[Ambulatory treatment of congenital dislocation using a harness to maintain the hip in hyperflexion (author's transl)].

Eighteen children aged between zero and seven months have been treated by a method using Scott's harness to maintain the hips in hyperflexion. Twenty one hips were subluxated. In two cases, the method was inadequate and the hips had to be treated by progressive traction. In one case osteochondritis developed. In other cases the treatment was successful. The technical details of this technique are given.

Evaluating femoral head collapse risk post-fixation removal: a finite element analysis.

BACKGROUND: Femoral neck fractures are prevalent in orthopedic injuries, often leading to complications such as nonunion and osteonecrosis of the femoral head (ONFH). Studies indicate that after healing and removal of internal fixation devices, some patients develop ONFH, while others experience osteosclerosis around the screw holes due to prolonged fixation, increasing ONFH risk. Despite such observations, biomechanical studies on this phenomenon are limited. This study assesses the risk of femoral head collapse post-internal fixation device removal and investigates the biomechanical effects

[Intraoperative imaging with the mobile C-arm : Technique, image creation and radiation protection].

Imaging techniques in the discipline of orthopedics and trauma surgery are essential not only for making a diagnosis but also play a central role in the performance, documentation and monitoring of the corresponding treatment. Procedures such as open reduction and internal fixation (ORIF) and closed reduction and internal fixation (CRIF) of fractures, kyphoplasty and vertebroplasty as well as posterior stabilization of spinal fractures are carried out under X‑ray control. In the relevant operating rooms of emergency departments and operating theaters, mobile fluoroscopy devices are available,

Surgical vs nonsurgical treatment of adults with displaced fractures of the proximal humerus: the PROFHER randomized clinical trial.

IMPORTANCE: The need for surgery for the majority of patients with displaced proximal humeral fractures is unclear, but its use is increasing. OBJECTIVE: To evaluate the clinical effectiveness of surgical vs nonsurgical treatment for adults with displaced fractures of the proximal humerus involving the surgical neck. DESIGN, SETTING, AND PARTICIPANTS: A pragmatic, multicenter, parallel-group, randomized clinical trial, the Proximal Fracture of the Humerus Evaluation by Randomization (PROFHER) trial, recruited 250 patients aged 16 years or older (mean age, 66 years [range, 24-92 years]; 192 [

Posterior instrumentation for thoracolumbar trauma.

The majority of thoracolumbar spine fractures and fracture dislocations may be considered acute sagittal plane deformities. Unstable thoracolumbar spine injuries require stabilization to (1) allow mobilization of the patient to prevent pulmonary and venous complications; (2) to relieve pain; (3) to realign the spine and spinal canal, and (4) to decompress directly or indirectly the neural elements. Posterior spinal instrumentation is a safe, available, familiar, and effective method to achieve these goals. Posterior spinal instrumentation techniques used rod hook systems or screw rod and screw

Biodegradable Devices Across Orthopedic and Maxillofacial Surgery: A Review of Biomaterials, Advances, and Challenges.

The use of biodegradable alternatives to conventional metallic orthopedic devices addresses several inherent limitations of permanent implants by providing temporary mechanical support, obviating the need for secondary removal surgeries, and potentially lowering overall healthcare costs. This review summarizes the principal classes of biodegradable materials-metals (e.g., magnesium, zinc), polymers (e.g., PLGA, PLLA), and bioceramics-and their applications across diverse device types, including screws, nails/rods, plates, and scaffolds. Drawing upon evidence from clinical and preclinical studi

RURS' elbow guard: an innovative treatment of the thumb-sucking habit in a child with Hurler's syndrome.

Thumb sucking is the process of sucking on the thumb for oral pleasure. Thumb and finger sucking habits, or nonnutritive sucking, are considered to be the most prevalent of oral habits. Some parents are concerned by thumb sucking and may even try to restrain the infant or child. In most cases, this is not necessary. Most children stop thumb sucking on their own. When older children continue to suck their thumbs, it could mean they are bored, anxious, or have emotional problems such as depression. This article presents a case report of a child with Hurler's syndrome along with thumb sucking/bit

[Current approaches to the treatment of developmental dysplasia of the hip].

