A retrospective cohort study comparing outcomes of spica casting in pediatric femoral shaft fractures by orthopedic surgeons fellowship trained in pediatrics vs. those not fellowship trained in pediatrics.

BACKGROUND: Femoral shaft fractures are common among the pediatric population and are commonly treated with spica casting depending on the patients' age. Both pediatric fellowship-trained and non-pediatric-trained orthopedic surgeons may be expected to treat these injuries while taking call. The purpose of this study is to determine differences in outcomes between casting performed by pediatric-trained orthopedic surgeons and non-pediatric-trained orthopedic surgeons. MATERIALS AND METHODS: A retrospective cohort study of 276 patients aged 0-5 years treated with a hip spica cast for isolated

Perioperative goal-directed therapy and postoperative complications in different kind of surgical procedures: an updated meta-analysis.

BACKGROUND: Goal-directed therapy (GDT) aims to assure tissue perfusion, by optimizing doses and timing of fluids, inotropes, and vasopresso … high- and low-risk patients. A significant effect was observed in those RCTs enrolling exclusively orthopedic procedures (OR 0.53, 95% CI 0.35-0.80, p = 0.002, 7 RCTs, 650 patients. Also neurosurgical procedures seemed to benefit from GDT (OR 0.40, 95% CI 0.21-0.78, p = 0.008, 2 RCTs, 208 patients). In both major abdominal and orthopedic surgery, a strategy adopting f …

The Incidence of Perioperative Cardiac Events after Orthopedic Surgery: A Single Institutional Experience of Cases Performed over One Year.

BACKGROUND: Orthopedic patients with ischemic heart disease are at risk for postoperative cardiac complications. QUESTIONS/PURPOSES: Using … s ratios from logistic regression models. RESULTS: During a one-year period, from 10,627 inpatient orthopedic procedures, 805 patients were identified as at risk for postoperative myocardial ischemia. A total of 20.6% (166/805) of these patients had elevated serum cTnI levels (cTnI > 0.02 ng/mL), and there were ten documented postoperative MIs (10/805; 1.2%). For the …

The Management of Orthopaedic Emergencies for the Community Surgeon.

Orthopaedic emergencies demand rapid recognition and decisive action, particularly in resource-constrained or nontertiary settings where the community surgeon often serves as the first and only line of defense. It is important to provide a practical, evidence-informed guide for the management of musculoskeletal injuries, including irreducible joint dislocations, ultra-low-energy knee dislocations, open fractures with vascular compromise, compartment syndrome, and femoral neck fractures in young patients. Emphasis is placed on early diagnosis, stabilization strategies, soft-tissue management, a

Can we ever have evidence-based decision making in orthopaedics? A qualitative evidence synthesis and conceptual framework.

BACKGROUND: The perception and use of scientific evidence in orthopaedic surgical decision-making is variable, and there is considerable variation in practice. A previous conceptual framework described eight different drivers of orthopaedic surgical decision-making: formal codified and managerial knowledge, medical socialisation, cultural, normative and political influence, training and formal education, experiential factors, and individual patient and surgeon factors. This Qualitative Evidence Synthesis (QES) aims to refine the conceptual framework to understand how these drivers of decision-

Streamlining orthopaedic trauma surgical care: do all patients need medical clearance?

INTRODUCTION: Preoperative medical optimization is necessary for safe and efficient care of the orthopaedic trauma patient. To improve care quality and value, a preoperative matrix was created to more appropriately utilize subspecialty consultation and avoid unnecessary consults, testing, and operating room delays. Our study compares surgical variables before and after implementation of the matrix to assess its utility. METHODS: A retrospective review of all orthopaedic trauma cases 6 months before and after the use of the matrix (2/2021-8/2021) was conducted an urban, level one trauma centre

Variability in Hand Surgery Training Among Plastic and Orthopaedic Surgery Residents.

BACKGROUND: A career in hand surgery in the United States requires a 1-year fellowship after residency training. Different residency specialty programs may vary in case volume. The purpose of this study was to characterize variation in hand surgery training within and between orthopaedic and plastic surgery residents. METHODS: Publicly available hand surgery case logs for graduating orthopaedic and plastic surgery residents during the 2010 to 2011 to 2018 to 2019 academic years were obtained through the Accreditation Council of Graduate Medical Education. Student t-tests were used to compare

The role of geographic disparities in outcomes after orthopaedic trauma surgery.

INTRODUCTION: Healthcare disparities linked to patient rurality and socioeconomic status are known to exist, but few studies have examined the effect of urban versus rural status on outcomes after orthopedic trauma surgery. The aim of this study was to examine the correlation between patient rurality, socioeconomic status, and outcomes after orthopedic trauma. MATERIALS AND METHODS: This is a retrospective cohort study of patients diagnosed with a hip or long bone fracture between January 2016 and December 2017. Data were collected from the Nationwide Inpatient Sample (NIS), a 20% weighted sa

Congenital syndactyly: a retrospective study of 18 cases at the Department of Orthopaedic Surgery and Traumatology of the Habib Bourguiba University Hospital, Sfax, Tunisia.

Congenital syndactylies are frequent congenital malformations of the hand. They can be an isolated finding or they can be found in association with other polymalformative syndromes. Several surgical techniques used to treat them have been described in the literature. The most used is the dorsal commissural omega-flap technique. We here report a study of 18 patients with congenital syndactyly, with multiple involvement in several cases, whose data were collected at the Department of Orthopedics and Traumatology of Sfax (Tunisia). All patients were operated using the dorsal commissural omega-fla

Factors Considered When Ranking Military Orthopedic Surgery Residency Candidates.

INTRODUCTION: The intangible personal characteristic grit has become a popular topic of concentration within contemporary psychology studies. Grit is defined as persistent work on a focused topic for an extended period of time until a desired goal is achieved. Its application to physician selection and development is not well known. We sought to determine which factors were considered most important among leadership within the U.S. Army's orthopedic surgical training programs when selecting applications and ascertain if grit was one of the primary factors. MATERIALS AND METHODS: A standardize

Precision Medicine Applications to Manage Multiply Injured Patients With Orthopaedic Trauma.

Precision medicine offers potential for improved outcomes by tailoring interventions based on patient-specific demographics and disease-specific data. Precision methods are relatively unexplored in trauma patients. New research is being looked at for precision methods to treat patients with large extremity wounds, nonunions, and fractures associated with polytrauma. Precision-based clinical decision tools are being validated to optimize timing for open wound definitive closure. Early patient-specific biomarkers to stratify nonunion risk within 1 week of fracture are being explored. Patient-spe

Orthopaedic Considerations in the Management of Skeletal Sarcoidosis.

Advanced imaging has demonstrated that musculoskeletal manifestations of systemic sarcoidosis are more common than previously thought. A definitive strategy for the management of osseous sarcoidosis has not been defined. Some lesions resolve spontaneously, and no systemic medication for sarcoidosis consistently resolves lesions. The orthopaedic surgeon treating patients with musculoskeletal sarcoidosis must make an appropriate diagnosis of bony lesions, seek multidisciplinary input from specialists in pulmonology and rheumatology regarding systemic treatment, and decide when surgery is necessa
