[Management of infected fractures of the mandible by open reduction and rigid internal fixation with reconstructive plates].

PURPOSE: To evaluate the epidemiology and the treatment of infected fractures of the mandible. METHODS: Thirty-five cases of infected mandible fractures treated by extra oral open reduction and rigid internal fixation with reconstructive plates were retrospectively investigated. RESULTS: The fracture healing and occlusion recovery of 34 cases were satisfied without any nonunion or deformity, except 1 case healed after bone grafting for bone defect. CONCLUSIONS: The reconstructive plate fixation is applicable to the treatment of infected mandible fractures by its excellent stability and reli

Reoperation following open reduction and plate fixation of displaced mid-shaft clavicle fractures.

BACKGROUND: Operative fixation of displaced, mid-shaft clavicle fractures has become an increasingly common practice. With this emerging trend, data describing patient outcomes with longer follow-up are necessary. PATIENTS AND METHODS: We retrospectively reviewed the medical records of subjects treated with plate fixation for displaced mid-shaft clavicle fractures from 2003 to 2009 at a Level I trauma hospital. All subjects were greater than 12 months post-index surgery. Treatment involved ORIF with either a low-contact dynamic compression plate (LCDC) or a contoured plate (pre-contoured or p

Outcomes of open reduction and internal fixation of proximal humerus fractures managed with locking plates.

We conducted a study to evaluate the outcomes and complications of open reduction and internal fixation (ORIF) of 2-, 3-, and 4-part proximal humerus fractures using a standard management protocol with locking plates. Of 72 patients with acute proximal humerus fractures managed with ORIF and locking plates, 63 were available at the minimum follow-up of 1 year and met the inclusion criteria. At each follow-up, radiographs were reviewed for healing, hardware failure, osteonecrosis, shoulder range of motion, and DASH (Disabilities of the Arm, Shoulder, and Hand) scores; any complications were rec

Open reduction and internal fixation combined with hinged elbow fixator in capitellum and trochlea fractures.

BACKGROUND AND PURPOSE: The current surgical treatment for displaced fracture of the capitellum and trochlea is open reduction and internal fixation (ORIF), but the results are often unsatisfactory, particularly with complex fractures. Furthermore, the surgical approach, the kind of osteosynthesis, and postoperative management are controversial. We evaluated the results of internal fixation combined with hinged external fixation. METHODS: We analyzed 15 patients with a mean age of 47 (18-65) years. Based on the Bryan-Morrey-McKee classification, the fractures were identified as type I in 6 ca

Open reduction and endobutton fixation of displaced fractures of the lateral end of the clavicle in younger patients.

Displaced fractures of the lateral end of the clavicle in young patients have a high incidence of nonunion and a poor functional outcome after conservative management. Operative treatment is therefore usually recommended. However, current techniques may be associated with complications which require removal of the fixation device. We have evaluated the functional and radiological outcomes using a novel technique of open reduction and internal fixation. A series of 16 patients under 60 years of age with displaced fractures of the lateral end were treated by open reduction and fixation using a t

Comparison of closed reduction alone versus primary open repair of acute nasoseptal fractures.

OBJECTIVE: Nasoseptal injuries have traditionally been treated via closed reduction. Historically, the high incidence of postreduction deformities has led some surgeons to consider alternative approaches to obtain superior results. Here we compare simple closed reduction versus primary open repair of the nasoseptal fracture. STUDY DESIGN: This was a prospective study of 40 consecutive patients treated with simple closed reduction of their combined nasal bone and septal fracture versus 40 patients treated with closed reduction of their nasal bone fracture and open treatment of the septum. Plating of femoral shaft fractures: open reduction and internal fixation versus submuscular fixation.

BACKGROUND: The standard treatment for femoral shaft fractures is intramedullary nailing. However, there are indications for which plating can be performed either openly or in a submuscular manner. METHODS: Between June 1996 and May 2002, two fellowship-trained orthopedic trauma surgeons treated 40 acute diaphyseal femoral fractures in 37 patients with use of plating techniques. Traditional open plating with emphasis on preservation of soft tissue integrity was performed exclusively before February 1999 (n = 19). After that time point, in all but one case (n = 21) submuscular plating techniqu

Open reduction of intracapsular hip fractures using a modified Smith-Petersen surgical exposure.

The Smith-Petersen or modified direct anterior hip surgical exposures have not previously been described for open reduction of femoral neck fractures. This technique of reduction provides a direct approach to the femoral neck and hip joint. Displaced fractures of the femoral neck can easily be reduced through this approach, local osseus defects resulting from impaction can be supported with bone graft, and fracture fixation is then placed through a separate lateral exposure or through small stab incisions. The technique of reduction is presented.

