Heterotopic bone formation within a missile track.

A case is presented which is thought to be the first described example of heterotopic ossification occurring within the path of a bullet. Although the information was not available from prior medical records, the bullet presumably passed though bone or periosteum, thereby seeding the permanent cavity and facilitating ossification within the surrounding muscle and soft tissue.

New experiences in treating postburn talipes equinovarus associated with bone and joint pathologic changes.

This article aims to solve the problem of postburn talipes equinovarus associated with bone and joint pathologic changes by simplified and modified techniques. In addition to lengthening the shortened Achilles tendon and the contracted scar above it, we performed triple arthrodesis directly on the densely scarred, deformed foot without replacing the scar tissue with normal tissue tube or flap. Only the proximal scar-periosteum wound edge of the curved incision, 2 cm distal to the conventional one, was elevated to expose the bones to be excised. The two excised wedge-shaped bone blocks, one on …

Spontaneous bone regeneration in surgically induced bone defects in young rabbits.

Defects of the femoral shaft (15%, 20%, 25%, and 30% of the femoral length) with intact periosteum were made in young rabbits to investigate differences in union time. The mean union time was 7.3, 7.1, 7.4, and 7.0 weeks, respectively and there was no significant difference ( >0.05) between the groups. The mean healing indices were 6.7, 4.6, 3.9, and 3.1 weeks/cm, respectively and these were significantly different ( <0.05) between the 15% bone defect group and the other bone defect groups. These results suggested that union time was not affected by the amount of bone defect. Femoral lengtheni…

The anterior labroligamentous periosteal sleeve avulsion lesion: a cause of anterior instability of the shoulder.

Recurrent anterior unidirectional instability of the shoulder is not always associated with a classic Bankart lesion, which is an avulsion of the anterior labroligamentous structures from the anterior glenoid rim. Because the anterior scapular periosteum ruptures, the labrum and the attached ligaments are found to float out anterior to the glenoid rim when viewed arthroscopically. The anterior labroligamentous periosteal sleeve avulsion (ALPSA) of the supporting anterior inferior ligamentous and labral structures of the shoulder joint was found in four of eight acute primary anterior shoulder …

Laryngotracheal reconstruction in adults with the sternocleidomastoid myoperiosteal flap.

Subglottic or tracheal reconstruction may be required in cases of subglottic stenosis, invasive thyroid carcinoma, or trauma. The sternocleidomastoid myoperiosteal flap uses clavicular periosteum on a muscle pedicle to provide vascularity. Clavicular periosteum is fibrous and durable and will conform to the shape of the trachea, forming bone to provide stability to the airway. The procedure is relatively simple and involves single-stage reconstruction. Success has been achieved in reconstruction of long-standing subglottic and/or tracheal stenosis and in cases of extreme tracheal defects. On t…

A case of peripheral odontogenic fibroma arising in the mandibular premolar region of a teenager.

Peripheral odontogenic fibroma (POdF) is a rare, benign ectomesenchymal tumor. Herein, we report a case of a 15-year-old female patient who developed POdF in the mandible. The lesion was resected along with the periosteum. Histopathological findings revealed a small mass and cord-like epithelium. There was no recurrence 16 months postoperatively.

Proliferative periostitis of Garré: Report of a case.

Proliferative periostitis of Garré is described as a productive and proliferative inflammatory response of periosteum to infection or other irritation. This can be odontogenic or non-odontogenic in nature. This is a case report of an odontogenic periostitis resulting from periapical inflammation of endodontic origin. It was successfully treated by nonsurgical endodontics. Antibiotic therapy was not used during the treatment of this patient.

Posterior labrocapsular periosteal sleeve avulsion complicating locked posterior shoulder dislocation.

This case presents the imaging features of a posterior shoulder dislocation complicated by a rare but surgically relevant lesion of the posterior labrum. Due to the attachment of the posterior capsule to the posterior portion of the labrum, which in itself is attached to the posterior scapular periosteum, stripping of the labrum by the posterior capsule resulted in a posterior labrocapsular periosteal sleeve avulsion.

Deep onchocercomata close to the thigh bones of a Liberian patient.

During nodulectomies on a 53 year old Liberian woman, multiple onchocercomata were detected on the femur distal to the greater trochanter on both sides of the body. The nodules were attached to the periosteum. This location along the shaft of the thigh bone provides a further hint as to where clinically undetectable deep onchocercomata can be located.

