Nuclear, internuclear, and supranuclear ocular motor disorders.

In the brainstem, lateral and vertical eye movements are controlled by separate structures, the former mainly in the pons and the latter in the midbrain. The abducens nucleus (VI) in the pons controls all ipsilateral eye movements, i.e., ipsilateral saccades as well as the horizontal vestibulo-ocular reflex (VOR). This nucleus contains the abduction motoneurons, but also the internuclear neurons involved in adduction, passing through the contralateral medial longitudinal fasciculus (MLF) before relaying in the third-nerve nucleus in the midbrain. Lesions affecting the abducens nucleus result i

[Paroxysmal tonic upgaze of childhood].

We described a child who developed paroxysmal abnormal eye movement. At the age of 12 months, she had a high fever and a febrile seizure. On the next day she showed tonic upward deviation of the eyes for 1 to 2 seconds, and downward saccades on attempted downward gaze. The upward deviation was repeated for a period of 2 to 3 hours, and disappeared during sleep. The administration of L-dopa was not effective. The symptoms subsided gradually over 10 months without other neurological impairment. She walked alone at 1 year and 6 months. At the beginning of her walking she showed truncal ataxia, bu

Neuro-ophthalmology of multiple sclerosis.

Neuro-ophthalmic manifestations are important in multiple sclerosis. Optic neuritis is a frequent harbinger of the disease, and is characterized by subacute painful visual loss, with disproportionate loss of color and contrast sensitivity, central or cecocentral scotoma, and an afferent pupillary defect. Visual recovery is the rule, but tests have been developed to detect most cases of recovered optic neuritis. Intraocular inflammation has come to be recognized as a frequent accompaniment of multiple sclerosis. Because of the predilection of multiple sclerosis for the infratentorial white matt

Opsoclonus-myoclonus syndrome: an autopsy study of three cases.

Opsoclonus-myoclonus is a rare clinical syndrome. We report three patients who died with a pure opsoclonus-myoclonus syndrome. The presentation, management and associations of this distinctive condition are discussed, and the results of detailed pathological examination described. In each case the only abnormality was perivascular collections of lymphocytes widely distributed in the brain. This supports an immune-mediated rather than structural aetiology.

Acute oculogyric crisis after administration of prochlorperazine.

We report a case of acute oculogyric crisis due to prochlorperazine administration in a young black woman with a concomitant viral infection. Neuroleptic medications are the most common cause of drug-induced acute dystonic reactions such as oculogyric crisis. Prochlorperazine is an antiemetic agent with a phenothiazine-type chemical structure and is known to cause dystonic reactions. Drug-induced acute dystonic reactions are most common in young adults and in men. Viral infections may also predispose patients to these adverse reactions. Caution is warranted when this drug is used in patients w

Ophthalmic surgical management of facial paralysis.

Combinations of these procedures have been performed on more than 50 patients to date, with the longest follow-up being greater than 8 years. All of these patients presented with complaints of ocular irritation, tearing, photophobia, and impaired vision. Several had severe epithelial keratopathy. Some patients have also been treated following neurosurgical procedures which caused paralysis of the fifth and seventh cranial nerves, resulting in both corneal anesthesia and facial paralysis. Patients have done very well after these procedures and have achieved ocular comfort, reduced tearing, and

[Oculomotor syndromes resulting from mesencephalic lesions in man].

Midbrain lesions may give rise to the most complex eye movement disorders observed in clinical neurology. Three main types of dysfunction may be delineated, which may be combined: 1) intra-axial fascicular syndromes of the third and fourth cranial nerves; 2) nuclear syndromes of the third and fourth cranial nerves; 3) prenuclear and supranuclear syndromes, in which vertical gaze dysfunction is the main abnormality, but frequently with associated horizontal gaze dysfunction. The fascicular syndromes of the third nerve may be diagnosed when the oculomotor palsy is associated with some specific n

[Vertigo and oculomotor disorders: diagnosis, anatomical classification and etiology].

Dizziness and vertigo are frequent leading symptoms also in the ophthalmological practice and hospitals. They require a structured clinically oriented diagnostic approach. This article provides a practical approach based on a focused history taking using four key questions as well as a systematic examination of ocular motility (four static and four dynamic tests) and four tests assessing the vestibular system. The focus is on differentiating peripheral from central causes through structured history taking and clinical examination. In addition, the functional anatomy of the brainstem and cerebe

Two Syndrome Progressing to Nine Syndrome.

Internuclear ophthalmoplegia with ipsilateral horizontal gaze palsy is called one and a half syndrome. There are a number of numerical one and a half spectrum disorders. One such syndrome is nine syndrome, which is characterized by one and a half syndrome, ipsilateral lower motor neuron type facial palsy, and contralateral hemiparesis or hemihypesthesia or hemiataxia. Here we describe a case of nine syndrome with left one and a half syndrome, left lower motor neuron type facial palsy, and right hemiataxia.

"Pediatric Brown syndrome in the setting of hypercholesterolemia: case report of a possible new association".

Introduction and clinical importance: Brown Syndrome is a form of vertical strabismus characterized by limitation in elevation of the eye in the adducted position, main pathology attributed to superior oblique tendon-trochlear complex, which can either be congenital or acquired, secondary to trauma, inflammation, or iatrogenic in nature. Case Presentation: A 7-year-old male presented for a routine checkup to the out-patient department and upon clinical examination showed a limitation of extraocular movement of the right eye in adduction in elevation, with no deviation in primary gaze. On slit

Pearls & Oy-sters: Radiologic Lag in Pediatric-Onset Multiple Sclerosis.

Recognition of radiologic lag, the presence of clinical symptoms despite absence of a visible corresponding lesion on MRI, is essential when evaluating a patient suspected to have a demyelinating disease. In this report, we present the case of a 16-year-old girl with bilateral internuclear ophthalmoplegias (INO), in whom initial high-resolution 3T MRI failed to reveal an abnormality within the brainstem. Repeat MRI performed 2 months later demonstrated a lesion in the paramedian midbrain tegmentum, corresponding to the anatomical localization of her clinical symptoms. This case aims to raise a
