Ultrasound in the diagnosis of peripheral neuropathy: structure meets function in the neuromuscular clinic.

Peripheral nerve ultrasound (US) has emerged as a promising technique for the diagnosis of peripheral nerve disorders. While most experience with US has been reported in the context of nerve entrapment syndromes, the role of US in the diagnosis of peripheral neuropathy (PN) has recently been explored. Distinctive US findings have been reported in patients with hereditary, immune-mediated, infectious and axonal PN; US may add complementary information to neurophysiological studies in the diagnostic work-up of PN. This review describes the characteristic US findings in PN reported to date and a

Pain, nerve dysfunction and fatigue in a vibration-exposed population.

The long term use of hand-held vibrating tools may cause vasospastic and neuromuscular problems. Symptoms include painful blanching of the fingers at low temperatures, intermittent paresthesia and numbness, impaired dexterity, a tendency to drop tools, and an increasing inability to identify small objects by touch alone. Neurological as well as vasospastic problems are graded according to the Stockholm workshop scales. Accurate and early diagnosis is particularly important (e.g., in vibration-exposed patients presenting a carpal tunnel syndrome, it is necessary to distinguish whether compressi

What's known and what's new in adipose lesions of peripheral nerves?

BACKGROUND: Adipose lesions of nerve primarily include intra- and extraneural lipomas and lipomatosis of nerve (LN). This paper will summarize the advances that have been made in the past decade, particularly related to LN and nerve territory overgrowth that have improved our understanding of the natural history, genetic background, diagnosis, imaging features, and clinical management. METHODS AND MATERIALS: Articles about adipose lesions of nerve were reviewed from 2011, when the last comprehensive review on this topic was published. Papers reporting advances on natural history, genetic back

[Sarcoid Peripheral Neuropathy and Myopathy: A Diagnostic and Therapeutic Challenge].

Sarcoidosis is an idiopathic granulomatous multi-organ disease, primarily affecting the respiratory system, eyes, and skin, with less involvement in peripheral neurons and muscles. Sarcoid peripheral neuropathy encompasses cranial and spinal nerve impairment. Muscle involvement is often asymptomatic and revealed through imaging. Symptomatic muscle involvement is categorized into three clinical types: nodular myopathy, acute myopathy, and chronic myopathy. The identification of noncaseating granulomas in peripheral nerves or muscles, coupled with the exclusion of other diseases, is essential fo

Genetics of inherited peripheral neuropathies and the next frontier: looking backwards to progress forwards.

Inherited peripheral neuropathies (IPNs) encompass a clinically and genetically heterogeneous group of disorders causing length-dependent degeneration of peripheral autonomic, motor and/or sensory nerves. Despite gold-standard diagnostic testing for pathogenic variants in over 100 known associated genes, many patients with IPN remain genetically unsolved. Providing patients with a diagnosis is critical for reducing their 'diagnostic odyssey', improving clinical care, and for informed genetic counselling. The last decade of massively parallel sequencing technologies has seen a rapid increase in

Ultrasonography in Peripheral Nervous System Diagnosis.

PURPOSE OF REVIEW: High-resolution ultrasound has made it possible to view most nerves and muscles in real time and to identify pathologic change in size, echo texture, and vascularity. This article focuses on the principles underlying ultrasound imaging and the application of ultrasound imaging to clinical disorders commonly seen in an electrodiagnostic laboratory. RECENT FINDINGS: Ultrasound is a sensitive and specific tool for evaluating myopathic and neurogenic muscle disease. It provides useful information about muscles difficult to study with other technologies, such as the tongue and d

Sensory and autonomic polyneuropathy associated with hypergammaglobulinemia.

Sensory and autonomic polyneuropathy is a rare disease characterized by a sensory nerve disorder and postganglionic autonomic dysfunction. The etiology of this disease is unknown. We described a 51-year-old woman who had a chronic sensory dominant polyneuropathy and dysautonomia associated with hypergammaglobulinemia. In the previous reports of sensory and autonomic polyneuropathy, not much attention was given to coexisting hypergammaglobulinemia. By reviewing the literatures, hypergammaglobulinemia was frequently present in these case reports. This fact leads us to consider that an immunologi

Nerve Fiber Regeneration in Toxic Peripheral Neuropathy.