Lateral thoracic expansion for Jeune's asphyxiating dystrophy: a new approach.

A new procedure is described for thoracic expansion in Jeune's asphyxiating dystrophy. The chest wall is enlarged by division of ribs and underlying tissue in a staggered fashion so that either rib or periosteum covers the lung. New bone formation has been demonstrated so that a viable enlargement has been obtained. The clinical result is excellent to date.

Percutaneous Intramedullary Application of Stem Cells for Fifth Metatarsal Fractures Treated With a Cannulated Screw.

Non-union and refracture of fifth metatarsal fractures are common and devastating complications in the athletic population. Stem cell application at the fracture site, for biologic enhancement, is utilized to address this challenge. We present a simple technique to approach both the endosteum and the periosteum percutaneously, under a local anesthetic, in cases of cannulated screw intramedullary fixation.

Compressive rib fracture: peri-mortem and post-mortem trauma patterns in a pig model.

Despite numerous studies on high impact fractures of ribs, little is known about compressive rib injuries. We studied rib fractures from a biomechanical and morphological perspective using 15, 5th ribs of domestic pigs Sus scrofa, divided into two groups, desiccated (representing post-mortem trauma) and fresh ribs with intact periosteum (representing peri-mortem trauma). Ribs were axially compressed and subjected to four-point bending in an Instron 3339 fitted with custom jigs. Morphoscopic analysis of resultant fractures consisted of standard optical methods, micro-CT (μCT) and scanning elect…

The Osteocutaneous Anchor of the Face: Zygomatic Cutaneous Ligament-Quantitative Topographic Map and Histologic Proof.

Quantitative 3-dimensional data on the zygomatic cutaneous ligament (ZCL) are scarce, hindering nerve-sparing planning in midface procedures. This cadaveric study evaluated the segmental vertical profile of the ZCL relative to the Frankfort horizontal (FH) plane and verified its periosteum-to-dermis continuity. Fifteen formalin-fixed adult heads (30 hemifaces) were examined; ZCL height was measured with a digital caliper at the AB, BC, CD, and EF segments of an FH-based. Mean FH-perpendicular distances were AB 7.5±1.8 mm, BC 14.2±3.1 mm, CD 10.9±2.5 mm, and EF 19.6±2.6 mm, with intersegment va…

Customized Cutting Template to Assist Sternotomy in Pectus Arcuatum.

PURPOSE: Pectus arcuatum is an anterior chest wall deformity that requires transverse wedge sternotomy. Determining and delivering the correct cutting angle are crucial for successful correction. This report describes the early clinical experience with a novel cutting template technology able to deliver the optimal cutting angle. DESCRIPTION: From patients' computed tomographic scans, the optimal cutting angle is obtained using computer-aided design. A template comprising slots tilted at the right cutting angle and a safety block to avoid damaging the posterior periosteum is printed through a…

Technique of forearm osteotomy for pediatric problems.

Correction of a rigid forearm deformity in children is often desired in congenital radioulnar synostosis, brachial plexus palsy, cerebral palsy, or posttraumatic torsional deformity. Osteotomies at the diaphyseal level present difficulties with maintenance of reduction, whether or not internal or pin fixation is used. The stabilizing and healing potential of the periosteum in these cases can be used to advantage in the correction of these deformities.

[The morphogenesis and pathogenesis of a metadiaphyseal infarct of the long bones in different types of ischemia].

In experiment on the animal's bones the various kinds of ischemia was simulated (obturational, compressive, traumatic) it was established that the central mechanism of the bone infarction pathogenesis is irreversible blockade of microcirculatory bed by cellular aggregates and microthrombi. The most extended and constant infarction occurs in embolization of the main nourishing artery and in osteotomy of diaphysis with the bone marrow intersection and the periosteum detachment on the ends of fragments.

The advantage of wide subperiosteal exposure in primary surgical correction of labial maxillary clefts.

Wide subperiosteal undermining in primary surgical correction of labio-maxillary clefts not only enhances the osteogenic activity of the periosteum but in addition, if the exposure is extended from the superior limit of the ascending maxillary process and the nasal bone to the inferior orbital rim above the infra-orbital foramen and the malar eminence, good suppleness of the overlying muscles can be achieved.
