A progressive spastic myelopathy is one of the principal manifestations of the human T cell lymphotropic virus type 1. Recent research is reviewed on the geography and epidemiology of this specific form of tropical spastic paraparesis. First recognized in the Caribbean, Colombia and Japan, it is now also confirmed as a major neurological problem in areas of eastern Brazil, western equatorial Africa, Natal and Seychelles, with other scattered foci world-wide. Accurate surveys call for sophisticated techniques including polymerase chain reaction amplification. The only defined modes of spread ar

The authors report a case of macular involvement in a patient with HTLV-I associated myelopathy (HAM). The patient was a 10-year-old girl who noticed sudden decreased vision in her right eye in November 1987. The corrected visual acuity was 0.01 in the right eye and 1.0 in the left eye. Fundus examination of the right eye disclosed mild optic disc pallor. The macula appeared to have pigmentary mottling with superficial irregular retinal reflex that was three disc diameters in size. Fluorescein angiography revealed a discoid hypofluorescent area in the macula, surrounded by mottled hyperfluores

We reported a 37-year-old woman showing HTLV-I associated myelopathy (HAM) with flutter-like oscillation of the eyes. She had a 15-year history of spastic paraplegia and noticed visual quaking at the age of 37 years. On neurological examination, she showed spasticity in both lower extremities, hyperreflexia in the extremities, neurogenic bladder, and transient, horizontal and pendular ocular oscillation. Anti HTLV-I antibody titers in serum and cerebrospinal fluid were significantly elevated. Brain CT scan revealed no particular abnormalities. In electrooculography, the oscillation consisted o

Anti-HTLV-I antibody was measured in 69 cerebrospinal fluids (CSFs) of cases with typical HTLV-I associated myelopathy (HAM) and other disorders whose symptoms were similar to HAM in order to evaluate the diagnostic significance of anti-HTLV-I antibody in CSF. Both gelatin particle agglutination (PA; Serodia-ATLA) method and recombinant gag-env hybrid protein coated ELISA were employed simultaneously. Antibody titers of both methods showed linear correlation. Cases with typical HAM (24 cases) and HAM with additional neurological manifestations (7 cases) showed high positivity in both methods.

It has been 10 years since the discovery of the human T-cell lymphotropic virus type I (HTLV-I), the first human retrovirus. During the past decade, significant progress has been made in understanding the transmission of the virus and defining its geographic distribution. It has been shown conclusively that HTLV-I is a causal factor in the induction of both adult T-cell leukemia/lymphoma and HTLV-I-associated myelopathy. However, the pathogenesis of each of these conditions is not clear, and in the light of the evidence of immune dysfunction seen among carriers of the infection, it is likely t

Admission analysis of adult non-traumatic paraplegia and paraparesis from 1981 to 1988 was carried out. Information was collected from charts of 223 consecutive admissions according to a pre-set protocol. Paraplegia or paraparesis was responsible for 164(13.4%) of all neurological admissions over the study period. The median age of the patients was 36 years with a male to female ratio of 1.7:1. Tuberculosis was the leading cause of paraplegia or paraparesis accounting for 47% of the cases. Tumours (20%), Landry-Guillain-Barre' syndrome (12.2%) and tropical spastic paraparesis (TSP) (9.8%) were

Human T-cell lymphotropic virus type 1 (HTLV-1) is a causative agent of adult T-cell leukemia and HTLV-1-associated myelopathy/tropical spastic paraparesis. HTLV-1-associated myelopathy/tropical spastic paraparesis is a chronic inflammatory disease characterized by loss of motor movement in response to spinal marrow cell destruction by T lymphocytes. To perform their cellular function, T cells need to be activated by antigen-presenting cells, such as dendritic cells (DCs). The aim of this work was to analyze DC differentiation and activation from monocytes of HTLV-1-infected individuals. We de

