Duration of hospitalization in extremely preterm infants.

This study reports the age at hospital discharge of 233 survivors and age at death of 209 infants who were born at 23-28 weeks gestation over a 10 year period, 1977-86. The mean duration of hospitalization of survivors was 95 days and was inversely related to maturity at birth; those born at 23-25 weeks remained in hospital on average 1-2 weeks beyond term while those born at 26-28 weeks went home on average at term or 1 week before term. The mean age at death was 12 days: 53% within 1 day, 23% between 2 and 7 days, 15% between 8 and 28 days and 9% between 28 days and 1 year. The proportions o

Evaluation of discharge management in the prediction of hyperbilirubinemia: the Jerusalem experience.

OBJECTIVE: We evaluated our program for prediction and follow-up of hyperbilirubinemia in preventing plasma total bilirubin (PTB) > or = 25 mg/dL and in limiting readmission for hyperbilirubinemia. STUDY DESIGN: Term and near-term neonates were screened before discharge for risk factors for hyperbilirubinemia. A PTB test was performed when visible jaundice was apparent. Formal postdischarge follow-up was integrated with a possibly unique religious/cultural support system complemented by ritual circumciser (mohel) home visits and a high rate of jaundice awareness in the community. RESULTS: Du

Separation of Maternal and Newborn Care in US Hospitals: A Systemic Threat to Survival, Health and Well-Being.

Through greater understanding of past social, cultural, economic, political, scientific and technological forces which shaped our current health systems to separate mothers and newborn infants, we can begin to devise effective approaches to reshape these systems to meet the needs of mothers and newborn infants today. Medical science and technology have evolved vastly in the last century; however, effects of historical factors persist in our current health care systems, reflected in separate maternal and neonatal care in different departments with distinct guidelines, providers, and treatment l

Functional EEG connectivity in infants associates with later restricted and repetitive behaviours in autism; a replication study.

We conducted a replication study of our prior report that increased alpha EEG connectivity at 14-months associates with later autism spectrum disorder (ASD) diagnosis, and dimensional variation in restricted interests/repetitive behaviours. 143 infants at high and low familial risk for ASD watched dynamic videos of spinning toys and women singing nursery rhymes while high-density EEG was recorded. Alpha functional connectivity (7-8 Hz) was calculated using the debiased weighted phase lag index. The final sample with clean data included low-risk infants (N = 20), and high-risk infants who at 36

Molecular epidemiology of cytomegalovirus infections in premature twin infants and their mother.

Premature identical twins are described who according to molecular fingerprinting of their viral isolates, demonstrate a nonmaternal nursery source for their acquired cytomegalovirus (CMV) infections. The babies were born via cesarean section at 29 weeks gestation. Weekly urine screening of the infants indicated that at birth both were CMV-negative. Twin B developed CMV at 6 weeks of age, while Twin A developed his infection when he was 9 weeks old. Three months following delivery cervical and urine cultures of the infants' mother were negative and she had no detectable CMV antibody. At 6 mont

Previous antibiotic use and the development of Kawasaki disease: a matched pair case-control study.

BACKGROUND: Kawasaki disease (KD) is an acute febrile illness with systemic vasculitides, mostly affecting infants and young children. The etiology of KD is still unclear; however, altered gut microbiota have been recently implicated as a contributing factor for the development of vasculitis. METHODS: We conducted an age- and gender-matched case-control study on 50 patients and 200 control subjects to search for potential factors leading to intestinal dysbiosis associated with KD. Data were analyzed using conditional multivariable logistic regression. RESULTS: Previous antibiotic administrat

Retinopathy of prematurity: lack of association with prenatal care.

