PURPOSE: This study investigates the accuracy of either computerized tomography (CT) or magnetic resonance imaging (MRI) for the evaluation of various orbital diseases.
METHODS: We collected 126 CT scans and 65 MRI scans from 144 subjects and asked two radiologists to interpret the images without clinical information. Images included 14 with a clinical diagnosis of orbital infection, 144 with orbital inflammation, and 33 with orbital neoplasm. The inflammatory diseases included thyroid eye disease (TED, n = 69), non-specific orbital inflammation (NSOI, n = 44), IgG4-related disease (IgG4-RD, …

Orbital plasmacytomas are uncommon soft-tissue plasma cell neoplasms that are associated with a monoclonal or myeloma protein. There are four types of plasma cell neoplasms: multiple myeloma (MM), monoclonal gammopathy of undetermined significance (MGUS), amyloidosis, and plasmacytoma. Plasmacytomas may be classified as medullary, occurring only within the bone, or extramedullary, occurring in soft tissues. Orbital plasmacytomas are extramedullary manifestations associated with MM and they may present with signs and symptoms such as unilateral proptosis, conjunctival injection, ocular pain, di…

We performed a retrospective review of patients who presented with delayed dystopia as a consequence of an orbital roof defect due to fractures and nontraumatic causes to search for a correlation between orbital roof defect size and surgical indications for the treatment thereof. Retrospective analyses were performed in 7 patients, all of whom presented with delayed dystopia due to orbital roof defects, between January 2001 and June 2011. The causes of orbital roof defects were displaced orbital roof fractures (5 cases), tumor (1 case), and congenital sphenoid dysplasia (1 case). All 7 patient…

Idiopathic orbital myositis (IOM) is a subtype of orbital inflammatory disease characterized by primarily involving the extraocular muscle. The signs and symptoms of IOM may also be seen in such processes as orbital cellulitis, primary or metastatic orbital neoplasm, carotid-cavernous fistulae, arteriovenous malformations, cavernous sinus thrombosis, and thyroid eye disease, and because there is no pathognomonic sign, symptoms, laboratory test, or radiologic findings, its diagnosis is often provisional. In our case, diagnosis of IOM was more difficult because our patient presented with proptos…

PURPOSE: Conformal radiotherapy (RT) has been used for all patients with orbital lymphoma treated at our institution since 1997. We retrospectively reviewed the charts of 23 consecutive patients to test the hypothesis that partial orbit RT is effective and less toxic than whole orbit RT.
METHODS AND MATERIALS: Twelve patients with limited lesions were treated to partial orbital volumes and 11 patients (1 with bilateral disease) with more extensive lesions received whole orbit RT. The dose was 20-30 Gy (median, 25.2 Gy) for 19 patients with low-grade lymphoma and 24-40 Gy (median, 39.6 Gy) for…

A 75-year-old man presented a twelve-day history of double vision and retro-ocular pain with rapid deterioration of visual acuity. Neurological examination on admission demonstrated right oculomotor palsy and abducence palsy, visual loss in the right eye, and hypesthesia and pain in the right supraorbital nerve. CT scan and MRI disclosed a mass in the right orbital apex. An emergency operation via a frontotemporal extradural approach was performed to decompress the optic nerve. The anterior clinoid process and minor wing of the sphenoid were drilled to expose the optic canal and superior orbit…

Before reconstruction of the orbital walls and other surgical procedures involving the orbits leading to a modification of pathologically altered orbital volumes, it is useful to measure these volumes in order to allow a more precise correction. Orbital volumetric studies on 22 patients and 6 dry skulls were performed using high resolution computer tomography. Fourteen patients presented enophthalmos of various origin, 3 patients fibrous dysplasia involving the orbits and 5 patients showed orbital pathology. In 10 patients with unilateral post-traumatic enophthalmos an increase of the bony orb…

