Association between Cone-Beam Computed Tomography and Histological and Immunohistochemical Features in Periapical Lesions Correlated with Thickened Maxillary Sinus Mucosa.

Background and Objectives: Odontogenic sinusitis is a frequently underestimated pathology with fewer symptoms in patients with periapical lesions, periodontal disease, or iatrogenic foreign bodies in the maxillary sinus. The aim of our study was to determine the correlation between maxillary sinusitis and periapical lesions using cone-beam computed tomography (CBCT) imaging and histological and immunohistochemical investigations. Materials and Methods: A total of 1450 initial patients diagnosed with maxillary sinusitis in the Ear-Nose-Throat (ENT) Department, University of Medicine and Pharmac

10-year follow-up of calcifying odontogenic cyst in the periapical region of vital maxillary central incisor.

INTRODUCTION: Radiographic images may lead to misinterpretations of lesions of endodontic and nonendodontic origin. This report describes a case of a 10-year follow-up of a calcifying odontogenic cyst (COC) in the periapical region of a vital maxillary central incisor in a 9-year-old boy. METHODS: The patient revealed a history of a swelling in the periapical area of tooth #9. The patient denied any dental trauma or history of pain. Clinical examination revealed no mobility, but there was discrete discomfort when horizontal pressure was applied. Pulp vitality was present in all maxillary ante

Histologic evaluation of periradicular tissues in dogs treated with calcium hydroxide in combination with HCT20 and camphorated P-chlorophenol.

OBJECTIVE: The aim of this study was to evaluate the biocompatibility of calcium hydroxide suspended in HTC20 or calcium hydroxide plus CMCP and their effects on the healing of periapical lesions in dogs. STUDY DESIGN: Experimental apical periodontitis was induced by opening the pulp chamber of 36 mandibular premolars in 6 3-year old dogs. The teeth were left opened for 1 week and then closed with IRM for 60 days in order to induce periapical lesions. The teeth were divided into 1 control and 2 experimental groups. Both experimental groups received intracanal dressings with a calcium hydroxid

Treatment of a human transosseous mandibular defect by guided bone regeneration.

The treatment of through-and-through human periapical lesions by barrier membrane techniques has not been reported in the periodontal literature. However, periodontists frequently perform this treatment. The authors present a case that demonstrates radiopacity after regeneration surgery of a human periapical transosseous mandibular defect, suggesting local bone regeneration. The surgical technique performed in this case was based on the principles of guided bone regeneration using a demineralized freeze-dried bone membrane (DFDBM). The bone membrane acted as an efficient barrier that excluded

Periapical changes following root-canal treatment observed 20-27 years postoperatively.

AIM: The aim of the present study was to identify periapical changes 20-27 years after root-canal treatment. METHODOLOGY: The periapical condition of 265 roots filled by undergraduate students was evaluated in two series of intraoral radiographs taken 10-17 and 20-27 years after treatment. Roots (72) not recorded with a normal periapical situation on both occasions by two observers, were re-evaluated by other two examiners, separately and jointly. Final decisions about diagnoses were made by all four examiners. A strict definition was used for the identification of cases with an unfavourable

Tertiary Lymphoid Structures Are Related to Inflammatory Progression and Bone Loss in Human Apical Periodontitis.

INTRODUCTION: Bone loss is strongly associated with the immunologic milieu in apical periodontitis (AP). Tertiary lymphoid structures (TLSs) are organized lymphoid cell aggregates that form in nonlymphoid tissues under persistent inflammatory circumstances. To date, there has been no relevant report of TLSs in periapical lesions. This work aimed to investigate the formation and potential function of TLSs in AP. METHODS: Tissues from human apical lesions (n = 61) and healthy oral mucosa (n = 5) were collected. Immunohistochemistry and multiplex immunofluorescence were used to detect the format

Interleukin 1-beta in symptomatic and asymptomatic human periradicular lesions.

Interleukin 1 beta (IL-1 beta) has been shown to be a potent mediator of bone resorption and has been implicated in resorption in periradicular lesions. In this study we compared the local concentrations of IL-1 beta in symptomatic and asymptomatic human periradicular lesions. Periradicular tissues from patients with symptomatic and asymptomatic lesions were obtained and stored at -70 degrees C. Pulpal tissues from unerupted third molars and chronically inflamed gingival tissues were also obtained and used as negative and positive controls, respectively. Tissue samples were homogenized, and su

Non-surgical Management of a C-shaped Mandibular Second Molar with Apical Periodontitis and External Root Resorption: A Case Report.

