Mesenteric Inflammatory Myofibroblastic Tumor in an Infant: A Case Report and Literature Review.

Inflammatory myofibroblastic tumor (IMT) is a rare mesenchymal neoplasm predominantly affecting children and young adults. Here, we present a case of IMT incidentally discovered in the mesentery of a six-month-old infant, managed through radical surgery. The patient initially presented with decreased oral intake and post-feeding vomiting, leading to the discovery of a large pelvic mass. Following comprehensive evaluation and multidisciplinary management, including surgical excision, the patient experienced a favorable clinical outcome. Histopathological examination confirmed the diagnosis of I

Primary seminal vesicle epithelioid smooth muscle neoplasm of uncertain biologic potential.

We present the case of a 73-year-old male patient who presented with obstructive urinary symptoms, pelvic pressure, and hematuria. CT imaging revealed a heterogenous prostate enlargement, and MRI demonstrated the mass to be arising from the seminal vesicle. Prostate biopsies showed benign tissue. Surgical excision was completed and pathology revealed it to be an epithelioid smooth muscle neoplasm of uncertain biologic potential. This is only the second known case of such a seminal vesicle tumour. As soft tissue sarcomas of the seminal vesicle emerge in the literature, we may develop a better u

Primary adenocarcinoma of the fallopian tube as an incidental finding during surgery in a postmenopausal patient with unspecific pelvic pain.

Primary fallopian tube carcinoma (PFTC) is a rare gynecologic neoplasm and is usually diagnosed late and presents classically with a characteristic group of symptoms. We describe a case of a 76-year-old woman who underwent TVS requested by the family physician due to unspecific pelvic pain. An adnexal mass was found with morphology associated with high levels of CA125 suggestive of a malignant tumor. During laparotomy, a mass located in the left tube was found. Histopathology confirmed PFTC. Total hysterectomy, salpingo-oophorectomy and adjuvant chemotherapy with carboplatin/paclitaxel were pe

Thymic carcinoma of the thymic hormone secretory type in a cow.

An 8-year-old Holstein cow had tumor nodules and enlarged lymph nodes in the mediastinum, and metastatic tumor masses in the pelvic cavity. The neoplastic cells were characterized by squamous features and intracytoplasmic vacuoles carrying microvilli, some of which contained periodic acid Schiff-positive globular cores, but tubular structures or goblet cells were absent. Many neoplastic cells stained positively for keratin, and occasional cells were positive for thymosin. The presence of secretory granules in the cytoplasm was confirmed by electron microscopy. This neoplasm was considered to b

A rare case of a concomitant ovarian fibroma and malignant steroid cell tumor: insights into pathogenesis and steroidogenesis.

BACKGROUND: Fibromas are common ovarian stromal tumors, while steroid cell tumors (SCTs) are rare, accounting for < 0.1% of ovarian neoplasms. Approximately, one-third of SCTs exhibit malignant behavior, but predicting malignancy remains challenging. CASE PRESENTATION: A 73-year-old woman presented with nonspecific pelvic pain, and imaging revealed multiple pelvic masses. She underwent a simple hysterectomy and bilateral adnexectomy. Pathological examination revealed a unique colocalization of a fibroma and a SCT in the right ovary. One year later, the SCT recurred with lymph node metastasis.

Disseminated Peritoneal Leiomyomatosis Status Post Laparoscopic Hysterectomy with Morcellation.

Uterine leiomyomas (fibroids) are common benign neoplasms, which develop from the muscular tissue of the uterus with an estimated incidence of 20-40% in women of reproductive age. In the early nineties, power morcellators were introduced and became commonly used during hysterectomy for symptomatic fibroids. However, if all fragments are not removed, they may parasitize to other blood supply and present as abdominal or pelvic masses. Unfortunate cases have also been reported in which uterine sarcomas seeded throughout the abdomen and pelvis secondary to morcellation. The Food and Drug Administr

Possible role of the long dorsal sacroiliac ligament in women with peripartum pelvic pain.

