A histological comparison of periapical inflammatory and neural responses to two endodontic sealers in the ferret.

Previous studies have shown that there is a proliferation of nerves beneath the apices of pulpectomized teeth. This may be due to the inflammation induced after the procedure, resulting, perhaps, from the irritant nature of the materials used to fill the root canal. The experiment reported here was conducted to determine whether this inflammation was induced by the sealer rather than arising as a result of tissue damage and whether, if inflammation is eliminated or reduced, the neural changes are also reduced. In 12 young adult ferrets under general anaesthesia the pulps of the lower canine te

The heterogeneity of T cell infiltration in human periapical lesions.

OBJECTIVES: T lymphocytes (T cells) are crucial important types of immune cells which are responsible for adaptive immune response during the progression of bacterial infectious diseases. Different subtypes of T cells can participate in regulating the progress of human periapical lesions. This study investigates the co-localization of different types of T cells by multiplex immunofluorescence staining (mIHC) from different types of human periapical lesions (PLs) which might elucidate the potential clinical relevance of T cell infiltration with PLs. MATERIALS AND METHODS: 20 PLs tissues and 6

ZEB1 expression in Th17 cells correlated with p-STAT3 in human apical periodontitis.

BACKGROUND: ZEB1, a zinc-finger E homeobox-binding transcription factor most frequently associated with developmental programs linked to epithelial-mesenchymal transition, has been demonstrated to regulate immune cell function. The study aimed to investigate the expression pattern of ZEB1 in Th17 cells and its colocalization with p-STAT3 in human apical periodontitis lesions. METHODS: Thirty-nine human periapical tissues were collected for ex vivo study, including periapical granulomas (PGs, n = 14), radicular cysts (RCs, n = 12), and healthy control tissues (control group, n = 13). Inflammat…

Portland cement use in dental root perforations: a long term followup.

Root canal and furcal perforations are causes of endodontic therapy failure and different materials that stimulate tissue mineralization have been proposed for perforation treatment. In the first case, a patient presented tooth 46 with unsatisfactory endodontic treatment and a periapical radiographic lesion. A radiolucent area compatible with a perforating internal resorption cavity was found in the mesial root. The granulation tissue was removed, and root canals were prepared. The intracanal medication was composed of calcium hydroxide and the perforation cavity was filled with Portland cemen

Oral green and red propolis attenuate bone resorption and inflammation in experimental apical periodontitis.

OBJECTIVE: To test whether systemic green or red propolis modulates inflammation and bone resorption in rat apical periodontitis (AP). DESIGN: Twenty-four male Wistar rats received AP induction in the first mandibular molars and were randomized to Control, Green propolis, or Red propolis (n = 8/group). Propolis (100 mg/kg in water) or vehicle was administered daily by gavage for 30 days. On day 30, periapical tissues were evaluated by micro-CT, histology (inflammatory score), and immunohistochemistry for RANKL, OPG, and TRAP-positive osteoclasts. Data were analyzed with Shapiro-Wilk, Kruskal-…

The presence of autophagy in human periapical lesions.

INTRODUCTION: Autophagy, a lysosome- or endosome-mediated self-degradation process, participates in diverse neurodegenerative diseases, cancer, and inflammatory diseases associated with apoptosis. This study aims to identify the presence of autophagy in human periapical lesions and its possible colocalization with apoptosis. METHODS: Forty-seven samples of human periapical lesions diagnosed as radicular cysts (RCs, n = 23) or periapical granulomas (PG, n = 24) were examined. The inflammatory infiltrates of lesions were evaluated by hematoxylin-eosin staining, and the presence of autophagy was

Periapical actinomycosis. An unusual case report.

An unusual case of actinomycosis in the oral cavity is reported. The strict anaerobic qualities of Actinomyces israelii make laboratory culturing and growing of this organism difficult. Accordingly, the diagnosis was based on the histopathologic report. Because of the atypical clinical presentation, i.e., lack of any discharging sinus tract on the skin surface, and the histopathologic reports, it was postulated that the actinomycosis was superimposed on an existing granulomatous tissue.

Management of radicular cysts using platelet-rich fibrin and bioactive glass: a report of two cases.

Platelet-rich fibrin (PRF) created by Choukroun's protocol concentrates most platelets and leukocytes from a blood harvest into a single autologous fibrin biomaterial. However, no current data is available concerning the use of PRF for the treatment of periapical lesions. Two cases of radicular cysts were reported using an interdisciplinary approach, including regular endodontic therapy followed by surgical management with PRF and bioactive glass. Two cases of radicular cysts presented as an incidental radiographic finding, appearing as an apical radiolucency with well-circumscribed sclerotic

CCR2 deficiency results in increased osteolysis in experimental periapical lesions in mice.