This case report describes the successful nonsurgical endodontic management of a mandibular second molar with a confirmed C-shaped canal configuration complicated by chronic apical periodontitis and active external inflammatory root resorption in a 16-year-old female patient. The patient presented with pain on biting, deep localized periodontal probing, and radiographic evidence of a large periapical radiolucency and resorptive changes in the distal root. Diagnostic tests confirmed pulpal necrosis and an endodontic-periodontal communication. Following access under magnification, the complex C-

[Justification and optimization of intraoral and panoramic radiographs in oral care].

The new guideline of the Dutch Society for Oral Health Care Guidelines (KIMO) Indication for intraoral and panoramic radiographs offers oral care providers a practical framework for the targeted use of intraoral and panoramic radiographs in screening, diagnosis, treatment and follow-up. It specifies, among other things, when and how often bitewings are indicated based on caries risk; which radiographs (horizontal/vertical bitewings, periapical) are appropriate in periodontal destruction, and how to use periapical images in endodontic treatment, including 1-year follow-up. Regarding implantolog

Management of perforating internal root resorption with periodontal surgery and mineral trioxide aggregate: a case report with 5-year follow-up.

Internal root resorption (IRR) is characterized by progressive loss of tooth substance initiating at the root canal wall as a result of clastic activity. The use of periodontal surgery and mineral trioxide aggregate is a good approach to repair lesions with periodontal communication (perforating IRR). This case describes the treatment and follow-up of a maxillary central incisor with perforating IRR in a 56-year-old male patient where root canal treatment, periodontal surgery, and white mineral trioxide aggregate were employed to achieve complete repair and to restore function. Squamous cell carcinoma presenting as an endodontic-periodontic lesion.

BACKGROUND: Regardless of advances in diagnosis and treatment during the past 40 years, the overall 5-year survival rates for oral and oropharyngeal squamous cancers have only slightly improved and remain around 50%. Thus, the early diagnosis and treatment of carcinoma by health care providers are essential in achieving a good prognosis. We report a case of invasive squamous cell carcinoma that presented as a benign endodontic-periodontic lesion with a 7-mm periodontal pocket on tooth #15 in a 40-year-old, non-smoking woman. The subsequent management of the case is also discussed. The study wa

Treatment of osseous defects with fibroblast-coated hydroxylapatite particles.

Several techniques of periodontal regeneration have been used, including hydroxylapatite (HA) grafting, demineralized free-dried bone allografts and guided tissue membranes, for which clinical merits still need further scientific study. The purpose of this pilot study was to examine the healing of periodontal defects using HA grafts coated with fibroblasts isolated from the periodontal ligament (PDL). We cultured fibroblast-like cells either from a clinically healthy site of the PDL or gingival tissues of the subject receiving the cell transplantation. We added HA to the cultures and allowed t

Radiographic assessment of periodontitis in African-Americans with Down syndrome.

OBJECTIVE: Down syndrome (DS) is a neurodevelopmental genetic disorder affecting millions of Americans. Previous studies, mainly in Caucasians, have shown periodontitis to be a major oral health problem among DS patients. Other research has shown that African-Americans (AA) are more susceptible to periodontitis than Caucasians. Our objective was to determine if DS negatively affects the already impaired periodontal status of AAs. METHODS: Radiographic alveolar bone levels were used to determine periodontal status. Full-mouth series of intraoral periapical (PA) radiographs of 25 AA DS patients

Patterns of keratin-expression in rests of Malassez and periapical lesions.

Using immunocytochemistry and a panel of monoclonal antibodies directed against various keratin polypeptides we examined specimens of normal periodontal ligament, periapical granulomas and inflammatory dental cysts. Epithelial elements with the appearance of rests of Malassez were identified in 6 specimens of normal periodontium and 10 periapical granulomas. Altered epithelium was present in 16 periapical granulomas and a lining epithelium in 10 inflammatory dental cysts. The patterns of binding of antibodies by these epithelia indicated that (a) keratin 19 was expressed by all epithelia, and