On 7 July 1993, a 61 year-old female presented a seven-day history of double vision and retro-ocular pain. Neurological examination on admission demonstrated right oculomotor palsy and pain in the right frontal nerve. Routine CT scan and MRI disclosed no abnormalities. Her symptoms deteriorated progressively and increased in severity, resulting in complete ptosis and visual disturbance in the right eye, for which she was hospitalized. Angiography also failed to demonstrate any lesion. Herrical CT scan demonstrated a small mass lesion at the right orbital apex. A frontotemporal extradural appro…

Access osteotomies allowing temporary displacement of various segments of the orbital rim provide direct visualization of deep orbital and paraorbital regions. These marginotomies are classified, according to the orbital region involved, as lateral, medial, superior, or inferior, and they provide adequate exposure of the corresponding orbital wall and its surrounding structures. Since these procedures are without complications and cause no unpleasant cosmetic effects, they are highly recommended for the treatment of various lesions in this area as well as for correction of malformations.

Three cases are presented of subperiosteal hematomas, which can be thought of as epidural hematomas of the orbit. One occurred in a child associated with an episode of violent vomiting, the second in an adult with a history of remote head trauma who presented with a slowly progressive course suggestive of thyroid eye disease, and the third in a young patient after acute trauma probably associated with the injections of a local anesthetic agent or possibly the passage of a deep suture. Skull x-rays, computed tomography, and ultrasonographic examinations detected and localized the lesion in each…

Introduction Mucormycosis is an aggressive, lethal fungal infection affecting the nasal and paranasal territory in immunocompromised patients. Orbital involvement is not uncommon and may require orbital exenteration. Objectives The management of orbital involvement in invasive fungal sinusitis is challenging, ranging from conservative retrobulbar amphotericin B injection in the early stages to orbital exenteration in late stages. Endoscopic endonasal debridement is a minimally invasive technique used to manage orbital fungal involvement in the late stages. Methods Endoscopic endonasal orbit…

Congenital anomalies of the orbital roof are rare occurrences. The case of a 2-year-old child with vertical orbital dystopia and abnormalities of the right bony orbit is presented. The patient underwent right orbital reconstruction to restore facial symmetry. A coronal approach with a frontal craniotomy was used for intracranial exposure. The abnormal angulation of the roof was corrected, and the defect was reconstructed with a split-calvarial bone graft harvested from the parietal region. The bone graft was secured with resorbable plate fixation. To preserve vision, reconstruction of this typ…

Oculoplastics is a subspecialty of ophthalmology that focuses on a wide range of disorders affecting the orbit, lacrimal system, and eyelids. In this review, we examine orbital fractures with a focus on mechanisms of injury, clinical and radiologic evaluation, and surgical decision-making. We outline the common fracture patterns and their functional and aesthetic implications, discuss the criteria guiding conservative versus surgical management, and review contemporary surgical approaches along with recent technological advances in orbital reconstruction.

Myxomas are benign, connective tissue tumors of mesenchymal origin, often observed in cardiac tissue, but occasionally arising in the head and neck region. Orbital involvement is exceedingly uncommon, particularly in pediatric patients. This case report presents a 5-year-old male with a progressive decline in ophthalmic function, found to have a periorbital myxoma occupying the right posterior ethmoid cavity and extending into the right orbit. Imaging revealed a nonenhancing expansile lesion with bony remodeling and displacement of extraocular muscles. Complete resection was completed through …

PURPOSE: Total exenteration of the orbit with removal of the eye globe and surrounding tissues is most frequently indicated for malignant tumors. The indications for exenteration of the orbit for benign orbital lesion are rare. Not adequately treated infection of the orbit by systemic antibiotics can lead to destructive changes of soft tissues in the region of the orbit and partial exenteration with eyelid sparing technique is necessary.
DESIGN: Retrospective case series.
METHODS: Data of all patients between 2010 and 2018 who underwent exenteration of the orbit for periocular lesions infilt…

Orbital emphysema is a rare postoperative complication that can cause diplopia and even visual loss. Therefore, immediate diagnosis and treatment are crucial. In this study, a healthy male patient sustained left facial trauma caused by a fall 2 months ago; orbit emphysema occurred when the patient sneezed on the 5th day after surgery. The patient was treated conservatively with oral antibiotics and oral dexamethasone. Sixth-month follow-up showed no residual lesion.