There is a striking difference in the potential for regeneration of injured axons in the central and peripheral nervous systems, which is important in neurotoxicologic studies. In contrast to the former, there is a ready mechanism for replacement of peripheral nerve axons that have degenerated following exposure to toxins, where long-distance axon regeneration and substantial functional recovery can occur. This relates at least in part to the nature of the glial and other supporting cells of the peripheral nerve. To provide background for these events, data on regeneration following traumatic

A case of cauda equina cavernous angioma coexisting with multiple cerebral cavernous angiomas.

The simultaneous presence of cavernous angiomas in both the brain and spinal cord is a very rare finding, as is the location of a cavernous angioma in the cauda equina. We reported a unique case of coexisting with multiple cerebral cavernous angiomas in the brain and cauda equina.

Clinicopathological features of neuropathy in anti-neutrophil cytoplasmic antibody-associated vasculitis.

Microscopic polyangiitis (MPA), Churg-Strauss syndrome (CSS), and Wegener's granulomatosis are anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitides that may affect both the central and peripheral nervous systems. In these diseases, the frequency of concomitant peripheral neuropathy is particularly high in MPA and CSS. As opposed to kidney and lung involvement, which may be life threatening, peripheral neuropathy due to vasculitis alone does not significantly affect survival, although it can disturb day-to-day functioning and quality of life of patients. In this article, we descr

Brain-Splenic Immune System Interactions in Hypertension: Cellular and Molecular Mechanisms.

Hypertension represents a major worldwide cause of death and disability, and it is becoming increasingly clear that available therapies are not sufficient to reduce the risk of major cardiovascular events. Various mechanisms contribute to blood pressure increase: neurohormonal activation, autonomic nervous system imbalance, and immune activation. Of note, the brain is an important regulator of blood pressure levels; it recognizes the peripheral perturbation and organizes a reflex response by modulating immune system and hormonal release to attempt at restoring the homeostasis. The connection b

Ischemic peripheral neuropathy.

Ischemic neuropathy from sources other than diabetes is less common, but can be encountered in clinical practice. Diagnosis can be challenging, and many patients may be referred to the electrodiagnostic laboratory. Overlapping mononeuritis multiplex is a common presentation, but distal symmetric polyneuropathy and monomelic neuropathy patterns can be seen. Depending on the disease associated with ischemic neuropathy, a mononeuropathy or a sensory-motor, axonal-demyelinating peripheral neuropathy may be seen as well. The treatment of ischemic neuropathy varies depending on the associated diseas

Developmental expression of human tau in Drosophila melanogaster glial cells induces motor deficits and disrupts maintenance of PNS axonal integrity, without affecting synapse formation.

Tauopathies are a class of neurodegenerative diseases characterized by the abnormal phosphorylation and accumulation of the microtubule-associated protein, tau, in both neuronal and glial cells. Though tau pathology in glial cells is a prominent feature of many of these disorders, the pathological contribution of these lesions to tauopathy pathogenesis remains largely unknown. Moreover, while tau pathology is predominantly found in the central nervous system, a role for tau in the cells of the peripheral nervous system has been described, though not well characterized. To investigate the effec

Cutaneous manifestations of neurological diseases: review of neuro-pathophysiology and diseases causing pruritus.

Pruritus does not always originate from stimulation to the skin associated with primary dermatological disorders. It may be caused by neurological or behavioural disorders. The essential role of the nervous system in the control (enhancement and inhibition) of pruritus and its pathophysiology are presented. In order to allow differentiation between dermatological and neurological disorders, inherited or acquired peripheral neuropathies and central nervous disorders (Arnold-Chiari syndrome, seizure-related disorders, central nervous system tumours) that may induce itch are discussed.
