[Cerebral paragonimiasis-a report of three cases (author's transl)].

Paragonimiasis is a benign parasitic disease caused by Paragonimus westermani and endemic in Japan, mostly in south-western area. Although the lung is the primary site of infection, ectopic involvement of the brain is not uncommon. Over 300 cases of cerebral paragonimiasis have been reported since the first case of this disease described by Ohtani in Japan in 1887. The patient with paragonimiasis in the lung and the brain were though to be decreasing because of the introduction of Bithionol treatment (Yokogawa in 1961) and public health instruction. However, about 20 cases of cerebral paragoni…

Diagnosis and surgical management of pericardial effusion due to paragonimiasis.

OBJECTIVES: The pericardial effusion (PE) caused by paragonimiasis is rarely reported. This study aims to present experience in the diagnosis and surgical management of PE due to paragonimiasis. METHODS: Medical records of 57 children who were diagnosed with PE due to paragonimiasis and underwent surgery at Children's Hospital of Chongqing Medical University between January 2012 and August 2018 were retrospectively reviewed. RESULTS: The average age of this group was 7.6 ± 3.0 years. Patients were mainly from Chongqing and Sichuan areas. ELISA for Paragonimus skrjabini in all 57 patients sho…

Intraspinal Paragonimiasis in Children: MRI Findings and Suggestions for Pathogenesis.

BACKGROUND AND PURPOSE: Intraspinal paragonimiasis is a rare entity for which imaging findings have seldom been described. The present study investigated the MR imaging features of spinal paragonimiasis, thus providing diagnostic imaging evidence and exploring the possible pathogenesis of intraspinal paragonimiasis. MATERIALS AND METHODS: The clinical and imaging findings of spinal paragonimiasis in 6 children were analyzed retrospectively. Spinal MR imaging was performed in all patients, 5 of whom also underwent enhanced MR imaging. The diagnosis was confirmed by enzyme-linked immunosorbent …

Examining human paragonimiasis as a differential diagnosis to tuberculosis in The Gambia.

OBJECTIVE: Paragonimiasis is a foodborne trematode infection of the lungs caused by Paragonimus spp., presenting clinically with similar symptoms to active tuberculosis (TB). Worldwide, an estimated 20.7 million people are infected with paragonimiasis, but relatively little epidemiological data exists for Africa. Given a recently reported case, we sought to establish whether paragonimiasis should be considered as an important differential diagnosis for human TB in The Gambia, West Africa. RESULTS: We developed a novel PCR-based diagnostic test for Paragonimus species known to be found in West…

Paragonimus and paragonimiasis in West and Central Africa: unresolved questions.

Paragonimiasis, human lung fluke disease, is a foodborne anthropozoonosis caused by the trematodes assigned to Paragonimus and is regarded by the World Health Organization as a Neglected Tropical Disease (NTD). The life cycle of this medically important parasite centres on a complex freshwater biological community that includes two intermediate hosts: a mollusc and a decapod, usually a brachyuran. Although there is a perception that the biology, symptoms, diagnosis and treatment of Paragonimus is well understood, in reality, this is not the case, especially in Africa. Much remains unknown conc…

[Effect evaluation of three ELISA kits in detection of paragonimiasis].

OBJECTIVE: To evaluate the effects of 3 kinds of ELISA reagents on the detection of human paragonimiasis. METHODS: A total of 45 serum samples from patients with paragonimiasis, 218 serum samples from patients with other parasitic diseases as well as 80 serum samples from healthy people were detected by GD-ELISA (IgG antigen ELISA detection reagent), ESELISA (using excretory/secretory (ES) products of Paragonimus westermani), and sAg-ELISA (using semi-purified antigen (sAg) of P. westermani), respectively. The effects of the 3 reagents were evaluated and compared. RESULTS: The sensitivities …

[A Retrospective Analysis on 47 Children with Paragonimiasis in Ningbo City].

A retrospective analysis was performed on 47 children diagnosed with paragonimiasis in Ningbo Women and Children’s Hospital from January 2004 to December 2014. The 47 children comprised 32 boys (68.1%) and 15 girls (48.9%), and 24 cases(51.1%) in urban areas and 23 (48.9%) in rural areas (P>0.05). There was a trend of increase in paragonimiasis occurrence in preschoolers since 2010. Forty-three cases had a history of eating raw or wine-preserved crabs and 4 cases had a history of drinking raw stream water. There were 2 cases of paragonimus encephalopathy and one case accompanied by subcutaneou…

[An Investigation on a Case of Family-clustered Paragonimiasis in Wenzhou City].

Paragonimus infection was reported in a family of 8 members after consuming wine-preserved raw crabs. Seven members developed symptoms of fever, cough, sputum, fatigueness, chest pain, and abdominal pain during 2-3 months after crab feeding, while one member was normal. Serum samples were collected from the 7 members for anti-paragonimus antibody test, with 5 members showing positive, 1 weakly positive and 1 negative. In view of the epidemiological history, clinical symptoms, imaging and laboratory test results, this case was diagnosed to be a family-clustered paragonimus infection.

Cerebral paragonimiasis: an unusual manifestation of a rare parasitic infection.

BACKGROUND: Paragonimiasis is a parasitic disease that typically produces a subacute to chronic inflammatory disease of the lung. Although rare in the United States, paragonimiasis is sporadically observed in the immigrant population. Rarely, paragonimiasis can affect the nervous system. This infection is even more unusual in the pediatric population, and therefore can be challenging to diagnose. PATIENTS: Here we present a child with cerebral paragonimiasis. She presented with new onset seizures in the setting of a febrile illness. Magnetic resonance imaging of the brain with contrast reveal…

Paragonimiasis acquired in the United States: native and nonnative species.

Paragonimiasis is a parasitic lung infection caused by lung flukes of the genus Paragonimus, with most cases reported from Asia and caused by P. westermani following consumption of raw or undercooked crustaceans. With the exception of imported P. westermani cases in immigrants, in travelers returning from areas of disease endemicity, and in clusters of acquired cases following consumption of imported Asian crabs, human paragonimiasis caused by native lung flukes is rarely described in the United States, which has only one indigenous species of lung fluke, Paragonimus kellicotti. Clinicians sho…

MRI features of pediatric cerebral paragonimiasis in the active stage.

We retrospectively reviewed the MR images of the brains of six children (age = 5-13 years) who had cerebral paragonimiasis in the early active stage. Diagnosis was based on a positive antibody test enzyme-linked immunosorbent assay (ELISA) for paragonimiasis in serum. The most common finding (in five patients) was irregular hemorrhage of various degrees. Moreover, in three cases some multiple irregular lesions with surrounding edema appeared to be conglomerated and aggregated. The rare appearance (in one patient) was a "tunnel sign," which showed the migrating track of the adult worm. In one p…

Pulmonary paragonimiasis in childhood--a cause of recurrent haemoptysis and pneumonia.

A 2-year-old Manipuri girl had haemoptysis and a chest roentgenogram disclosed diffuse-infiltrates in the right lung. Microscopic examination of sputum and morning gastric aspirate showed numerous golden-brown, operculated ova, and microscopic examination of stool specimens confirmed these ova to be those of the lung fluke, Paragonimus westermani. She responded favourably to bithionol therapy and was asymptomatic and growing normally during follow up for one and half years.
