Assessing the prevalence of young children living in households prepared for COVID-19 in 56 low- and middle-income countries.

BACKGROUND: The COVID-19 pandemic and governments' attempts to contain it are negatively affecting young children's health and development in ways we are only beginning to understand and measure. Responses to the pandemic are driven largely by confining children and families to their homes. This study aims to assess the levels of and associated socioeconomic disparities in household preparedness for protecting young children under the age of five from being exposed to communicable diseases, such as COVID-19, in low- and middle-income countries (LMICs). METHODS: Using data from nationally repr

MediLabSecure: A One Health Network Facing the COVID-19 Pandemic.

The COVID-19 pandemic did not take us completely by surprise. The experts in public health worldwide heard the warnings and tried to alert and act. In many places, surveillance systems to monitor the emergence of new pathogens were set up. Many countries worldwide worked on prevention and preparedness for epidemic-prone infectious diseases. The rapidity of the identification of a novel human virus, and the quick sharing of its full sequence compared to past epidemics is proof of the continuous improvement of our systems worldwide. Immediately after the emergence of SARS-CoV-2, the first diffic

Progress and shortcomings in European national strategic plans for pandemic influenza.

OBJECTIVE: To repeat and update our previous evaluation (2005) of Europe's national pandemic influenza preparedness plans and assess the progress that has been made. METHODS: We assessed published national pandemic influenza preparedness plans from the European Union countries, from the two acceding countries (Bulgaria and Romania) and from Norway, Switzerland and Turkey. Plans were eligible for inclusion if formally published before 30 September 2006. We referred to WHO guidelines and used a systematically applied data extraction form. We considered plans in relation to border control measur

Vulnerability and resilience to pandemic-related stress among U.S. women pregnant at the start of the COVID-19 pandemic.

RATIONALE: Women pregnant during the COVID-19 pandemic are experiencing moderate to high levels of emotional distress, which has previously been shown to be attributable to two types of pandemic-related pregnancy stress: stress associated with feeling unprepared for birth due to the pandemic (Preparedness Stress) and stress related to fears of perinatal COVID-19 infection (Perinatal Infection Stress). OBJECTIVE: Given the well-documented harms associated with elevated prenatal stress and the critical importance of developing appropriately targeted interventions, we investigated factors predic

"Spanish flu," encephalitis lethargica, and COVID-19: Progress made, lessons learned, and directions for future research.

One hundred years ago, an influenza pandemic swept across the globe that coincided with the development of a neurological condition, named "encephalitis lethargica" for the occurrence of its main symptom, the sudden onset of sleepiness that either developed into coma or gradually receded. Between 1917 and 1920, mortality of the flu was >20 million and of encephalitis lethargica approximately 1 million. For lessons to be learned from this pandemic, it makes sense to compare it with the COVID-19 pandemic, which occurred 100 years later. Biomedical progress had enabled testing, vaccinations, and

Enhancing European governance for health crisis management: A proposal for the effective integration of primary healthcare systems into emergency preparedness and response plans.

BACKGROUND: Primary healthcare (PHC) facilities are fundamental components of health systems across Europe, serving as the first point of contact and ensuring continuity of care. However, the COVID-19 pandemic exposed significant gaps in their preparedness for large-scale health crises. Although emergency preparedness frameworks developed by the World Health Organization (WHO) and the European Union existed, they often lacked adequate integration of PHC into comprehensive public health (PH) emergency preparedness and response plans (EPRPs). OBJECTIVES: To identify regulatory and operational g

Stockpiling antiviral drugs for the next influenza pandemic.

The threat of an influenza pandemic has been at the forefront of public health preparedness for more than 5 years. The national planning effort has included stockpiling antiviral drugs in the Centers for Disease Control and Prevention's Strategic National Stockpile (SNS). This article highlights the composition of the SNS before the 2009 H1N1 pandemic and includes considerations for future antiviral stockpile purchases, focusing on emergence of oseltamivir resistance and the need for additional pediatric supplies.

Elements of Viral Outbreak Preparedness: Lessons, Strategies, and Future Directions.

Emerging and re-emerging viruses continue to pose major threats to public health. Their ability to adapt, cross species barriers, and spread rapidly can trigger severe outbreaks or even pandemics. Strengthening preparedness with comprehensive and efficient strategies is therefore essential. Here, we explore the key components of viral outbreak preparedness, including surveillance systems, diagnostic capacity, prevention and control measures, non-pharmaceutical interventions, antiviral therapeutics, and research and development. We emphasize the increasing importance of genomic surveillance, wa

Systematic exploration of predicted quaternary structures within pandemic-relevant viral proteomes.

