Personalised Interventions for Self-Harm in High-Risk Adolescents: A Case Report.

Self-harm in adolescents is a complex clinical challenge often driven by profound emotional dysregulation and complicated by trauma history. This report describes the case of a 14-year-old female patient with a history of sexual abuse, post-traumatic stress disorder, and major depressive disorder, who was repeatedly hospitalised for severe self-inflicted wounds. Despite multiple hospitalisations following suicide attempts and low initial motivation for psychotherapy, a multidisciplinary team implemented a modified therapeutic approach. Due to the patient's inability to participate in high-inte

JAPAN Registry of Self-harm and Suicide Attempts: Study protocol and profile in 2022-2023.

BACKGROUND: Self-harm and suicide attempts (SA) are a significant cause of emergency department visits in Japan, where over 20,000 suicides occur annually. Each suicide death is estimated to correspond to over 20 attempts, highlighting the urgent need for intervention strategies. Therefore, in 2022, the JAPAN Registry of Self-harm and Suicide Attempts (JA-RSA) was established as the first nationwide prospective registry. AIM: This study describes the design, objectives, and initial findings of JA-RSA and evaluates its potential to inform national suicide prevention policies. METHODS: JA-RSA

Pulmonary nodules and the psychological harm they can cause: A scoping review.

More than 1.6 million pulmonary nodules are diagnosed in the United States each year. Although the majority of nodules are found to be benign, nodule detection and the process of ruling out malignancy can cause patients psychological harm to varying degrees. The present study undertakes a scoping review of the literature investigating pulmonary nodule-related psychological harm as a primary or secondary outcome. Online databases were systematically searched to identify papers published through June 30, 2023, from which 19 publications were reviewed. We examined prevalence by type, measurement,

Risk factors for self-harm in children and adolescents admitted to a mental health inpatient unit.

OBJECTIVE: The aim of this study was to identify risk factors for self-harm for children and adolescents in a mental health inpatient unit. METHODS: A retrospective file audit of patient files over three years (2006-2009) was conducted to determine risk factors associated with self-harm in children and adolescents admitted to a mental health unit. A checklist of potential factors was based on risk factors found in a review of the literature including demographic information, diagnosis, home situation, environmental stressors, childhood trauma and previous mental health care. The study compare

Harm reduction readiness for illicit IV drug use among safety-net primary care practices in the San Fernando Valley.

BACKGROUND: Harm reduction is an accumulation of strategies aimed at preventing adverse health outcomes associated with illicit substance use. Several harm reduction programs and services exist within Los Angeles County (LAC), however their success relies in part on the application of harm reduction principles by local primary care providers serving patients with substance use disorders. This study aims to assess the readiness of patient-centered medical homes in the San Fernando Valley to provide effective harm reduction to patients who use injection drugs and identify barriers to doing so.

From Response to Prevention: Enhancing EMS Impact With Harm Reduction and HIV Education.

EMS are crucial not only for immediate life-saving interventions but also for broader public health initiatives, particularly in harm reduction and HIV prevention. However, many EMS training programs lack comprehensive education and training in these areas, resulting in significant gaps in patient care and provider safety. As the opioid epidemic continues to devastate communities, the need for EMS personnel to be trained in harm reduction strategies, such as naloxone administration, and HIV prevention, has become increasingly urgent. Integrating harm reduction and HIV prevention into EMS train

Harm reduction nursing and the path toward developing best practice: Lessons from caring for people with xylazine-associated wounds in Philadelphia, PA.

Xylazine-associated wounds are a distinct, novel clinical entity characterized by co-occurrence with substance use, progressive necrosis of skin, muscle, tendon, and bone, and slow healing. In Philadelphia, the specter of limb loss, stigma, and shame has hung over hospital-based care for xylazine-associated wounds among people who use drugs (PWUD) and kept many people away from engaging in care. Continued engagement in harm reduction wound care nursing, however, offers an opportunity for PWUD to address their wounds and their fears with members of the medical world. In the absence of establish

Harm reduction in severe and long-standing Anorexia Nervosa: part of the journey but not the destination-a narrative review with lived experience.

Questions remain about the best approaches to treatment for the subset of patients with severe and long-standing Anorexia Nervosa, commonly described in the literature as "Severe and Enduring Anorexia Nervosa." When discussing the optimal strategies and goals for treating this group, there is uncertainty over whether to focus on refining current treatment methods or exploring alternative approaches. One such alternative is "harm reduction," which has generated a wave of positive interest from patients and clinicians alike because of its emphasis on individual autonomy, personal goals and quali

Expert Evaluation of the Perceived Accuracy, Relevance, and Safety of Large Language Model-Generated Patient Information in Geriatrics: Cross-Condition Study.