INTRODUCTION: Periapical lesions are chronic inflammatory disorders of periradicular tissues caused by etiologic agents of endodontic origin. The inflammatory chemokines are thought to be involved in the latter observed osteolysis. With a murine model of experimental periapical lesion, the objective of this study was to evaluate the role of the chemokine receptor CCR2 in the lesion progression, osteoclast differentiation and activation, and expression of inflammatory osteolysis-related mediators. METHODS: For lesion induction, right mandibular first molars were opened surgically with a 1/4 ca

The immune landscape in apical periodontitis: From mechanism to therapy.

Apical periodontitis (AP) is featured by a persistent inflammatory response and alveolar bone resorption initiated by microorganisms, posing … nt, ultimately impacting bone homeostasis. The destructive effect of excessive immune activation on periapical tissues is emphasized. This review aimed to systematically discuss the immunological basis of AP, the inflammatory bone resorption and the immune cell network in AP, thereby providing insights into potential immunotherapeutic strategies such as targeted th …

Massive extrusion of calcium hydroxide paste containing barium sulphate during endodontic treatment.

A 31-year-old woman was referred for the evaluation of persistent lower lip numbness following endodontic treatment of tooth #36. Imaging examinations showed a large amount of radiopaque/hyperdense material spread in an angiographic distribution in the left mandibular body region. Laboratory analyses of tooth #36 and adjacent periapical tissue, surgically extracted in an external Service due to acute pain following endodontic treatment, identified chronic inflammatory reaction and birefringent crystalloid foreign bodies rich in barium and sulphur, leading to the diagnosis of alveolar nerve inj…

Management of a Persistent Radicular Cyst in the Maxillary Right Lateral Incisor: A Case Report.

A 32-year-old male patient presented with a chief complaint of ongoing endodontic treatment and swelling in the hard palate, specifically in the rugae region. On examination, a soft, non-tender, non-fluctuant swelling was observed between teeth #11 and #12, accompanied by radiographic evidence of periapical radiolucency and perforation. Cone beam computed tomography (CBCT) scans confirmed a well-defined radiolucency in the affected region, indicative of a radicular cyst. Root canal treatment was completed with mineral trioxide aggregate (MTA) obturation during the second visit. An apicoectomy

Maturogenesis of non-vital immature permanent teeth.

Pulpal necrosis in young permanent teeth often results in teeth with open apex, thin root walls and poor crown root ratio. Out of the available treatment options maturogenesis has been the most conservative option that exploits full potential of pulp for dentin deposition. Maturogenesis involves disinfecting the root canal system followed by stimulation of blood clot from the periapical tissue, which provides a matrix into which the cell could grow and sealing the coronal excess. In the present case report, tri antibacterial paste (3 Mix) was used as an intracanal medicament that proved succes…

Stafne bone cavity in the anterior mandible.

Lingual mandibular bone defect, also known as Stafne bone cavity, is mostly seen in the posterior portion of the mandible. Inclusions in the anterior portion are very unusual, with around 50 cases reported in the English-language literature. They are often asymptomatic conditions diagnosed during a routine radiographic examination. This article describes a case of anterior Stafne bone cavity in a 52-year-old man mistaken for periapical pathologic defect and referred for treatment. In addition to clinical examination, the patient was imaged using conventional periapical, panoramic radiography,

Peri-implant hard tissue response to glow-discharged abutments: prospective study. Preliminary radiological results.

In the literature has been demonstrated that when an implant is placed in the oral environment, changes in peri-implant hard tissue level may occur. Several clinical procedures have been advocated to clinically minimize this bone resorption. However, minimal attention was paid to the soft- and hard-tissue abutment interaction. The present clinical preliminary study is intended to radiologically analyze the effect of glow-discharged abutments on hard tissue level changes after 18 months of prosthetic loading. Five patients needing an implant supported restoration in the anterior maxillary area

Endodontic applications of guided tissue regeneration in endodontic surgery.

This paper is intended to provide an overview of different endodontic applications in guided tissue regeneration. It is our expectation that using these techniques, where applicable, will increase endodontic surgical success. We have described six different clinical conditions including external resorption, root perforation, dehiscence, palatal grooves, oblique root fractures, and large periapical lesions. We believe the use of guided tissue regeneration techniques will allow us to predictably treat teeth that otherwise might be extracted.