Mechanistic understanding of viral protein-protein interactions enables global health security and pandemic preparedness, yet experimental characterisation remains difficult, costly, and often restricted to specialised laboratories. Here, we describe an in silico campaign using AlphaFold2 and AlphaFold-Multimer to predict monomers and dimer structures within 2,812 viral proteomes from 23 viral families relevant to human health. We report high-confidence predicted structures of 5,279 hetero-, and 2,749 homo-dimers. Structural clustering of interfaces reduces the set fivefold to 1,598 consolidat…

Adapting research ethics for global health crises: a systematic analysis of COVID-19 pandemic research guidelines and reflections on their post-pandemic implications.

BACKGROUND: the COVID-19 pandemic posed unprecedented challenges to global research ethics governance. In response to the urgent need for rapid scientific data, ethics review processes were accelerated, informed consent procedures adapted, and new research methods introduced. These changes were guided by the proliferation of research ethics guidelines issued by national, regional and international bodies. However, there has been limited systematic analysis of how these guidelines addressed key ethical challenges during the pandemic and shaped future ethical frameworks. This study aims to map t

Molecular Detection of Various Non-Seasonal, Zoonotic Influenza Viruses Using BioFire FilmArray and GenXpert Diagnostic Platforms.

Since 2020, the Gs/Gd H5N1 influenza virus (clade 2.3.4.4b) has established itself within wild bird populations across Asia, Europe, and the Americas, causing outbreaks in wild mammals, commercial poultry, and dairy farms. The impacts on the bird populations and the agricultural industry has been significant, requiring a One Health approach to enhanced surveillance in both humans and animals. To support pandemic preparedness efforts, we evaluated the Cepheid Xpert Xpress CoV-2/Flu/RSV plus kit and the BioFire Respiratory 2.1 Panel for their ability to detect the presence of non-seasonal, zoono…

Fifty Years of Influenza A(H3N2) Following the Pandemic of 1968.

In 2018, the world commemorated the centennial of the 1918 influenza A(H1N1) pandemic, the deadliest pandemic in recorded history; however, little mention was made of the 50th anniversary of the 1968 A(H3N2) pandemic. Although pandemic morbidity and mortality were much lower in 1968 than in 1918, influenza A(H3N2) virus infections have become the leading cause of seasonal influenza illness and death over the last 50 years, with more than twice the number of hospitalizations from A(H3N2) as from A(H1N1) during the past six seasons. We review the emergence, progression, clinical course, etiology

The WHO pandemic agreement-securing Africa's leadership in a fragmenting global order.

In May 2025, the World Health Assembly adopted the historic WHO Pandemic Agreement, aimed at strengthening global pandemic preparedness and equity. This legally binding treaty emerged from years of negotiation shaped by the COVID-19 pandemic's stark inequities-particularly those experienced by African nations. While the treaty introduces important innovations, notably the Pathogen Access and Benefit-Sharing system, significant challenges remain. Ambiguities in equity commitments, geopolitical fragmentation and rising nationalism threaten effective implementation. For Africa, realising the trea…

Preparing for Pandemics with Large Language Models: An Evaluation of Sensitivity Across COVID-19, Zika, and Monkeypox Case Reports.

Large language models (LLMs) have emerged as potential tools for early disease characterization and pandemic preparedness due to their ability to interpret complex textual data. This study evaluated the sensitivity of three LLMs: GPT-5, Claude Sonnet 4, and Gemini 2.5 Pro on early case reports of COVID-19, Mpox, and Zika. Each case report was modified to remove explicit diagnostic terms, and models were prompted to identify whether the presentation represented a disease of pandemic potential. Claude Sonnet 4 achieved the highest sensitivity overall across all three diseases. GPT-5 demonstrated…

The monkeypox virus: A public health challenge threatening Africa.

On 23 July 2022, the World Health Organization reported monkeypox cases in over 75 countries and, as a result, declared the virus a Public Health Emergency of International Concern (PHEIC). Despite Africa being the centre of the zoonotic disease evolution, its healthcare systems have not provided optimum attention to the problem. The African healthcare system is under the threat of a repeat of the situation that transpired during the COVID-19 pandemonium if stringent measures are not implemented immediately. Lessons learned from the COVID-19 pandemic should inform emergency preparedness and re