Open reduction of displaced pediatric supracondylar humeral fractures through the anterior cubital approach.

The authors performed a retrospective review of the anterior cubital approach, an alternative exposure for displaced pediatric supracondylar fractures. Anatomic structures that hinder reduction, such as the brachialis muscle or joint capsule, and neurovascular tissues can be identified easily with this approach. Sixty-one children with displaced supracondylar humeral fractures were treated surgically with Kirschner wires using the anterior cubital approach between January 1996 and August 2002. The cohort comprised 35 boys and 26 girls with a mean age of 8.7 years (range 3-13 years). Mean follo

Displaced fractures of the proximal humerus in children require open/closed reduction and internal fixation.

Fractures of the proximal humerus in children are rare and constitute only 3% of all epiphyseal injuries. From 1992 to 2002 sixteen patients aged 4 - 15 years with a displaced fracture of the proximal humerus were treated at our level I trauma unit. The mean follow-up of the patients was 23.8 months (8 - 72). Ten children had a metaphyseal fracture and six a Salter and Harris Type II injury. Only one metaphyseal fracture was treated conservatively; the other patients underwent surgery (ORIF [= open reduction internal fixation] in ten patients, CRIF [= closed reduction internal fixation] in fiv

Distal humerus fractures in elderly patients: results after open reduction and internal fixation.

PROBLEM: Fractures of the distal humerus are difficult to treat. In elderly patients, diminished bone mineral quality and increased trauma-associated joint destruction may make stable joint reconstruction even more problematic. Furthermore, comorbidities and poor tolerance of joint immobilization might be additional factors which influence elbow function negatively. Until now, disagreement has existed on how to treat these fractures in elderly patients. Recommendations range from conservative treatment to primary total elbow replacement. So far, reports in the literature on whether or not open

Open reduction and internal fixation of delayed unions and nonunions of fractures of the distal part of the humerus.

BACKGROUND: The purpose of the present retrospective study was to evaluate the results of open reduction and internal fixation of delayed unions and nonunions of fractures of the distal part of the humerus. METHODS: Between 1976 and 2001, fifty-two patients with a delayed union (thirteen patients) or nonunion (thirty-nine patients) of the distal part of the humerus were treated with open reduction and internal fixation along with selective elbow joint arthrolysis and bone-grafting. The average time to presentation was eighteen months (range, two to 192 months) after the injury. Thirty-nine of

Functional outcome of patients following open reduction internal fixation for bilateral calcaneus fractures.

Treatment of displaced intra-articular calcaneus fractures has historically been controversial, but recent developments have led to resurgence in open reduction internal fixation (ORIF) for displaced calcaneus fractures. Recent functional outcome studies comparing operative to nonoperative treatment of unilateral calcaneus fractures has shown a trend towards improved function with ORIF. No studies have investigated the functional outcome of patients who have required operative treatment of bilateral displaced calcaneus fractures. The purpose of this study was to review our operative experience

Long-term results of tibial plafond fractures treated with open reduction and internal fixation.

In this study a series of 41 consecutive cases of operatively treated fractures of the tibial plafond was retrospectively analysed. The fractures were classified into types I (9.75%), II (41.5%) and III (48.75%) as defined by Rüedi. The 10-year average follow-up revealed 66% good and 24% fair objective results overall. The recent literature uniformly reports good results in operatively treated type-I and -II fractures. In this series, these essentially low-velocity injuries resulted in a satisfactory outcome in 86% of cases. Controversy exists in reports of operative treatment of type-III frac

Open reduction of metatarsal fractures is associated with a safer risk profile than percutaneous fixation.

BACKGROUND: Despite open reduction internal fixation (ORIF) and percutaneous fixation being commonly used for the treatment of metatarsal fractures, the relative complication profiles are unclear. PURPOSE: To compare short- and medium-term complications between ORIF and percutaneous fixation of metatarsal fractures using a large multi-center database. STUDY DESIGN: We used the TriNetX research network to identify two patient cohorts with metatarsal fractures: those who underwent ORIF, and those that underwent percutaneous fixation. METHODS: Two propensity-matched cohorts of 846 patients eac

Risk factors for complications and poor function after open reduction and fixation of olecranon fractures.

BACKGROUND: Surgical treatment of olecranon fractures is associated with a high rate of complications, potential restricted elbow function, and the need for revision surgeries. The aim of this study was to evaluate risk factors for complications and reduced functional outcomes following surgical treatment. METHODS: All consecutive patients aged ≥18 years and treated at our level 1 trauma center for isolated open and closed olecranon fractures between January 2015 and March 2023 were retrospectively analyzed. Data on epidemiology, comorbidities, fracture type, treatment performed, and complica