BACKGROUND: Large language models (LLMs) are increasingly used to generate patient-oriented medical information. In geriatrics, such information must balance accuracy, relevance, and safety, as older adults may be particularly susceptible to misleading or harmful advice. However, systematic evaluations of expert perceptions across multiple geriatric conditions remain limited. OBJECTIVE: This study aimed to explore geriatricians' perceptions of the accuracy, relevance, and potential harm of LLM-generated patient information across common geriatric conditions and to examine variability and inte

Understanding the scale and nature of avoidable healthcare-associated harm for prisoners in England: protocol for a retrospective cross-sectional study.

INTRODUCTION: Around 1 in 20 patients experience avoidable healthcare-associated harm worldwide. Despite longstanding concerns, there is insufficient information available about the safety of healthcare for prisoners. To address this, this study will investigate the scale and nature of avoidable healthcare-associated harm for prisoners in England. METHODS: We will undertake a large retrospective cross-sectional study involving a case note review of patient healthcare records in 18 prisons in England. Prisons will be purposively sampled for maximum variation of characteristics based on prison

Patient safety incidents reported by patients and witnesses in prehospital emergency medical services: a multicentre prospective study.

BACKGROUND AND IMPORTANCE: Patient safety incidents - events or circumstances that could have resulted, or did result, in unnecessary harm to a patient, and referred to hereafter as incidents - fall into four categories: reportable circumstances, near misses, no-harm incidents, and harmful incidents ('adverse events'). In prehospital emergency medical services (EMS), most research has addressed only harmful incidents, leaving the other three categories poorly characterized. OBJECTIVES: The primary objective was to determine the frequency of incidents reported by patients and witnesses after p

Understanding healthcare utilisation for aboriginal people in New South Wales prisons with histories of self-harm and suicidal behaviour: a retrospective cohort study.

BACKGROUND: People in prison are at increased risk of suicide. Aboriginal people are overrepresented in Australian prisons, and their self-harm/suicide risk may be complicated by experiences of trauma, colonisation, loss of land and culture, and social injustices. This study aims to investigate mental health morbidities and in-prison service utilisation of Aboriginal people with histories of self-harm and/or suicidality. METHODS: Historical cohort study utilising Justice Health and Forensic Mental Health Network routinely collected data, including records of Aboriginal people entering NSW pub

The Association Between Hospital Harms and 1-Year Mortality Following a Hip Fracture in Ontario, Canada: A Cohort Study.

BACKGROUND: Hip fractures are a significant public health concern, associated with substantial morbidity and mortality. Mortality is the most serious consequence of hip fractures, with a 1-year rate ranging from 14% to 36%. Hospital harms are also more prevalent among older adults with hip fractures, further increasing their risk of mortality. This study aimed to assess the incremental risk of all-cause mortality up to one-year post-discharge among hip fracture patients who experienced hospital harm. METHODS: A population-based retrospective cohort study using linked health administrative dat

Comparison of Ophthalmologist and Large Language Model Chatbot Responses to Online Patient Eye Care Questions.

IMPORTANCE: Large language models (LLMs) like ChatGPT appear capable of performing a variety of tasks, including answering patient eye care questions, but have not yet been evaluated in direct comparison with ophthalmologists. It remains unclear whether LLM-generated advice is accurate, appropriate, and safe for eye patients. OBJECTIVE: To evaluate the quality of ophthalmology advice generated by an LLM chatbot in comparison with ophthalmologist-written advice. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study used deidentified data from an online medical forum, in which patient

Re-imagining poppers regulation in Canada: Perspectives from pharmacists, pharmacy students and public health practitioners.

Alkyl nitrites, commonly known as poppers, remain widely used in Canada despite regulatory prohibitions that restrict their sale as prescription-only substances. This incongruence between policy and practice has contributed to unregulated supply chains, product contamination, and missed opportunities for harm reduction - particularly among gay, bisexual, and other men who have sex with men, who use poppers at higher rates. This commentary examines the implications of reclassifying poppers as Schedule II drugs, permitting pharmacist-mediated, non-prescription access, drawing on perspectives fro

The implementation and first insights of the French-speaking Swiss programme for monitoring self-harm.

AIMS OF THE STUDY: Self-harm is a major risk factor for suicide but remains poorly documented. No data on self-harm in French-speaking Switzerland exist. To address this deficiency, the Swiss Federal Office of Public Health commissioned a specific self-harm monitoring programme. We present and discuss its implementation and first findings. METHODS: Every patient aged 18–65 years presenting for self-harm to the emergency departments of the Lausanne and Neuchâtel general hospitals were included in the monitoring programme over a 10-month period (December 2016 to September 2017).
