Pycnodysostosis Revealed by Recurrent Fractures: Report of Two Cases.

Pycnodysostosis is a rare autosomal recessive skeletal dysplasia caused by cathepsin K deficiency and characterized by osteosclerosis and bone fragility. Recurrent low-trauma fractures and delayed healing represent major orthopedic challenges, particularly when medullary canal narrowing limits surgical options. We report the case of two brothers born to consanguineous parents who presented with short stature, characteristic craniofacial dysmorphism, acro-osteolysis, and recurrent fractures after minor trauma. The older sibling had the more severe skeletal phenotype, with 14 fractures involving

Glomus tumour eroding the medial facet of the patella: a case report.

Glomus tumours are benign lesions occurring essentially in the hand, with a predilection for the subungueal area. Even though they have been reported in literature, extradigital localisations of glomus tumours are frequently misdiagnosed; they never lead to bone osteolysis. We report the case of a patient who presented with a typical patellofemoral syndrome due to a glomus tumour that eroded the medial facet of the patella. MRI, which is the gold standard for the diagnosis, showed a well-delineated spherical tumour with enhancement after Gadolinium injection. CT Scan excluded an osteoid osteom

p38 MAPK as a potential therapeutic target for inflammatory osteolysis.

Inflammatory osteolysis is a relatively frequent and incapacitating complication of rheumatoid arthritis and other inflammatory diseases, and is induced by accelerated osteoclast recruitment and activation in bone under the aegis of cytokines produced in the inflammatory environment. The success of antitumor necrosis factor-alpha and interleukin-1 therapy in correcting this condition highlights the central role of these cytokines in this process. Recent years have witnessed a revolution in understanding the molecular mechanism and pathogenesis of this family of diseases. It is now clear that p

Changes in protein expression associated with chronic in vitro exposure of hexavalent chromium to osteoblasts and monocytes: a proteomic approach.

Cr (VI) is a well-recognized environmental toxin and carcinogen. It is known to be released from orthopedic metal implants in-situ by biocorrosion and is speculated to play a role in periprosthetic osteolysis. It is hence essential to understand its long-term biological effects. We have assessed the in vitro responses of osteoblasts and monocytes to chronic exposure (3 weeks) to Cr (VI), at concentrations that have been measured in patients with metal implants, using two-dimensional gel electrophoresis. Cr (VI) exposure resulted in a differential time-dependent regulation of glycolytic, stress

Genistein and coumestrol reduce MCF-7 breast cancer cell viability and inhibit markers of preferential metastasis, bone matrix attachment and tumor-induced osteoclastogenesis.

The propensity of breast cancer to preferentially metastasize to the skeleton is well known. Once established in bone metastatic breast cancers have a poor prognosis due to their ability to promote extensive bone loss which augments tumor burden. Unfortunately, current anti-resorptive therapies for skeletal metastasis are typically prescribed after secondary tumors have formed and are palliative in nature. One group of compounds with the potential to reduce both tumor burden and osteolysis are phytoestrogens (PE), but the mechanisms mediating a beneficial effect are unclear. Therefore, the cur

Gorham's Disease With Chest Wall Involvement: A Case Report and a Review of the Literature.

INTRODUCTION: Gorham's disease is a rare disorder characterized by osteolysis and abnormal vascular growth within bones. Diagnosis of Gorham's disease is often delayed and for accurate and early diagnosis high clinical suspicion is crucial. No specific treatment is available. Management options include surgery, radiation therapy and medical therapy. We aimed to present the first case of Gorham's disease with chest wall involvement in Iran. By review of the literature we discussed important issues of this rare disease including clinical findings, diagnosis and treatment options. CASE PRESENTAT

Myeloma and bone disease: "the dangerous tango".

Osteolytic bone disease is the most debilitating manifestation of myeloma. However, myeloma-induced effects on the bone-active cells in the bone marrow are more than just a manifestation of disease--the myeloma derives essential support from the changed balance between bone-forming and -resorbing cells. This observation has lead to the notion that effective control of myeloma bone disease by reducing osteoclast activity and restoring osteoblast activity will contribute to long-term control of myeloma progression. Unlike osteolysis associated with other tumors that metastasize to bone, myeloma-

Multifocal Gorham-Stout disease associated with Chiari I malformation and recurrent aseptic meningitis: Case report and review of literature.

