Specially trained registered nurses can safely manage epidural analgesia infusion in laboring patients.

PURPOSE: To discover evidence for defining the registered nurse's (RN's) role in the management of epidural analgesia in the labor and delivery setting. DESIGN: The Labor Epidural Nurse Safety (LENS) study consisted of two parts. The first part was a 10-year retrospective review of the outcomes of 2,568 laboring women for whom epidural catheters had been placed and verified by an anesthesiologist or certified registered nurse anesthetist, then continuous epidural infusion initiated, and basal rate or patient-controlled epidural analgesia (PCEA) dose increased, if needed, within specified para

Caregiver-infant interaction--comforting post-operative neonates.

Analysis of 40 h of videotaped caregiving to four male infants was conducted using ethology. All infants were full term and had major chest surgery 12 h previously. All incidents of caregiver-infant interaction were examined, and the verbal and tactile comforting behavior of the caregiver was coded for duration, context, infant cue and infant state prior and following the interaction. A total of 191 comforting incidents occurred during the study. Stroking was the most common form of touch used. Most of the comforting (60%) was given in the context of caregiving; sleeping infants were touched o

Organochlorine pesticides and PCBs in human milk collected from mothers nursing hospitalized children.

Organochlorine compounds in milk were analyzed in samples collected over a nine-year period (1987-1995) from nursing mothers (N = 139) whose children were hospitalized for various disorders. All samples contained p,p'-DDE and PCBs; the median concentrations were 318 micrograms/kg milk fat and 220 micrograms/kg milk fat resp. Higher levels were found in mothers (N = 12) nursing neonates with impaired neurodevelopmental competencies or an inappropriate arousal reaction. No difference was observed between mothers nursing children with respiratory or gastrointestinal diseases, urinary tract infect

Oral contraceptives contraindicated for nursing mother.

F.S. Schroeder (Eureka, Montana) asked: Is it wise or advisable for a nursing mother to take oral contraceptives? Answer (by W.N. Spellacy): It is neither wise nor advisable to prescribe oral contraceptives to nursing mothers. This is because the steroid components, as is the case with most drugs, are excreted into the milk. Their effects on the neonate are variable and dose-dependent. For example, an androgen-type progestagen may masculinize a female infant, and steroid metabolites may contribute to neonatal jaundice. Accordingly, most pharmaceutical companies include a Food and Drug Administ

Targeted educational program improves infant positioning practice in the NICU.

QUALITY PROBLEM OR ISSUE: Infant positioning may interfere with neuromotor development. Bedside education and Infant Positioning Assessment Tool (IPAT) improve nurses' and doctors' proficiency in applying proper infant positioning. INITIAL ASSESSMENT: Nursing compliance with proper positioning is suboptimal due to many factors. One factor was the inadequate knowledge and practice of infant positioning, since the baseline mean IPAT score was 3.4. CHOICE OF SOLUTION: Three experienced neonatal intensive care unit (NICU) nurses were chosen as position champions to help other NICU nurses apply p

Mortality among sons of female dental personnel--a national cohort study.

AIMS: Dental personnel are exposed to mercury when using dental amalgam. This exposure constitutes a potential hazard to offspring of women working in dentistry. The present study examined increased mortality risk in offspring of mothers working in dentistry. METHODS: Mortality was compared between sons of dental personnel and sons of nondental health-care personnel. Hazard ratios were calculated for three decades (1960s-1980s), when the magnitude of mercury exposure in dentistry was likely to have varied. RESULTS: During the 1960s, there was a statistically significant increase in the risk

Nursing assessment and diagnosis of respiratory distress in infants by children's nurses.

Many children's nurses have significant contact with children who have breathing difficulties and should be using systematic criteria to assess their nursing needs. Children's nurses do not appear to follow systematic criteria but are strongly influenced by the medical model and this may be detrimental to holistic assessment and the development of nursing diagnoses based on nursing needs. As nursing, along with other disciplines, develops evidence-based practice, children's nurses should develop evidence for best practice mindful of children's views, parental needs and the education of future

Understanding gastroschisis: an abdominal wall defect.

Gastroschisis is an abdominal wall defect found in the newborn that requires immediate interventions in the delivery room. To provide appropriate management, clinicians must have a clear understanding of the etiology and pathophysiology of gastroschisis. Stabilization in the delivery room is important and begins with establishing an airway, preventing hypothermia, and protecting the exposed bowel from the environment. Surgical treatment is required in the early neonatal period. Adequate preparation prior to repair is essential to ensure optimal outcomes.

Phenytoin as an alternative treatment for preeclampsia.

Magnesium sulfate is the preferred treatment for preeclampsia in the United States. Its use has been criticized because of the maternal, fetal, and neonatal side effects and its tocolytic action during labor. Phenytoin has been identified as an alternative for the treatment of preeclampsia and the prevention of eclampsia. The effects of magnesium sulfate with those of phenytoin on the mother, the fetus, and the neonate are compared. A nursing protocol summarizes nursing care for the obstetric patient receiving phenytoin. Phenytoin has certain demonstrable clinical advantages when used in the i

Evidence-Based Management of Persistent Pulmonary Hypertension.

Persistent pulmonary hypertension of the newborn is characterized by elevated pulmonary vascular resistance, impaired oxygenation, and delayed transition from fetal to adult circulation. Managing infants with pulmonary hypertension is particularly challenging because of the intricate and multifactorial nature of the underlying pathophysiology. A comprehensive and consistent approach to care for these critically ill infants is essential. Although numerous interventions have been extensively studied, a lack of consensus persists in clinical practice, and until recently no formal evidence-based g
