Rates and factors for 30-day readmission in patients with substance use disorders during the COVID-19 pandemic.

BACKGROUND: Substance use disorders (SUD) are associated with frequent hospitalizations, premature discharge, and high rates of early readmission. The COVID-19 pandemic further disrupted the continuity of addiction care, heightening the vulnerability of hospitalized patients. Identifying inpatient factors linked to reduced 30-day readmission may guide strategies to improve post-discharge outcomes in this population. METHODS: We conducted a retrospective cohort study of adults with SUD admitted to Penn State Health Hershey Medical Center between 1 January and 30 December 2021, who received add

In-Hospital Outcomes and Readmissions in High-Risk Pulmonary Embolism Patients Requiring Extracorporeal Membrane Oxygenation Support.

There are limited data on the use of extracorporeal membrane oxygenation (ECMO) in high-risk pulmonary embolism (PE) patients. We analyzed the use of ECMO in high-risk PE patients (defined as requiring vasopressors, with cardiogenic shock, or cardiac arrest) using the National Readmission Database (2016-2020) to assess the outcomes of in-hospital mortality, hospitalization costs and length of stay (LOS). Among 130,486 patients, 1,685 (1.3%) received ECMO. The ECMO cohort was on average younger (54 vs. 65 years), male, admitted to urban hospitals, and had higher rates of multiorgan failure. The

Vulnerability to functional decline is associated with noncardiovascular cause of 90-day readmission in hospitalized patients with heart failure.

BACKGROUND: Hospital readmission is common among patients with heart failure. Vulnerability to decline in physical function may increase the risk of noncardiovascular readmission for these patients, but the association between vulnerability and the cause of unplanned readmission is poorly understood, inhibiting the development of effective interventions. OBJECTIVES: We examined the association of vulnerability with the cause of readmission (cardiovascular vs. noncardiovascular) among hospitalized patients with acute decompensated heart failure. DESIGNS, SETTINGS, AND PARTICIPANTS: This prosp

Rates, predictors, and causes of readmission after transcatheter aortic valve replacement in patients with chronic kidney disease.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) is a revolutionary procedure for severe aortic stenosis. The coexistence of chronic kidney disease (CKD) and TAVR introduces a challenge that significantly impacts patient outcomes. AIM: To define readmission rates, predictors, and causes after TAVR procedure in CKD stage 1-4 patients. METHODS: We used the national readmission database 2018 and 2020 to look into readmission rates, causes and predictors after TAVR procedure in patients with CKD stage 1-4. RESULTS: Out of 24758 who underwent TAVR and had CKD, 7892 (32.4%) patients were

In-Hospital Use of Long-Acting Injectable Antipsychotics and Readmission Risk in Patients With First-Admission Schizophrenia in Taiwan.

IMPORTANCE: Long-acting injectable antipsychotics (LAIs) can help decrease the rate of nonadherence to medications in patients with schizophrenia, but these drugs are underutilized in clinical practice, especially in Asian countries. One strategy for the early prescription of LAIs is to administer the drugs during patients' first admission, when they have more time to absorb medication-related knowledge. OBJECTIVE: To estimate the prevalence of and risk factors for in-hospital use of LAIs among first-admission patients with schizophrenia in Taiwan and to examine the association of early disco

Impact of early cholecystectomy on the readmission rate in patients with acute gallstone cholangitis: a retrospective single-centre study.

BACKGROUND AND AIMS: The pathogenesis of acute cholangitis (AC) occurs with biliary obstruction followed by bacterial growth in the bile duct. The leading cause of AC is obstructing gallstones. There have been conflicting theories about the optimal timing for cholecystectomy following AC. The aim of this study is to assess the impact of early cholecystectomy on the 30-day readmission rate, 30-day mortality, 90-day readmission rate and the length of hospital stay. METHODS: This retrospective study was performed between January 2015 and January 2021 in a high-volume tertiary referral teaching h

Home health care after discharge is associated with lower readmission rates for patients with acute myocardial infarction.

OBJECTIVE: We studied the utilization of home health care (HHC) among acute myocardial infarction (AMI) patients, impact of HHC on and predictors of 30-day readmission. METHODS: We queried the National Readmission Database (NRD) from 2012 to 2014identify patients with AMI discharged home with (HHC+) and without HHC (HHC-). Linkage provided in the data identified patients who had 30-day readmission, our primary end-point. The probability for each patient to receive HHC was calculated by a multivariable logistic regression. Average treatment of treated weights were derived from propensity score