Gorham-Stout disease is a rare condition of uncertain aetiology characterised by lymphatic proliferation within osseous structures and subsequent massive osteolysis. This report describes the index case of a patient with multifocal Gorham-Stout disease involving the skull base with Chiari I malformation and recurrent aseptic meningitis without fistula. A five-year-old male presented following decompression of a Chiari I malformation with headaches, vomiting, and stiff neck and cerebrospinal fluid pleocytosis without growth of a pathogenic organism. Ongoing symptoms prompted a further three pre

A case report of Gorham-Stout disease diagnosed during the course of recurrent meningitis and cholesteatoma.

BACKGROUND: Gorham-Stout disease is a rare bone disorder. Here, we present a case of Gorham-Stout disease diagnosed during follow-up of a patient with cholesteatoma; the disease affected the temporal bone and other sites of the skull. To the best of our knowledge, this is the first report of Gorham-Stout disease diagnosed with recurrent cerebrospinal leakage after surgery to treat cholesteatoma. CASE PRESENTATION: A 25-year-old male patient re-presented to our department for the first time in 7 years with otorrhea in the right ear and recurrent meningitis. The patient had a history of multipl

Management of massive osteolysis of the mandible: a case report.

The management of a previously reported case of massive osteolysis of the mandible with intravenous bisphosphonate, vascularized free bone graft, and dental implants is reported. Restoration of adequate cosmetics and masticatory function was achieved. Despite the use of bisphosphonates, there were no complications with osseointegration. The reconstruction remained stable and functional 6 years afterward.

Gorham-Stout Disease Presenting as Acute Unilateral Proptosis.

Gorham-Stout disease (GSD) is a rare angiomatous disorder characterized by osteolysis. We report the case of a 66-year-old woman who developed sudden left eye proptosis and facial pain. Neuroimaging demonstrated a destructive mass involving the roof and lateral wall of the left orbit with adjacent abnormal orbital soft tissue. An initial biopsy of the soft tissue was nondiagnostic; however, transcranial biopsy of the orbital roof revealed an intraosseous cavernous angioma, with infiltration of orbital fat by angiomatous tissue. Despite resection of the orbital roof and lateral wall, the orbita

Gorham-Stout syndrome: a monocyte-mediated cytokine propelled disease.

UNLABELLED: We studied the biological features and the immunophenotype of a cell culture established from the lesion of soft tissues of a woman affected by Gorham-Stout syndrome. We found that these cells belonged to a monocytic lineage with some characteristics of immature osteoclasts and were able to release large amounts of osteoclastogenic and angiogenic molecules that may contribute to disease progression. INTRODUCTION: Gorham-Stout syndrome is a rare disease characterized by osteolysis and proliferation of vascular or lymphatic vessels, with a severe outcome. Its etiology and the identi

Long-term estrogen replacement therapy prevents bone loss and fractures.

Although several case-control studies have shown an inverse association between postmenopausal estrogen use and fractures, quantitation of fracture incidence has been lacking. To quantify the degree to which estrogen replacement therapy prevents postmenopausal osteoporosis, a retrospective study was done comparing the occurrence of fractures in 245 long-term estrogen users and 245 case-matched controls, followed for an average of 17.6 years. Quantitative bone mineral assessments were obtained from 18 women using estrogen replacement therapy and their controls (average age, 73 years). Osteoporo

Bone disease, hypophosphatemia and hyperparathyroidism after renal transplantation.

UNLABELLED: Long-term survivors after renal transplantation (RT) show: 1) decreased s-phosphorus 2) increased s-PTH 3) no correlation between s-PTH and s-phosphorus 4) increased amount of non-mineralized bone (osteomalacia), inversely correlated to s-phosphorus 5) decreased amount of trabecular bone (osteopenia) in RT patients with aseptic necorsis of bone. CONCLUSION: aseptic necrosis of bone after RT is mainly due to osteoid-induced osteopenia, perhaps aggravated by hypophosphatemic osteomalacia.

[The significance of ungulocoriitis septica (traumatic) in the toe of the bovine claw].

Due to the specific anatomy at the toe of the claw, inflammation of the corium at this site is characterised by a rapid spread to the distal phalanx. Radiography is important for the diagnosis and essential for determining the prognosis. In the early stage of the condition, therapy consists of correct wound care and relieving the affected claw from weight-bearing by applying an orthopaedic shoe to the partner claw. Resection of the claw toe is indicated if there is evidence of osteolysis. The postoperative healing period frequently lasts more than two months; there is a tendency to hypergranul