Human T-cell lymphotropic virus type 1 (HTLV-1) is a human retrovirus associated with a broad spectrum of clinical manifestations. While most people living with HTLV-1 remain asymptomatic, a proportion develops severe conditions such as adult T-cell leukemia/lymphoma or HTLV-1-associated myelopathy/tropical spastic paraparesis. Increasing evidence also indicates that HTLV-1 infection is related to multiple systemic complications involving different organ systems (including neurological, pulmonary, dermatological, rheumatological, and ophthalmological disorders) and opportunistic infections. Human T-cell lymphotropic virus type 1 (HTLV-1)-associated myelopathy/tropic spastic paraparesis (HAM/TSP) is an insidiously progressive spinal cord disease for which there is no effective treatment. There is great interest in developing potential biomarkers to predict the pathogenesis of HAM/TSP disease. In this study, Illumina Massive Parallel Sequencing (MPS) technology was used to investigate the cellular global noncoding RNAome expression profile in HAM/TSP patients (n = 10), asymptomatic HTLV-1-infected carriers (ASP, n = 8), and a second group of healthy controls (n = 5). OBJECTIVE: In the diagnosis of HTLV-1-associated myelopathy (HAM), while magnetic resonance imaging (MRI) is essential to exclude other diseases, its power is limited regarding HAM diagnosis, as only 30% of affected patients present with spinal cord atrophy. Diffusion tensor imaging (DTI) may enable the detection of damage in the white matter microstructure. Here, we quantitatively assess spinal cord damage using DTI and evaluate conventional MRI parameters of the spinal cord in HTLV-1-infected individuals.
METHODS: This cross-sectional study involved 33 HTLV-1 carriers, 28 patients with defi

Throughout my research life, I experienced to discover the causes of some neurological diseases in Japan. 1) SMON (subacute myelo-optico-neuropathy). Since the early 1960s, a peculiar neurological disease became prevalent throughout Japan. Through the chemical analysis of the green urine, characteristic of this disease, it was found that this disease was caused by intoxication of the administered clioquinol, an anti-diarrheal drug. This discovery is a big topic in the history of Japanese medicine. 2) In early 1970s, I experienced many young patients with oedema and polyneuropathy in Kagoshima.

The Brazilian Ministry of Health (STD and Aids Program) invited specialists to make up an informative guide to deal with HTLV patients. Among the different topics, the neurological aspects associated to HTLV were contemplated. A suspected case should include changes in force and reflexes, distal paresthesiae and autonomic dysfunction. The investigation of such case should be based on the syndrome shown by the patient. For patients with spinal cord syndrome, magnetic resonance imaging or myelography as well as spinal fluid studies should be carried out. For patients with neuropathic or myopathi

BACKGROUND: Human T-cell lymphotropic virus I (HTLV- 1) infection can lead to myelopathy/tropical spastic paresis and adult T-cell leukemia/lymphoma (ATLL). Infection with HTLV-1 has also been associated with clinically significant immunosuppression. Crusted scabies, also known as Norwegian scabies, is an uncommon presentation of scabies that may occur in conjunction with immunosuppression. Although crusted scabies has been reported in association with HTLV-1 infection, to our knowledge it has never been described in association with HTLV-1 associated myelopathy.
OBJECTIVE: The aim is to desc

Tropical spastic paraparesis (TSP) has been reported from various parts of the world for several decades. Recently reports have emerged from Japan and also countries in tropical zones associating endemic TSP with abnormally high titres of antibodies to human T-lymphotropic virus type I (HTLV-I). Data indicate that this lymphotropic retrovirus is neuropathogenic, either by direct invasion or via immunopathological mechanisms. A South African man is described who met the diagnostic criteria of TSP. Both serum and cerebrospinal fluid were antibody-positive for HTLV-I. It is possible that HTLV-I i

A spectrum of carrier states in human T lymphotropic virus type-I (HTLV-I) infection is proposed. The suspected process of clonal selection of HTLV-I infected cells results in the spectrum of nonfatal states from polyclonal to monoclonal virus integration. This idea is based on the discovery that monoclonal proviral HTLV-I DNA was detected in the fresh peripheral blood lymphocytes from some patients with HTLV-I-associated myelopathy/tropical spastic paraparesis (HAM/TSP), a nonfatal chronic neurological disorder.
