[Treatment of ileus and carcinomatous peritonitis].

In patients with carcinomatous peritonitis caused by the invasion and peritoneal dissemination of gastrointestinal cancer, disease progression can trigger complications such as ileus, ascites, and hydronephrosis.Anorexia, impaired oral intake, nausea, vomiting, abdominal pain, abdominal bloating, anuria, and other symptoms can develop, negatively affecting patients' general condition and quality of life. The treatment of carcinomatous peritonitis is an important determinant of outcomes, but the guidelines for its diagnosis, the evaluation of its response to chemotherapy, and the question of whi…

Fatal peritonitis caused by Rhizopus microsporus.

A 52-year-old woman with a history of Crohn's disease was admitted due to peritonitis which followed an intestinal perforation. After transitory treatment with voriconazole, resolution of the infection was complicated by recurring peritonitis resulting from necrosis and perforation of the left colon which ultimately lead to the death of the patient. Microscopic examination of the peritoneal fluid revealed the presence of broad, irregular hyphae and culture of the fluid yielded Rhizopus microsporus. The identification of the fungus was confirmed by its characteristic microscopic morphology and …

Candida albicans peritonitis in a dog.

OBJECTIVE: To describe the clinical management of a case of Candida albicans peritonitis in a dog and discuss the pathogenicity of Candida peritonitis along with treatment using the fungistatic agent, fluconazole. CASE SUMMARY: A 15-week-old Papillon developed peritonitis secondary to enterectomy site dehiscence. A pure growth of Candida albicans was obtained from the abdominal fluid. Surgical repair of the dehiscence was performed and antifungal therapy instituted with fluconazole postoperatively. A marked exudative process was noted postoperatively with production of large volumes of fluid …

Spontaneous bacterial peritonitis: few additional points.

Spontaneous bacterial peritonitis (SBP) is a treatable complication of decompensated cirrhosis. Coagulopathy with evidence of hyperfibrinolysis or clinically evident disseminated intravascular coagulation precludes paracentesis. Alcoholic hepatitis with fever, leucocytosis and abdominal pain should be evaluated for SBP. Oral ofloxacin is as effective as parenteral cefotaxime in treatment of SBP except for inpatients with vomiting, encephalopathy, or renal failure. Albumin is superior to hydroxyethyl starch in treatment of SBP.

[Conservative laparoscopic treatment of diverticular peritonitis].

GOAL: To evaluate the results of a strategy of conservative laparoscopic treatment of peritonitis due to perforated diverticulitis for all patients, without exception for intraoperative findings or general patient condition, and to study the feasibility of eventual second-stage laparoscopic colectomy. MATERIALS AND METHODS: Between January 2003 and May 2007, 25 consecutive patients were urgently hospitalized with acute peritonitis due to perforated diverticulitis. All patients underwent laparoscopic peritoneal lavage and debridement; when there was a large perforation (ten cases), suture clos…

Acremonium spp. peritonitis in an infant.

Fungal peritonitis is a rare but serious complication in children on peritoneal dialysis. Clinical presentation of fungal peritonitis is similar to bacterial peritonitis and Candida spp. are the most common agent. Fungal peritonitis has been usually associated with high morbidity, mortality and its treatment is difficult. In this report, we present an infant with Acremonium spp. peritonitis. A 7-month-old boy with Down syndrome, congenital heart disease, pulmonary hypertension and congestive heart failure required peritoneal dialysis for his persistent pulmonary oedema and symptomatic hyponatr…

[Surgical treatment of sclerosing encapsulating peritonitis].

OBJECTIVES: Sclerosing encapsulating peritonitis (SEP) or abdominal cocoon is a rare disease characterized by the formation of a fibrocollagenous membrane that wraps around the loops of the small bowel, producing obstruction. We present the results of the surgical treatment of this disease in three patients with chronic renal failure who developed SEP during the course of peritoneal dialysis (PD) and provide a literature review on the subject aimed at the general surgeon. PATIENTS AND METHOD: In our area, the PD program treated 150 patients in 14 years, with seven cases of suspected SEP. In t…

Spontaneous peritonitis attributed to actinomyces species.

Abdominal actinomycosis is a rare condition caused by actinomyces species found in the normal flora of the oral cavity, gastrointestinal and genital tract. All cases reported describe localized forms demonstrating masses, pseudotumors or abscess during surgery or radiology studies and there are no reports about spontaneous peritonitis caused by actinomycetes. We report a case in which this disease present as symptomatic ascitic fluid infection refractory to antimicrobial therapy for intra- abdominal sepsis and detected during unsuspected cytology test. The case was successfully treated with a …

Spontaneous bacterial peritonitis: a therapeutic update.

Spontaneous bacterial peritonitis (SBP) is one of the main infectious complications of cirrhosis and occurs in 8-30% of hospitalized patients with ascites. SBP is characterized by infection of the ascitic fluid (AF) in the absence of any primary focus of intra-abdominal infection. The main route by which the AF becomes infected is the hematogenous route. The pathogenic mechanism by which infection develops is bacterial translocation from the intestinal flora to the mesenteric lymph nodes and from there to the bloodstream. Contributing factors are an increased growth of Gram-negative aerobic ba…

Streptococcus viridans-associated peritonitis after gastroscopy.

Transient bacteremia during and after endoscopic procedures is a well-documented phenomenon. Streptococcus viridans peritonitis is frequently associated with peritoneal dialysis, and the infection is probably attributable to hematogenous spread, dental procedures, or transluminal contamination with oral flora. To our knowledge, no reports exist of peritonitis occurring after gastroscopy in peritoneal dialysis patients. Here, we report the case of a 69-year-old male patient receiving automated peritoneal dialysis who required emergency gastroscopy and sclerotherapy plus heat-probe coagulation t…

[Tuberculous peritonitis: a Third World's disease?].

Tuberculous peritonitis has a high morbidity and mortality. The clinical outcome of tuberculous peritonitis depends on an early diagnosis and treatment. We review five cases of tuberculous peritonitis diagnosed in a short stay medical unit over a one-year period. All patients were successfully treated with antituberculous drugs. The clinical management of this disease is described and current views regarding the value of computed tomography and other diagnostic techniques are discussed.

[A report of generalized secondary peritonitis].

Acute peritonitis not treated within the first 24-48 hours leads to complications that have to be solved by the surgeon. These complications represent MOF and the prognosis of the case is reserved. Our case has the following features: young female of 29 years of age admitted into the hospital in the gynaecologic department as she was in pain in the hypogastric area with suspicion of diagnosis by bilateral metroanexite. She had her coil devise removed three weeks before. After wards she followed the prescribed treatment with powerful antibiotics. After six days of medical treatment, she suffers…

Trichosporon inkin peritonitis treated with caspofungin.

Trichosporon inkin is one of six pathogenic species of the genus Trichosporon and the etiologic agent of pubic white piedra. Trichosporon species have been reported as a cause of disseminated infections, particularly among immunosuppressed patients. We describe the third reported case of T. inkin peritonitis associated with peritoneal dialysis and the first to be treated with caspofungin.

Spontaneous cryptococcal peritonitis in cirrhotic patients.

Spontaneous bacterial peritonitis is a common complication in patients with cirrhosis and ascites. However, spontaneous peritonitis caused by Cryptococcus neoformans is uncommon. Delayed diagnosis of cryptococcal peritonitis often results in death. We describe three cases of spontaneous cryptococcal peritonitis in patients with decompensated cirrhosis. One case had associated symptomatic human immunodeficiency virus infection. Clinical awareness of this entity may lead to the early diagnosis and proper treatment.