Treatment modalities for developmental dysplasia of the hip are variable. Many authors advocate conservative treatment for infants. However, controversy arises as to the type and timing of osteotomies for older age groups. Besides classic treatment methods, different experimental and clinical studies have been performed. This article aims to summarize new treatment methods in the light of our classical knowledge.

One-stage three-dimensional correction and fusion: a multilevel posterior lumbar interbody fusion procedure for degenerative lumbar kyphoscoliosis. Technical note.

Surgery for degenerative lumbar kyphoscoliosis (DLKS) is very challenging because the curve has become rigid due to circumferential osteoarthritic changes. Therefore, a standard procedure involving correction and fusion after decompression of the nerves has not yet been established. The authors have been searching for an effective procedure that provides adequate decompression and three-dimensional (3D) correction for symptomatic DLKS. In this report they describe a new 3D correction and fusion technique involving multilevel posterior lumbar interbody fusion. They analyze the results obtained

Biomechanical comparison of posterior cervical fixation.

STUDY DESIGN: Biomechanical stability using four different posterior cervical fixation techniques was evaluated in human cadaveric spine. OBJECTIVES: To introduce an alternative interspinous fixation technique using wavy-shaped rods, and to compare its in vitro biomechanical stability with that of other posterior cervical fixation techniques. SUMMARY OF BACKGROUND DATA: Fixation of the posterior cervical spine with interspinous wiring is well known as Rogers' or Bohlman's technique. Recently, several plate fixation techniques have been used for posterior cervical stabilization. Since 1983, t

Review of Harrington rod treatment of spinal trauma.

Harrington rod treatment for spinal trauma has become the gold standard against which other treatment modalities are judged. A review of the results of Harrington rod treatment is essential to establish a baseline level of efficiency in terms of rehabilitation time, correction of deformity, canal decompression, motion segment loss, and device-related complications. With economic concerns becoming more important in medical treatment, the value of newer techniques must be clearly superior to established methods. Harrington rod-augmented spine fusion is reliable and cost-effective in the thoracic

The selection of wires for sublaminar fixation.

The authors report a comparative biomechanical study of the suitability of four types of stainless steel wire for sublaminar fixation. Tensile and fatigue tests were performed on wires in the configuration used at surgery. They conclude that annealed 18-gauge (1.2 mm) wire should be used at each end of a scoliosis fusion, at the upper end of a lumbar fusion, and throughout for thoracolumbar fractures. Cold-worked 20-gauge (0.87 mm) wire may be used in all other situations, but the use of annealed 20-gauge wire should be restricted to the cervical spine. The authors emphasize the importance of

Esophageal perforation following anterior cervical spine surgery.

The authors surveyed the Cervical Spine Research Society to compile a series of esophageal perforations following anterior cervical spine surgery. Twenty-two cases were assembled. Six occurred at the time of surgery, 6 in the postoperative period, and 10 weeks to months later. Eight surgeries were because of fracture. Hardware was implicated in 25% of cases occurring after surgery. Diagnosis was confirmed most often by direct vision at reexploration or esophography. Treatment usually consisted of drainage, repair, and parenteral antibiotics; 2 cases were successfully treated by enteral feeding

Grade 4 spondylolisthesis. Two-stage therapeutic approach of anterior vertebrectomy and anterior-posterior fusion.

The authors present the case of a 23-year-old white woman gymnast with a nine-month history of severe low back pain, spasm and radiation of pain into the buttocks. When back pain had first begun, she was diagnosed elsewhere as having spondylolisthesis, grade unknown. She was started on conservative therapy which included bracing, flexion exercise, and anti-inflammatory and analgesic medication. There were periods of improvement followed by periods of relapse until two months prior to being seen by the authors, when back pain and spasm became constant. Physical examination and x-rays now reveal
