Migration of the ventriculoperitoneal shunt into the thoracic cavity: A case report and pitfalls of the rib structure.

BACKGROUND: Although ventriculoperitoneal shunting (VPS) is a universal treatment for hydrocephalus, it is generally well-known that the pro … s from the head to the abdomen. Several months later, the peritoneal catheter had migrated from the peritoneal cavity to the thoracic cavity. A computed tomography scan showed that the peritoneal catheter tunneled subcutaneously over the clavicle, penetrated the thoracic wall through the intercostal space between ribs 1 and 2, and entered the thoracic cavity. CONC …

Combination of "patch, drain, and wait" and home total parenteral nutrition for midgut volvulus with massive ischemia/necrosis.

The successful use of a combination of "patch, drain, and wait" (PDW) and home total parenteral nutrition (TPN) in the management of a case of acute, catastrophic midgut volvulus in a 2-year-11-month-old boy with near-total ischemia/necrosis of his small intestine is reported. The PDW approach to the highly effective management of acute midgut ischemia/necrosis in infancy and childhood (necrotizing enterocolitis and midgut volvulus) involves maximum gut salvage by avoidance of resection, stoma formation, or both through the use of extensive peritoneal cavity drainage by Penrose drains, TPN, an…

Peritonitis in childhood: aspects of pathogenesis and therapy.

The pathogenesis of peritonitis due to hollow-viscus perforation is currently accepted as being mainly based on the local and systemic release of pro- and anti-inflammatory mediators triggered by the presence of bacteria and bacterial products in the abdominal cavity. Therefore, treatment consists in focal restoration, intraoperative debridement and lavage, and postoperative measures such as drainage, continuous peritoneal lavage, or scheduled reoperation aiming at the removal of infectious agents from the peritoneal cavity to prevent persisting peritonitis and sepsis. In order to evaluate the…

[A case of gastric cancer with peritoneal dissemination which showed the intraperitoneal CR by administrating TS-1 orally and paclitaxel intraperitoneally].

A 66-year-old male was diagnosed with advanced gastric cancer with pylorus stenosis, and the first abdominal computed tomography (CT) revealed a large amount of ascites. A staging laparoscopy revealed peritoneal dissemination and positive cytology for numerous amounts of ascites (s-T3(SE), N0, M0, p(+), cy(+), H0, s-Stage IV). The patient received TS-1 orally and paclitaxel administered to the peritoneal cavity. After finishing the second course of the combined chemotherapy, second-look staging laparoscopy was performed, which showed the disappearance of peritoneal dissemination and negative c…

Specific collaboration between rat membrane complement regulators Crry and CD59 protects peritoneum from damage by autologous complement activation.

BACKGROUND: The peritoneal cavity is isolated from the outside and is usually a sterile environment. Patients on peritoneal dialysis (PD) have PD fluid (PDF) infused into the peritoneal cavity. We previously showed that unregulated complement activation could contribute to the development of peritoneal inflammation in yeast peritonitis in PD therapy. In that situation, suppression of local complement activation is essential to protect the host from further injury. The membrane complement regulators (CRegs), Crry, CD55 and CD59, are expressed in the rat peritoneum, especially along the mesothel…

Two giant peritoneal loose bodies were simultaneously found in one patient: A case report and review of the literature.

INTRODUCTION: Peritoneal loose body(PLB) is usually small, therefore giant Peritoneal loose body(gPLB) with a diameter >5cm has rarely been described in the literatures. We report a case of two gPLB simultaneously found in one patient. PRESENTATION OF CASE: A healthy 79-year-old man palpated himself a solid mass with alternating localizations in his peritoneal cavity 6 months ago. It was not the complaint of frequency of urinatior until he saw the doctor a week ago. Surprisingly, two oval-shaped masses were simultaneously discovered by computed tomography (CT). One was in the peritoneal cavit…

Biodistribution of oncolytic measles virus after intraperitoneal administration into Ifnar-CD46Ge transgenic mice.

In support of a proposed phase I clinical trial, we studied the biodistribution of virus-infected cells after intraperitoneal administration … y were heavily concentrated in "milky spots". Infected macrophages were also identified outside the peritoneal cavity, along the peritoneal fluid drainage pathway and in the spleen. Thus, diaphragmatic stomata, thoracic lymphatic vessels, and parathymic lymph nodes contained numerous measles-infected cells, as did the marginal zones of the white pulp of the spleen. …

Peritoneal macrophages regulate distal wound healing via endocrine release of plasma fibronectin.

The peritoneal cavity contains a large population of GATA6-expressing large peritoneal macrophages (LPMs), known to support healing of intraabdominal organs. In this study, we aimed to explore their full sphere of influence by examining their ability to perform wound healing at distant sites outside the cavity. In a mouse model combining a remote skin injury with peritoneal stimulation we observed a significant acceleration of skin wound healing in response to LPM activation. Tracking GATA6-expressing LPMs, we demonstrated that LPMs do not migrate to distant wound sites following peritoneal ac…

The "pouch" of the Douglas's pouch.

