Endolymphatic pressure in experimental hydrops.

Perilymphatic and endolymphatic pressures were consecutively measured by means of a servomechanical nulling pressure system through the round window and basilar membrane one to 2.5 months after obliteration of the left endolymphatic sac and duct in seven guinea pigs. The position of the pipette was monitored by the simultaneous recording of the direct current potential through the micropipette used for the pressure measurement. (1) The perilymphatic and endolymphatic pressures of the control ears were equal. (2) The histological evaluation showed that the ears operated on had a significantly g

Effects of combined rupture of Reissner's membrane and the round window on hearing in the guinea pig.

Hearing impairment caused by sequential rupture of Reissner's membrane and the round window was investigated in guinea pigs to determine if the double-membrane break can cause acute profound sensorineural hearing loss. Reissner's membrane was lacerated through the stria vascularis at one of the turns of the cochlea, followed by rupture of the round window to create leakage of the perilymph. Action potentials (APs) to tone pip stimuli were recorded from the Fallopian canal electrode before and 90 min after these procedures. The results showed that the ears with the double-membrane break at the

Whiplash, postural control, and the inner ear.

Many patients with "whiplash syndrome" experience unrelenting neck stiffness and pain. This abnormal muscular tension is postulated to be causally related to a central disorder of postural control, which has evolved secondary to injury of the inner ear labyrinthine structures. Moving platform posturography was used to demonstrate the presence or absence of a static or dynamic equilibrium disorder in 48 patients who had experienced the oscillation forces induced by a rear-end automobile collision. Other vestibular tests were used to document dysfunction of the semicircular canals and the otolit

Recovery and probable persistence of cytomegalovirus in human inner ear fluid without cochlear damage.

Cytomegalovirus (CMV) was recovered from a 5-month-old infant with probable congenital infection. In life, no hearing impairment had been observed. Auditory brain stem evoked responses were bilaterally intact. At necropsy, both temporal bones were morphologically normal, as demonstrated by light and electron microscopy. Sensory hair cells of the organ of Corti appeared intact. Cytomegalovirus was recovered from a mixture of perilymph and endolymph, but not the brain, CSF, or vitreous humor. This appears to be the first report of an individual with an inner ear CMV infection in which neither st

Otosclerosis surgery.

Results of otosclerosis surgery using the stapes footplate removal with various types of reconstruction are reported in 456 ears (360 patients). Primary success was obtained in around 95% while 83% retained their improvement over the years. The posterior crus on fascia has become the preferred method because of its stability and relative lack of inner ear complications. Inner ear damage is mostly due to perilymph fistulae, which should be promptly recognized and repaired, since revision after the ear has become deaf does not restore hearing. Some late inner ear losses may be due to reactions t

Characteristic findings in the human fetus vestibule: A human temporal bone study.

OBJECTIVE: The "collapse," a highly flexed, dented, or caved membrane between the endo- and peri-lymph of the saccule and utricle in adults, is considered as a morphological aspect of Ménière's syndrome. Likewise, when mesh-like tissues in the perilymphatic space are damaged or lost, the endothelium loses mechanical support and causes nerve irritation. However, these morphologies were not examined in fetuses. METHODS: By using histological sections from 25 human fetuses (crown-rump length[CRL] 82-372 mm; approximately 12-40 weeks), morphologies of the perilymphatic-endolymphatic border membra

[Stapes dislocation and opening in the inner ear due to tympanoplasty].

During tympanoplasty, the stapes is often exposed to considerable manipulations, i.e. the dissection of cholesteatoma matrix from the oval window niche or the insertion of a prosthesis for ossicular chain reconstruction. Too much strain may rupture the annular ligament resulting in an opening of the inner ear. Furthermore it is conceivable that microperilymphatic fistulas, which remain undetected, may develop prior to a visible luxation of the stapes. Therefore, temporal bone experiments with definite manipulations on the stapes were performed to study the rupturing strength of the annular lig

Inner ear fluids dynamics and endolymphatic hydrops.

The inner ear possesses remarkably stable homeostatic mechanisms for the maintenance of the functional integrity of the inner ear end-organs. The inner ear fluid maintains its homeostasis by a variety of subtle regulatory mechanisms, both locally and systemically. Any disturbance in one of these mechanisms can induce the disruption of homeostasis expressed by ionic, osmotic, or metabolic imbalance between the compartments. This can be manifested as membrane displacement or abnormal functions of the inner ear, depending on the degree of the disturbance. Further studies are necessary to clarify

Membranous or Hypermobile Stapes Footplate: A New Anatomic Site Resulting in Third Window Syndrome.

Objectives: To describe a potentially underappreciated pathology for post-traumatic persistent intractable dizziness and third window syndrome as well as the methods to diagnose and surgically manage this disorder. Study Design: Observational analytic case studies review at a tertiary care medical center. Methods: Patients suffering persistent dizziness following head trauma and demonstrating Tullio phenomena or Hennebert signs are included. All had reportedly normal otic capsules on high resolution temporal bone CT scans (CT). The gray-scale invert function was used to visualize the stapes fo

Enhancement of subarachnoid space during magnetic resonance imaging of endolymphatic hydrops: a case report.