OBJECTIVES: The overall goal of this study was to prospectively assess risk factors for retinopathy of prematurity (ROP) in infants of birth weight <1250 g in an urban population at Bellevue Hospital Center, New York, New York. The hypothesis tested was that lack of prenatal care increases the incidence of ROP. METHODS: A consecutive sample of patients admitted to Bellevue Hospital Center's neonatal intensive care unit/special care nursery who weighed <1250 g at birth and survived until their ophthalmology screening examinations were included in the study. The main outcome measures were prese

Early Bloomers: A Nursing-Driven Program to Support Human Milk Feeding Among Late Preterm Infants in the Mother-Baby Unit.

Late preterm infants have lower breastfeeding rates than term infants, yet few published interventions or guidelines exist to guide hospitals in managing them in the non-intensive nursery setting. Here we describe the development and implementation of an interdisciplinary, hospital-based, breastfeeding support program among late preterm infants not requiring intensive care. The Early Bloomer Program includes an order set applied at birth, immediate lactation consultation, availability of donor milk, parent education on late preterm infants, hand expression teaching and kit including spoon and

Prevention of necrotizing enterocolitis in extremely low birth weight infants by IgG feeding?

UNLABELLED: Oral immunoglobulin has been described as preventing necrotizing enterocolitis(NEC) in preterm infants. To prevent NEC in extremely low birth weight infants (ELBW), we have carried out oral IgG prophylaxis since April 1991. The efficacy of this prophylaxis was examined in a study comparing historical cohorts. ELBW infants delivered in the Department of Obstetrics and Gynaecology of the University of Ulm and treated until day 28 in the level III intensive care nursery, Division of Neonatology, University of Ulm were included. Cohort 1, born between 1.1.1988 and 31.3.1991, received n

Can transcutaneous bilirubinometry safely guide phototherapy treatment of neonatal jaundice in Malawi?

OBJECTIVES: To assess the correlation between total serum bilirubin (TSB) and transcutaneous bilirubin (TcB) values in Malawian newborn infants, and to investigate whether TcB can be used safely to guide phototherapy treatment in the absence of TSB results. METHODS: 128 newborn jaundiced infants were studied in the neonatal nursery at Queen Elizabeth Central Hospital, Blantyre. Paired TSB and TcB measurements (from forehead and sternum) were compared using the linear regression and Bland-Altman methods. Clinical decisions based on TcB results were compared with those based on 'gold standard'

Evidence-based approach to change in clinical practice: introduction of expanded nasal continuous positive airway pressure use in an intensive care nursery.

OBJECTIVE: Recent studies provide evidence that nasal intermittent positive pressure ventilation (NIPPV) may stabilize the airway of extremely low birth weight infants after endotracheal extubation. The objective of this project was to introduce the use of NIPPV into a busy level 3 intensive care nursery. METHODS: This report describes the process of NIPPV introduction using a series of rapid-cycle improvement projects, as proposed by the Vermont Oxford Network. RESULTS: In the first cycle, 7 (88%) of 8 infants were successfully extubated with NIPPV after meeting criteria for reintubation on

Intermodal perception of adult and child faces and voices by infants.

This research investigated the ability of 4- and 7-month-old infants to match unfamiliar, dynamic faces and voices on the basis of age or maturity. In Experiment 1, infants received videotaped trials of an adult and a child of the same gender, side by side, speaking a nursery rhyme in synchrony with one another. The voice to one and then the other face was played in synchrony with the movements of both faces in a random order across 12 trials. On one block of 6 trials a man and a boy were presented, and on the other block a woman and a girl. Results indicated significant matching of the faces

Targeted oxygen therapy in special care nurseries: is uniformity a good thing?

AIM: There is wide variation in the commencement of inspired oxygen (FiO2) and the oxygen saturation (SpO(2) ) targets set in special care nurseries (SCNs). Evidence supports minimising unnecessary oxygen exposure. Does the introduction of a protocol advocating the uniform approach of commencing FiO2 at 30% and targeting SpO2 of 94-96% for infants ≥ 33 weeks gestation with respiratory distress reduce oxygen exposure? METHODS: A 'Before After' study was undertaken in three SCNs. Data were recorded for all infants admitted to the SCNs who required oxygen over a 3-year period. Infants were analy
