Stapedectomy in Teunissen-Cremers Syndrome: Intraoperative Findings and Hearing Outcomes.

OBJECTIVE: To describe the intraoperative findings and outcomes of stapedectomy surgery in Teunissen-Cremers syndrome. PATIENTS: A family of three patients with bilateral conductive hearing loss because of Teunissen-Cremers syndrome. INTERVENTION: Six exploratory tympanotomies and stapedectomies, including one revision operation. MAIN OUTCOME MEASURES: Intraoperative findings and postoperative hearing results. RESULTS: There was an increased distance between the incus and the vestibule, a thicker long process of the incus, and slight variation in the relative position of the ossicles in al

[Microsurgery of large middle ear abnormalities. Technical surgical considerations].

From 1988 to 1994, 75 patients with congenital aural atresias were operated on at the University ENT Department, Würzburg. Sixty-two patients (70 ears) could be followed up in their postoperative outcomes. The surgical approach used in all patients was a direct one through the atresia plate. To avoid infection and stenosis of the new meatus, a neo-tympanic membrane was built up in a far lateral position. This was possible by using a long ossicular prosthesis. The mean postoperative gain was 17.8 dB and the mean postoperative airbone gap was 31.0 dB. Twenty-two percent of the ears had postopera…

New perspectives for middle ear implants: first results in otosclerosis with mixed hearing loss.

Middle ear implantation is an efficient procedure to restore moderate to severe sensorineural hearing loss (HL) in selected patients. Implantation of such devices requires ossicular chain integrity. Patients suffering from otosclerosis with mixed HL should be eligible for this treatment after stapes surgery with air-bone gap closure. To address this issue, we report four cases of middle ear implantation after or during stapes surgery. Results and complications obtained with Vibrant SoundBridge, MedEl and Middle Ear Transducer, Otologics are reported. Audiologic results were similar to those ob

Titanium incus interposition in ossiculoplasty: A retrospective study.

PURPOSE: This study aimed to evaluate the outcomes of ossiculoplasty using titanium incus interposition prostheses, focusing on audiological benefits, extrusion and dislocation rates. METHODS: This retrospective cohort study was conducted at a tertiary referral center and included 27 patients with conductive hearing loss who underwent ossiculoplasty using titanium incus interposition prostheses between 2015 and 2023. Preoperative and postoperative (minimum 6 months) otoscopic examinations and audiological assessments (average thresholds at 0.5, 1, 2, and 3 kHz) were performed. Patients were c

Scoping Review Protocol: Quality of Life in Children with Active Middle Ear Implants.

Active middle ear implants, particularly the Vibrant Soundbridge (VSB), represent an established audiological treatment option for children with conductive, sensorineural, or mixed hearing loss when conventional hearing aids are insufficient or cannot be worn. In addition to functional improvements, the assessment of health-related quality of life (HRQoL) is gaining increasing importance. While extensive data on quality of life exist for adults with cochlear implants, only a few methodologically heterogeneous studies have examined HRQoL in children with active middle ear implants. This protoco

Measurement of Incus and Stapes Velocity Following Sound Stimulus in the Setting of Incudostapedial Joint Separation and Reconstruction.

OBJECTIVE: Understand the biophysical property changes of incudostapedial joint (ISJ) separation and ossicular hydroxyapatite application on middle ear function. STUDY DESIGN: Basic science. SETTING: Cadaveric temporal bone research laboratory. SUBJECTS AND METHODS: A complete mastoidectomy was performed on five human temporal bones. A Laser Doppler Vibrometer was utilized to obtain velocity transfer function measurements of the incus and stapes across a range of frequencies in response to an acoustic stimulus. Under binocular microscopy the ISJ was separated and subsequently repaired with

[The practice of stapes surgury in Russia].

The aim of this study - to investigate the practice of stapes surgery in Russia using a special questionnaire (Appendix 1). Thirty-three questionnaires were sent to specialists dealing with stapes surgery in various Russian medical institutions. Responses were received from all ENT specialists, of whom 75.7% use stapedotomy as a technique. In most cases, colleagues perform more than 30 operations per year, there were also reports of performing about 100 operations per year and in one case 370 operations annually. 45.5% of respondents preferred a combination of general and local anesthesia for

[Tendoplasty and its effectiveness in the rehabilitation of hearing loss in patients with otosclerosis].

The article briefly presents the physiology of the stapes muscle tendon (SMT), and features in its defeat. Its length was studied using high-resolution multispiral computed tomography of the temporal bones. We also studied the possibility of its restoration using the author's method of tendoplasty using metallized stapes prostheses. Tendoplasty with stapedoplasty was performed in 74 patients with otosclerosis (OS), and 48 patients had stapedoplasty without tendoplasty. As a result of research, the average length of the SMT was 2.38±0.02 mm, which explains the need to use a 3 mm long venous aut

Surgical and Technical Modalities for Hearing Restoration in Ear Malformations.

Malformations of the external and middle ear often go along with an aesthetic and functional handicap. Independent of additional aesthetic procedures, a successful functional hearing restoration leads to a tremendous gain in quality of life for affected patients. The introduction of implantable hearing systems (bone conduction and middle ear devices) offers new therapeutic options in this field. We focus on functional rehabilitation of patients with malformations, either by surgical reconstruction or the use of different implantable hearing devices, depending on the disease itself and the seve

The effect of angulation of the vibrating floating mass transducer on stapes velocity.

HYPOTHESIS: Changes to the angular position of the vibrating floating mass transducer (FMT) coupled to the long process of the incus will not affect stapes velocity. OBJECTIVE: The MED-EL Vibrant Soundbridge is an active middle ear implantable device, which constitutes an effective alternative to acoustic hearing aids for the rehabilitation of patients with sensorineural and mixed hearing loss. Because of varied anatomy, it is not always possible to position the FMT in line with the vibrating axis of the stapes. Changes in stapes velocity after angulation of the FMT are measured using laser D

Indications and candidacy for active middle ear implants.

Currently, there are two active middle ear implants available commercially: the Vibrant Soundbridge system and the Carina system. A third active middle ear implant, the Esteem, is under clinical evaluation. All devices are indicated for patients with moderate-to-severe hearing loss. Because active middle ear implants are directly coupled to middle ear structures, many of the problems that patients with conventional hearing aids report, such as acoustic feedback, occlusion, and irritation of the outer ear canal, are avoided. In addition, AMEI patients perform well in background noise. However,

[Tympanoplasty].

The first goal of tympanoplasty as treatment for chronic otitis media is to obtain a clean, disease-free ear, the second one hearing improvement. Prerequisite for success is the sufficient functioning of the inner ear as well as of the Eustachian tube. Therefore, patients very often have to undergo adjuvant treatment such as adenotomy, rhino-surgery and surgery of the paranasal sinuses. Microsurgery of the ear has become very refined. Thus even after removal of extensive inflammation it is now possible to rebuild the middle ear using various proved as well as newly developed biomaterials. Ofte