Enhancement of the subarachnoid space after intravenous administration of gadolinium contrast agent is not common. Enhancement usually occurs in pathological conditions that increase the permeability of the blood-cerebrospinal fluid barrier, most notably in meningitis. We herein describe possible subarachnoid enhancement in patients with no apparent effect on the meninges. These patients had clinical signs of Meniere's disease and underwent specific magnetic resonance imaging of the inner ear to possibly visualize endolymphatic hydrops. The endolymphatic space can be noninvasively imaged by in

Vestibular drop attack: A potential origin from perilymphatic fistula?

Vestibular drop attacks are defined as a sudden fall to the ground without an obvious loss of consciousness lasting a few minutes or seconds. They are usually associated with Meniere's Disease during Tumarkin's phase. This hypothesis was produced in consideration of the case of a young girl affected by perilymphatic fistula and her Drop attacks. Is possible to consider perilymphatic fistulas a drop attack cause? Drop attack could be related with a vestibular sympathetic reflex alteration because it is involved in blood pressure control. Probably drop attack aetiopathogenesis may be due to a su

[Vertigo in children and adolescents. Part 1: Epidemiology and diagnosis of peripheral vestibular disorders].

Migraine equivalents are the most common cause of vertigo in children and adolescents. Vertigo and balance disorders occur frequently in children during the course of otitis media, middle ear effusion and viral infections. If otitis media is associated with reduced hearing and vertigo, labyrinthitis must be considered. Craniocerebral injury is another important cause of vertigo in children. In contrast, spontaneous benign paroxysmal positional vertigo is rare among children. The isolated cases of endolymphatic hydrops that occur in children are usually secondary. Perilymph fistula can have con

[A clinical study of traumatic tympanic membrane perforation].

We reviewed 165 cases of traumatic tympanic membrane perforation treated in the last 9 years (2000-2008). Of these, 103 sustained direct injury and 62 indirect injury. Ear picking accounted for 90.3% of direct injuries. Perforation size followed the classification of Yoshikawa, with Grade I perforation the most common, according for 129 (78.2%). The anteroinferior quadrant perforation site was the most common, with 98 (59.4%). Of the 165, 66 were lost during followup. Of the remaining 99, perforations closed spontaneously in 85 (85.9%), within a mean 25.9 days. Tympanoplasty was done in 14, in

Effect of tympanic perforations on the detection of distortion-product otoacoustic emissions.

The detection of distortion-product otoacoustic emissions (DPOAEs) depends on the viability of the ear's conduction apparatus. However, tympanic membrane perforations and other conductive disorders have not been fully investigated with regard to the examination of DPOAEs. Using the guinea pig model, we made perforations of different sizes and loci on the tympanic membrane and collected DPOAEs data for frequencies between 2,193 and 5,508 Hz for each condition. We found that small perforations, up to 25% of the area of the tympanic membrane, still allow us to detect emissions at the specified fr

[Diagnosis and management of Mondini malformation].

Owing to the development of imaging technology and audiology, some of the sensorineural hearing loss cases that were previcusly considered to be of unknown cause have been found to be inner ear malformation. Five cases of Mondini malformation are reviewed in this paper. CSF otorhinorrhea occurred in four cases, Klippel-Feil syndrome in three, and concurrent otosclerosis in one patient. In discussion, the authors point out that: 1) CT scanning is an supplement to audiologic tests for such patients; 2) perilymph fistula in Mondini malformation is often found at the oval window and its vicinity,

Bone lining cells of the mammalian cochlea.

It is generally believed that all bone surfaces are covered by a nearly continuous layer of cells known as bone lining cells (BLC) which separate the general extracellular fluid (GECF) from bone and its fluid compartment. Within the cochlea of some mammals regions of bone matrix are exposed to extracellular fluid. Within the scalae of the cochlea, perilymph is in contact with bone matrix; there is no evidence of a lining endothelium. Within the modiolus and subjacent to the spiral ligament, bone matrix is in contact with GECF. These findings may have importance in understanding calcium homeost

Pharmacokinetics of furosemide in endolymph.

To investigate the pharmacokinetics of organic anions in the endolymph of the guinea pig, 100 mg/kg furosemide, an organic anion, was intravenously given to measure the concentration in the cochlear endolymph by high-performance liquid chromatography with fluorescence detection. In the endolymph, the concentration of the furosemide increased slowly for 1 hr to 1.6 micrograms/ml and gradually declined thereafter. Pretreatment with 200 mg/kg probenecid, an anion transport inhibitor, had no effect on the furosemide elimination in the endolymph except on the concentration at 2 hr. This was contrar