Hernia is described as the protrusion of an organ into the wall of its normal containing cavity. Internal hernia (IH) involves protrusion of viscera through: a peritoneal or mesentery defect, a normal or abnormal compartment of the peritoneal cavity. Hernias occurring in the pelvis cavity are usually classified according to the fascial margins breached and include sciatic, obturator and those through the rectouterin pouch: elytrocele and enterocele. Those hernias are defined by the protrusion of a viscus through the wall of the pelvis due to weakness of the pelvic fascia and/or muscles. Pelvic…

Acute LPS exposure enhances susceptibility to peripheral prion infection.

After peripheral infections, the initial accumulation of prions within secondary lymphoid tissues is essential for the transmission of disease to the brain. Macrophages are considered to sequester or destroy prions, but little was known of their impact on disease susceptibility after a peripheral infection. Inflammation in the peritoneal cavity can trigger the macrophage disappearance reaction, whereby the macrophages are temporarily contained within cellular aggregates on the mesothelium. We studied the impact of the bacterial lipopolysaccharide (LPS)-mediated macrophage disappearance reactio…

Effects of dextran sulfate 500 on protective responses to sublethal Trichomonas foetus infection in mice.

In order to analyze the host-parasite interactions in experimental trichomoniasis, the growth of Trichomonas foetus in the peritoneal cavity and changes in the peritoneal exudate cells were followed in mice treated with dextran sulfate 500 (DS 500), a known macrophage-toxic agent. Light microscopic observation showed that DS 500 treatment induced degeneration of peritoneal macrophages within about 48 hr after the treatment and the damaged macrophages did not phagocytize the parasites, whereas peritoneal neutrophils and lymphocytes were not affected by the drug. In the DS 500-treated mice, grow…

Glucose degradation products in peritoneal dialysis fluids may have both local and systemic effects: a study of residual fluid and mesothelial cells.

OBJECTIVE: When peritoneal dialysis (PD) fluids are heat sterilized, glucose is degraded to carbonyl compounds. These compounds are known to interfere with many cellular functions and to promote the formation of advanced glycation end-products. However, little is known about what actually happens with glucose degradation products (GDPs) after infusion into the peritoneal cavity. The aim of the present study was to investigate possible targets for GDPs in the peritoneal cavity. DESIGN: In vitro reactions between residual fluid and GDPs were studied by incubating unused PD fluid with overnight …

Structure of bursae ovaricae surrounding the ovaries of golden hamster.

The ovaries of many mammals lie within membranous sacs called bursae ovaricae. In this study, we have examined the morphology of the bursa surrounding the hamster ovary using light and electron microscopy. The bursa is composed of three layers: (1) an inner, discontinuous bursal epithelium that faces the ovary; (2) a middle layer of connective tissue that contains fibroblasts, bundles of smooth muscle cells, and blood vessels; and (3) an outer, continuous epithelium that faces the peritoneal cavity. One side of the bursa has a thin layer of connective tissue, and because the ovary may be seen …

Optimal imaging conditions for the diagnosis of pleuroperitoneal communication.

A 70-year-old woman with end-stage renal disease caused by a polycystic kidney disease developed massive right-sided pleural effusion 10 days after the initiation of peritoneal dialysis (PD). Although pleuroperitoneal communication (PPC) was suspected, computed tomographic peritoneography on usual breath holding did not show leakage. Therefore, we instructed her to strain with maximal breathing, which caused a jet of contrast material to stream from the peritoneal cavity into the right pleural cavity and allowed the identification of the exact site of the diaphragm defect. Following the thorac…

Peritoneal sarcomatoid mesothelioma in a sika deer.

A slaughtered 2-year-old female sika deer (Cervus nippon yesoensis) had diffusely distributed multinodular lesions on the serosal surface of the peritoneal cavity and several nodules in the pleural cavity. Histologically, they were composed of proliferating spindle-shaped neoplastic cells, arranged in a fascicular fashion. The cells in the invasive foci transitioned from a sarcomatoid to an epithelioid appearance. Immunohistochemically, both the spindle-shaped and epithelioid cells were at least focally positive for pancytokeratin, vimentin, calretinin, α-SMA, and desmin. From these findings, …

Bilateral Pleural Mesothelioma In Situ and Peritoneal Mesothelioma In Situ Associated With BAP1 Germline Mutation: A Case Report.

Mesothelioma in situ is a recently described precursor to invasive mesothelioma. Thus far, all cases in the literature have involved one pleural cavity or the peritoneal cavity. We describe a patient with biopsy-proven mesothelioma in situ involving both pleural cavities and the peritoneal cavity. Genetic analysis results revealed that the patient had a BAP1 germline mutation. This is the first report of mesothelioma in situ involving multiple body cavities and raises a question of whether such patients will all have BAP1 germline mutations.

Radiology of vernix caseosa peritonitis: case report and discussion.

Vernix caseosa peritonitis is a rare complication of pregnancy that can occur following Caesarean section. If fetal vernix enters the peritoneal cavity, it may incite an acute inflammatory response leading to peritonitis. This diagnosis should be considered when patients present with an acute abdomen pain following Caesarean section. Radiological imaging may identify lesions in the abdominal wall wound or peritoneal cavity, and the diagnosis can be made by image-guided biopsy.