BACKGROUND: To enhance the understanding of the pathophysiology of women with peripartum pelvic pain, it is necessary to couple anatomical insights with relevant clinical research. In this context, the long dorsal sacroiliac ligament is especially of interest because it was noticed that women diagnosed with peripartum pelvic pain frequently experience pain within the boundaries of this ligament. Njoo (1) found a high intertester reliability and a high specificity for long dorsal sacroiliac ligament pain. The present article focuses on the possible role of the long dorsal sacroiliac ligament in

The Great Mimicker: Variable Presentations of Xanthogranulomatous Salpingo-Oophoritis in a Case Series of Seven Patients From a Rural Tertiary Care Center.

Xanthogranulomatous salpingo-oophoritis (XGSO) is an uncommon, non-neoplastic chronic destructive inflammatory condition. Its clinical and radiological features frequently masquerade as advanced ovarian malignancy or refractory pelvic inflammatory disease, posing a notable diagnostic challenge. Histopathological examination remains the gold standard for diagnosis. A retrospective clinicopathological evaluation of seven distinct cases of xanthogranulomatous adnexal inflammation was performed. Clinical symptoms, diagnostic imaging, intraoperative details, surgical approaches, and gross and micro

Lost in the ligament: a diagnostic odyssey of a massive broad ligament fibroid.

True broad ligament leiomyomas are rare, accounting for less than 1% of all fibroids, and pose significant diagnostic and surgical challenges due to their atypical location and proximity to vital pelvic structures. We report a case of a 3.4 kg true broad ligament leiomyoma presenting as an abdominopelvic mass, initially suspected to be an ovarian neoplasm. Preoperative imaging was inconclusive, and a definitive diagnosis was made intraoperatively. Surgical excision required meticulous dissection to avoid injury to the ureters and iliac vessels. This case highlights the importance of differenti

Radiological findings in pelvic solitary fibrous tumour.

Solitary fibrous tumour (SFT) is an uncommon, usually benign mesenchymal neoplasm. SFT was first described in the pleura, but has subsequently been reported to occur in numerous anatomic locations including the abdomen and pelvis. Abdominopelvic SFTs are typically an indolent process, in spite of reaching a large size by the time of diagnosis. The preferred treatment is complete resection followed by extended follow-up surveillance. The risk of local recurrence and metastasis correlates with tumour size and the histological status of surgical margins. We present the imaging findings of a large

Clear cell adenocarcinoma of a female urethra: A case report and review of the literature.

CONTEXT: Clear cell adenocarcinoma of the urethra is an extremely rare tumour. Its histogenetic derivation remains controversial. CASE REPORT: We report a new case of clear cell adenocarcinoma of the proximal urethra in a 56-year-old woman who presented with grossly hematuria. Urethral cystoscopy revealed a tumour protruding from the posterior urethral wall at the bladder neck. Treatment consisted of urethrocystectomy with pelvic lymph node dissection. Histologically, the neoplasm consisted of clear cell adenocarcinoma of the urethra. CONCLUSION: It appears that female urethral adenocarcinom

[One case report of clitoral priapism and literature review].

OBJECTIVE: To investigate the clinical characteristics of female clitoral priapism. METHODS: A 29-year-old case of painful priapism of the clitoris lasting 60 hours was reported. The etiology, diagnosis and treatment of clitoral priapism were discussed and the literature reviewed. RESULTS: The patient was cured by intracavernous administration of adrenaline. CONCLUSION: Clitoral priapism is extraordinarily rare. It is primarily due to oral antidepressant drugs or pelvic malignant neoplasm which leads to tissue infiltration of the clitoral veins and obstruction of the clitoral corporeal bloo

Ovarian mucinous borderline tumor with anaplastic carcinomatous nodules in adolescents.

Bilateral ovarian epithelial neoplasms in adolescents are rare. Moreover, borderline mucinous neoplasms with local intraepithelial carcinoma with anaplastic carcinoma are even more infrequent. Herein, we presented a single case (a 17-year-old female) with regular menstrual cycles and stomach pain when eating who was diagnosed with a left ovarian tumor accompanied by mural nodules. The right ovarian cyst, the left ovary, and the fallopian tube were removed by surgery. Intraoperative diagnosis suggested a bilateral ovarian tumor with mural nodules, which include three different pathological type

Endometrial Stromal Sarcoma Arising in Colorectal Endometriosis.

