Palliative psychological care in Germany: Results of a national online survey with psychologists and psycho-oncologists in specialist palliative care settings.

BACKGROUND: Knowledge about the structure and type of palliative psychological care is limited and heterogeneous in most countries. Due to the lack of legal regulations in Germany, it is unclear how psychologists are integrated into multiprofessional palliative care teams (MPCT). OBJECTIVES: To describe how palliative psychology is organised in the various specialist palliative care settings for adults in Germany, how psychologists/psycho-oncologists assess their professional attitude and how satisfied they are with their work. DESIGN: Online survey with psychologists/psycho-oncologists work…

Making patient-oriented decisions with collegial support as an anchor: Oncologists' experiences of late-line treatment selection in metastatic breast cancer.

BACKGROUND: Treatment guidelines support oncologists in treatment decision-making for patients with metastatic breast cancer (MBC). However, treatment decision-making is complicated by the rapid pace of therapeutic advances, the complexity of incorporating patient preferences, and the underrepresentation of diverse populations in clinical trials. This study explored oncologists' experiences of treatment selection in late-line MBC when evidence and guidelines provide limited guidance. MATERIALS AND METHODS: This qualitative study was conducted using a constructivist approach and involved indiv…

Status of Pulmonary Metastasectomy After PuLMiCC Trial: A Survey Amongst Oncologists, Gynecologists, Urologists and Dermatologists on Medical Needs for Local Therapy.

BACKGROUND: The role of pulmonary metastasectomy has been increasingly questioned in the surgical community after the PulMiCC trial challenged its benefit in colorectal cancer. However, the view on pulmonary metastasectomy among people in non-surgical disciplines remains unclear. This study explored interdisciplinary attitudes toward pulmonary metastasectomy and identified the clinical expectations shaping its future role. METHODS: An anonymous online survey of active board-certified physicians in oncology, urology, gynecology and dermatology was conducted (December 2024-June 2025). Twenty it

OBJECTIVE: This survey was designed to evaluate exposure to sentinel mapping for cervical and endometrial cancers in addition to the quality … lymph node staging. The main challenges in applying sentinel procedure for early-career gynecologic oncologists were no access to hands-on training (n = 22, 12.1%) and no clinical routine in performing systematic pelvic (n = 15, 8.2%) and para-aortic (n = 35, 19.2%) lymph node dissection in case of failed mapping. CONCLUSIONS: Although sentinel lymph node bi …

How to deal with renal toxicities from immune-based combination treatments in metastatic renal cell carcinoma. A nephrological consultation for Oncologists.

We are witnessing a revolution in the treatment of metastatic renal cell carcinoma (mRCC). Indeed, several immune-based combinations (ICI [immune checkpoint inhibitor] + ICI, or ICI + antiangiogenic agents) have been approved as first-line therapy for mRCC after demonstrating superior efficacy over the previous standard. Despite all the improvements made, safety remains a critical issue, adverse events (AEs) being the main reason for drug discontinuations or dose reductions, ultimately resulting in an increased risk of losing efficacy. Thus, a good understanding of the AEs associated with the

Risk attitudes and value of hope: survey results from Japanese hematologists and oncologists treating patients with diffuse large B-cell lymphoma.

The "value of hope" refers to treatments that offer a small probability of cure without necessarily improving average survival. While this concept has gained attention in the academic literature, little is known about how physicians perceive and integrate it into clinical decision-making. This study explored the treatment preferences of Japanese hematologists and oncologists in managing diffuse large B-cell lymphoma (DLBCL), with a focus on factors influencing the choice between riskier and more predictable options. An online survey was distributed to 231 physicians, using hypothetical patient…

Patient, care partner, and oncologist perspectives on clinically important survival thresholds after considering time toxicity in advanced cancer.

A treatment's time toxicity (measured using contact days) can decrease days spent at home (home days). We evaluated the minimum clinically important difference (MCID) and non-inferiority margin (NIM) of home days, and treatment acceptance when all additional survival time was spent in health care by surveying people with lived (patients, n = 42; informal care partners, n = 29) and professional (oncologists, n = 8) experience of advanced cancer. All 3 groups reported similar MCIDs and NIMs (6-month survival; median MCID range, 14-15; P = .79; median NIM range, 5-11; P = .14). Care partners (48%…

Use of low-dose immune checkpoint inhibitors in low- and middle-income countries: a national survey of oncologists and hematologists in Mexico.

Immune checkpoint inhibitors (ICIs) have transformed the treatment landscape for multiple cancers but remain largely inaccessible in low- and middle-income countries (LMICs) due to high costs and limited availability. Pharmacokinetic and pharmacodynamic data, together with emerging clinical evidence, suggest that standard approved doses may exceed the threshold required for therapeutic efficacy, supporting the biological plausibility of low-dose strategies. In LMICs, such approaches could broaden access, yet their adoption is still limited. Eighty-one Mexican oncologists and hematologists answ…

High-Grade Pancreatic Neuroendocrine Carcinoma Presenting as Dorsal Spinal Cord Compression: A Diagnostic and Surgical Challenge for the Orthopedic Oncologist.

INTRODUCTION: Pancreatic neuroendocrine carcinomas are rare, high-grade malignancies accounting for <2% of pancreatic cancers. They frequently metastasize to the liver and lymph nodes, while skeletal involvement is uncommon. Spinal metastasis as the initial manifestation is exceedingly rare and poses a diagnostic challenge, especially for orthopedic surgeons who often encounter such lesions under the suspicion of infection or primary bone tumor. CASE REPORT: A 20-year-old male presented with acute-onset paraparesis and loss of bladder control. Radiographs and magnetic resonance imaging reveal

"We're going to get through this together": patient, parent, and oncologist recommendations for providing emotional support in advanced childhood cancer.

A diagnosis of pediatric cancer generates emotional distress for patients and parents, particularly during prognostic discussions in the context of upsetting information and uncertainty. Emotional support from pediatric oncologists is cited as a key factor in psychological well-being during these conversations; however, gaps persist in addressing emotional needs. This qualitative study explored perspectives from 25 patients with cancer aged 12-25 years with an estimated survival of ≤50%, 40 parents, and 20 pediatric oncologists on best practices for providing emotional support during prognosti…

Management of gastrointestinal adverse effects in immune-based combination therapy for advanced renal carcinoma: when the oncologist meets the gastroenterologist.

The advent of combination immunotherapy has revolutionized the treatment of metastatic renal cell carcinoma (mRCC), leveraging immune checkpoint inhibitors and tyrosine kinase inhibitors to improve survival outcomes. However, these advancements come with a significant challenge: the management of adverse effects, which can impair patients' quality of life and lead to therapy discontinuation. This review explores the prevalence, mechanisms, and management of gastrointestinal (GI) toxicity associated with immunotherapy combinations for mRCC from the gastroenterologist's perspective.
