INTRODUCTION: Dynamic movement in the hospital environment promotes the transmission of nosocomial pathogens and multidrug resistance mechanisms through the dissemination of organisms that carry genetic determinants. Healthcare workers play an important role in the spread of pathogens; however, the role of visitors in this environment is poorly understood.
OBJECTIVE: This study aimed to molecularly identify and examine the antibiotic resistance of the palmar microbiota of patients' companions in a hospital waiting room.
METHODOLOGY: Twenty-five palmar surface and interdigital space sample sw

BACKGROUNDS: The treatment of complex distal radius fractures, particularly those involving both intra-articular disruption and significant metaphyseal comminution, remains a considerable surgical challenge. While volar locking plates have become a widely accepted standard for stabilizing these fractures, alone they may not address all fracture patterns. This study presents a surgical approach that incorporates the use of a mini-plate to provide targeted support for the metaphyseal region. We hypothesize that combining a mini-plate for metaphyseal stabilization with a volar locking plate for o

OBJECTIVE: Distal radial fractures are common. Modern trends favour operative treatment in many instances, providing stable fixation and early functional recovery. Recent biomechanical evidence suggests that volar locking plates (VLPs) enable adequate stability for dorsally displaced fractures, both in dorsally intact (DI) and in dorsally comminuted (DC) fractures. The aim of the study was to compare the clinical outcome of these two fracture groups treated with a VLP.
METHODS: Retrospective case-control analysis of 91 distal radial fractures treated surgically using VLP by a single surgeon b

Fractures of the scaphoid waist that fail to heal tend to collapse into a flexed posture. The so-called "humpback deformity" of the bone is accompanied by collapse of the proximal carpal row with dorsal tilt of the lunate. It is possible to restore scaphoid height and carpal alignment by insertion of a tricortical wedge of graft from the iliac crest. In this article, we demonstrate the surgical technique of volar wedge grafting for scaphoid nonunion.

BACKGROUND: Injuries to the proximal interphalangeal joint (PIPJ) of the fingers are commonly treated in hand therapy departments. Conservative management for PIPJ volar plate injuries typically involves a dorsal blocking orthosis and flexion exercises. Historically hand therapists have placed the PIPJ in varying degrees of flexion but the optimal angle is unknown.
PURPOSE: To compare the outcomes of two treatment groups who received dorsal blocking orthoses: Those who the orthosis was positioned in neutral compared to those in 25-30° of flexion.
STUDY DESIGN: Retrospective cohort study.

BACKGROUND: Distal radius fractures (DRFs) are the second most common fractures, after hip fractures, seen in clinical practice. The high incidence of low-energy trauma DRFs in elderly patients raises questions about the best treatment method in terms of function, pain, and quality of life. Although the majority of these fractures are treated non-operatively with cast immobilization, valid scientific evidence of the optimal cast immobilization is lacking. In addition, several publications, including Cochrane review have outlined the need for more evidence to determine the most appropriate meth

The relationship between increased volar tilt of the distal radius and distal radioulnar joint stability was examined. Distal radioulnar joint stiffness was recorded at 10° intervals from 10° dorsal angulation to 20° of volar angulation from the anatomical position of the radius. Tests were performed with the intact radioulnar ligament and repeated after partial and then complete sectioning of the radioulnar ligament at the ulnar fovea. With the intact radioulnar ligament, distal radioulnar joint stiffness increased significantly at 10° and 20° of volar angulation. Partial sectioning of the ra

BACKGROUND: Kienböck's disease poses challenges in plate placement during radial shortening osteotomy due to steep metaphyseal inclination of the distal radius. While coronal plane analyses have been extensively studied, sagittal plane deformities remain underexplored. This study addresses the anatomical variations in the sagittal plane associated with Kienböck's disease. The primary questions include: (1) Are sagittal plane deformities more prevalent in Kienböck's disease? (2) Do these deformities influence treatment strategies or outcomes?
HYPOTHESIS: We hypothesize that anatomical variatio

Background and aim Distal radius fractures (DFRs) are among the most common orthopedic injuries, particularly in elderly and postmenopausal women due to osteoporosis and low-energy trauma, such as falls on an outstretched hand. Restoration of anatomical alignment is essential for optimal functional outcomes, as residual deformity can lead to stiffness, reduced grip strength, malunion, and early degenerative changes. With evolving treatment strategies, open reduction and internal fixation (ORIF) using volar locking plates has become a preferred modality due to its biomechanical stability and fa

There is no clear consensus about the best management of intra-articular distal ulnar fractures associated with distal radial fractures in older adults. We describe a treatment wherein the distal radial fractures were securely fixed with a palmar plate, leaving the associated ulnar fractures unfixed. The wrists of 14 patients with a mean age of 74 years were reviewed at an average of 18 months after surgery. The results were excellent in 11 cases and good in three, according to the modified Gartland and Werley score. All fracture sites displayed union, and there was no instability of the dista

Closed tendon ruptures of the thumb that require secondary reconstruction can affect the extensor pollicis longus (EPL), extensor pollicis brevis (EPB) and flexor pollicis longus (FPL) tendons. Treatment of rupture of the EPB tendon consists of refixation to the bone and temporary transfixation of the joint. In the case of preexisting or posttraumatic arthrosis, definitive arthrodesis of the thumb is the best procedure. Closed ruptures of the EPL and FPL tendons at the wrist joint cannot be treated by direct tendon suture. Rupture of the EPL tendon occurs after distal radius fractures either d

UNLABELLED: This study reports the results of minimally invasive surgical treatment of distal radius fractures using a 15mm incision with arthroscopic assistance. This was a retrospective series of 20 distal radius fractures in young or middle-aged patients caused by high-energy trauma and/or with articular involvement. The surgical procedure included two stages: fixation using a locking plate and then arthroscopy. The incision for plating was always 15mm long. Arthroscopy was used to reduce osteochondral fragments in two cases and to suture and pin six scapholunate lesions. After a mean follo

Swan neck deformity is a progressive and disabling condition that commonly affects rheumatoid arthritic hands. During a 4-year period, 101 fingers in 43 patients had this deformity corrected using a new procedure combining the distally based extensor lateral band technique described by Littler and the flexor digitorum superficialis (FDS)-palmar plate pulley introduced by Zancolli. The ranges of motion of the metacarpophalangeal, proximal interphalangeal (PIP) and distal interphalangeal (DIP) joints were assessed pre-operatively and 12 months after surgery. An average PIP joint hyperextension o

INTRODUCTION: Pronator quadratus (PQ) is an important contributor to forearm pronation, and there is concern that volar plating of a distal radius fracture (DRF) may damage the PQ function. The purpose of this study was to determine whether isokinetic pronation strength would decrease considerably after volar locking plating of a DRF, and whether clinical outcomes would be affected by any pronation strength decrease.
MATERIALS AND METHODS: Thirty-four patients of mean age of 55 years (range, 21-74 years) underwent bilateral isokinetic testing at 6 months and 1 year after open reduction and in

PURPOSE: Despite the high prevalence and impact of distal radius fractures (DRFs) on older patients, the currently available literature regarding DRFs in older adults lacks adequate comparative treatment data. The purpose of this prospective, controlled outcomes study is to compare outcomes using the volar locking plating system (VLPS) for DRFs in both older and younger adults and to assert the eligibility of older patients for surgical management with the VLPS.
METHODS: Consecutive, eligible patients were enrolled into our prospective study over a 2-year period on the basis of strict inclusi