The malignant transformation of endometriosis is very uncommon. Whereas 75% of tumors arising from endometriosis arise in the ovary, location in extra-genital organs is rare and mesenchymal neoplasms are exceptional. A 47 year-old woman who underwent hysterectomy with bilateral salpingo-ooforectomy due to endometriosis 13 years before presented with abdominal pain. The magnetic resonance imaging (MRI) showed a 9.7×7.5 cm solid-cystic supravesical mass and a recto-vaginal tumor, as well as endometriotic nodules in the sigma, right parametrium and peritoneum that had significantly increased in s

[Intraoperative frozen sections in diseases of the female genital tract].

Intraoperative frozen sections are particularly important for ovarian tumors because definitive preoperative histology is not possible. The diagnostic accuracy of frozen sections is highest for primary invasive ovarian carcinomas and benign ovarian lesions, followed by borderline tumors and poorest for ovarian metastases and rare neoplasms, such as germ cell tumors. Endometrial carcinoma should be diagnosed preoperatively by curettage or biopsy. For endometrioid endometrial carcinomas the indications for lymphadenectomy are often based on intraoperative assessment of the uterus. The differenti

Secondary ovarian neoplasms in children: imaging features with histopathologic correlation.

BACKGROUND: Although the pathologic features and imaging appearance of childhood primary ovarian neoplasms have been well described, little information is available about the malignancies that may secondarily involve the ovary. OBJECTIVE: To determine the relationship between the imaging features and the histopathology of secondary ovarian neoplasms in children treated at our institution. MATERIALS AND METHODS: We searched our institutional database for codes indicating metastatic ovarian disease. Of the 35 patients with such codes, 18 had pathologically proven secondary ovarian disease. Fro

Oncovascular Resection in Pelvic Exenteration: A Systematic Review.

BACKGROUND: Resection of major vascular structures may be required to achieve R0 resection in pelvic exenteration surgery for advanced pelvic malignancy. The evidence base for pelvic oncovascular techniques is limited, with patient outcomes following these procedures not widely reported. OBJECTIVES: This systematic review synthesises the current literature on R0 resection and 30-day mortality rates following oncovascular pelvic exenteration. METHODS: A systematic review was conducted according to a pre-defined protocol (PROSPERO ID CRD42024613055). MEDLINE, EMBASE, Cochrane Library and Web o

Navigating pelvic anatomy for exenteration: a clinical guide for radiologists.

Pelvic exenteration (PE) is a complex, potentially curative option for locally advanced or recurrent pelvic malignancies in which achieving an R0 margin is paramount. This clinical review provides a pragmatic, anatomy-driven framework to optimize preoperative assessment and multidisciplinary planning. Using a compartment-based schema-central, anterior, posterior, and lateral-the article maps tumor spread to structures that determine technical feasibility and functional outcomes. For each compartment, surgical implications and reporting "pearls and pitfalls" are summarized to ensure radiologica

Pubic Bone Excision and Perineal Urethrectomy for Radical Anterior Compartment Excision During Pelvic Exenteration.

BACKGROUND: Malignant infiltration of the pubic bone traditionally is considered inoperable. Consequently, there is little published on surgical approaches to resection of the anterior pelvic bone. En bloc partial or complete pubic bone excision can be performed depending on the degree of involvement. OBJECTIVE: This article describes our surgical approach of pelvic exenteration with en bloc composite pubic bone excision. DESIGN: The surgical technique describes 2 distinct aspects of the surgery, first, a perineal as opposed to abdominal transection of the urethra, and, second, varying exten

Aggressive Angiomyxoma of the Lower Female Genital Tract: A Review of the MITO Rare Tumors Group.

Aggressive angiomyxoma (AAM) is a rare, locally aggressive, myxoid mesenchymal neoplasm primarily found in the pelvic and perineal regions of young adult females. It is a slow growing and locally infiltrating tumor. Preoperative diagnosis is difficult due to the rarity of these tumors and absence of characteristic signs and symptoms. The primary management is tumor excision. Incomplete excision is common because of the infiltrating nature of the neoplasm and absence of a definite capsule. Other non- surgical modalities have been employed, such as radiotherapy, embolization, GnRH analogues or o
