Long-term stability of alveolar bone grafts in cleft palate patients.

INTRODUCTION: Many authors have examined the surgical bone treatment of cleft palate patients, but no study has emphasized the role of orthodontic therapy. The aims of this study were to evaluate the long-term stability of bone grafts when using an orthodontic-surgical protocol, to determine the success of bone grafts in minor vs severe clefts, and to develop a qualitative method for assessing the success of bone grafting. METHODS: Forty-nine patients were included in this study. Occlusal x-rays were taken before (T0), immediately after (T1), and at least 1 year after bone grafting (T2). Two

[Iatrogenic effects of orthodontic therapy].

Iatrogenic effects of orthodontic treatment are root resorption, pulpal changes, decalcifications and white spots, gingival and periodontal changes, enamel surface changes, temporomandibular dysfunction, immunological reactions, pain and discomfort, and accidents. The authors discuss these items and give, if possible, tips to prevent or reduce these effects.

Evidence-Based Evaluation of Malocclusion Management in the Mixed Dentition: From Prevention to Clinical Controversy.

The management of malocclusion during the mixed dentition is a crucial time for preventive and interceptive orthodontic intervention, given the complex interactions among craniofacial growth, tooth eruption, and orofacial function. Despite widespread use of early orthodontic treatment, uncertainty still remains about the optimal timing, choice of appliance, and long-term stability. This review collates recent evidence and proposes a functional-preventive approach to managing malocclusion in the mixed dentition, emphasizing risk assessment, etiologic control, and individualized treatment planni

À propos d'un cas de résorptions radiculaires sévères en orthodontie : quelle étiologie et quelle prise en charge?Orthodontics and root resorptions: a clinical case.

INTRODUCTION: Root resorptions are, with white spots, some of the inconveniences caused by orthodontic treatments. Although they are rare, they should not be ignored despite the many benefits gained by orthodontic treatment. Contrary to white spots, which are controllable by good dental hygiene, root resorptions can occur despite patient cooperation. Orthodontists should be aware of this phenomenon and make regular radiologic controls a priority for detection "before, during and after" treatment. MATERIAL AND METHOD: After literary references, the presentation of the case report will illustra

Orthodontic root resorption studied by electron microscopy.

The present study supports previous findings that root resorption takes place simultaneously with and after the elimination of hyalinized tissue. The cementoid layer and the more mature periodontal collagen fibers adjacent to cementum are possible barriers preventing root resorption. The microenvironment around hyalinized tissue is favorable for the introduction of hard-tissue resorbing cells. The findings show that the elimination of hyalinized tissue leads to the removal of the cementoid and the mature collagen thus leaving a raw cemental surface without a barrier. It is therefore hypothesiz

Management of Traumatically Intruded Permanent Incisors during an Ongoing Orthodontic Treatment.

Intrusive luxation is one of the most severe types of dental trauma that needs immediate intervention. Various treatment modalities are passive, active, and surgical repositioning performed to prevent healing complications such as root resorption, pulp necrosis, ankylosis, and infraocclusion of the traumatized teeth. This case report discusses a patient with traumatically intruded incisors while undergoing orthodontic treatment. Active repositioning, namely orthodontic traction using light and continuous forces were directed onto the intruded teeth commenced 1 month post the injury and brought

Root Resorption with Orthodontic Mechanics: Pertinent Areas Revisited.

Root resorption can occur at any time during orthodontic treatment and lead to a compromise in the prognosis of the tooth and the stability of the treatment results. Recent research has focused more on the cause and effect relationship as well as preventive or treatment options to combat this unwelcome event. Investigations have highlighted the genetic as well as molecular aspects of the process and enabled clinicians to determine which patients might be susceptible. A proper medical history, an assessment of predisposing factors, a radiographic evaluation for alterations in root morphology an

Mini-implant assisted palate expansion and digital design in junctional epidermolysis bullosa and amelogenesis imperfecta: Case report.

BACKGROUND: Junctional epidermolysis bullosa (JEB) is one of the four major types of EB caused by genetic variants in the genes coding the proteins of the lamina lucida. All patients with this major type of EB present syndromic hypoplastic amelogenesis imperfecta (AI), with either a pits and fissures or generalized hypoplastic phenotype. Severe forms of AI are associated with compromised oral health-related quality of life (QoL) mostly due to poor dental aesthetics, dentofacial anomalies, and oral pain. AIM: To present the comprehensive dental treatment of a patient with JEB and AI from the a

The intrasulcular application effect of bisphosphonate hydrogel toward osteoclast activity and relapse movement.

INTRODUCTION: Orthodontic relapse occurs after orthodontic treatment and shifting of teeth to unfavorable positions. Bisphosphonates' effects on bone resorption and relapse prevention have been extensively investigated. However, topical administration, which results in local effect, is still a problem. OBJECTIVE: This study aimed to investigate the effect of risedronate with gelatin hydrogel as a carrier to prevent relapse movement by inhibiting osteoclast activity. METHODS: Lower incisors of 75 guinea pigs were moved distally using an orthodontic appliance until ±3 mm length. Gelatin hydrog

Pre-operative parafunctional or dysfunctional oral habits are associated with the temporomandibular disorders after orthognathic surgery: An observational cohort study.

BACKGROUND: Temporomandibular disorders (TMDs) are frequent and disabling, and hence, preventing them is an important health issue. Combining orthodontic and surgical treatments for malocclusions has been shown to affect temporomandibular joint (TMJ) health. However, publications regarding the risk factors that predict negative TMJ outcomes after orthognathic surgery are scarce. OBJECTIVE: Present prospective cohort study was conducted to identify an association between pre-operative dysfunctional/parafunctional oral habits and the presence of TMD symptoms after orthognathic surgery. METHOD:

The use of muscle forces by simple orthodontic appliances.

Orthodontic appliances of simple construction have been described. They are valuable in intercepting the deforming action of perioral muscle function or malfunction. The same muscles that have the deforming potential may be enlisted to correct dental malocclusions. Early interception often prevents a problem of considerably greater magnitude at a later date.

Skin irritation in children undergoing orthodontic facemask therapy.

Orthodontic facemasks are extraoral orthodontic appliances that influence maxillary and mandibular development in children with skeletal Class III malocclusion. While a facemask is most effective in patients before the growth spurt, skin irritation is common during the treatment. Therefore, this retrospective study aimed to investigate the prevalence and pattern of such skin changes and identify their possible associated risk factors. We included 177 patients with skeletal Class III malocclusion who underwent facemask therapy. Patient age and sex, orthodontic parameters expressing the severity

Treatment of bimaxillary protrusion using intra- and extra-alveolar miniscrews associated to self-ligating brackets system.

INTRODUCTION: Moderate and severe bimaxillary protrusion impair the passive lip sealing and the facial and smile esthetics. The extraction of premolars can be avoided by the use of skeletal anchorage to retract both dental arches. This approach brings many advantages such as: prevents premolars extraction; simplifies orthodontic mechanics; there is no volume reduction of a premolar when smiling; control of overbite and gingival exposure. The utilization of this therapeutic approach, when associated with self-ligating brackets, can bring the possibility of spacing the appointments without damag
