A mathematical model of dorsal horn circuits' contribution to neuropathic pain.

The spinal cord is the first site for a synaptic relay in the somatosensory nervous system, where peripheral afferents synapse onto second order neurons. Noxious somatosensory afferent input is primarily transmitted to interneurons and projection neurons in the dorsal horn of the spinal cord, and projection neurons then transmit the information to supraspinal neural circuits for further processing. Despite this crucial role in pain processing, little emphasis has been placed on the dorsal horn circuits' possible contribution to neuropathic pain conditions such as phantom limb pain. Here, a mat…

21. Phantom pain.

Phantom pain is pain caused by elimination or interruption of sensory nerve impulses by destroying or injuring the sensory nerve fibers after amputation or deafferentation. The reported incidence of phantom limb pain after trauma, injury or peripheral vascular diseases is 60% to 80%. Over half the patients with phantom pain have stump pain as well. Phantom pain can also occur in other parts of the body; it has been described after mastectomies and enucleation of the eye. Most patients with phantom pain have intermittent pain, with intervals that range from 1 day to several weeks. Even interval…

A robot hand testbed designed for enhancing embodiment and functional neurorehabilitation of body schema in subjects with upper limb impairment or loss.

Many upper limb amputees experience an incessant, post-amputation "phantom limb pain" and report that their missing limbs feel paralyzed in an uncomfortable posture. One hypothesis is that efferent commands no longer generate expected afferent signals, such as proprioceptive feedback from changes in limb configuration, and that the mismatch of motor commands and visual feedback is interpreted as pain. Non-invasive therapeutic techniques for treating phantom limb pain, such as mirror visual feedback (MVF), rely on visualizations of postural changes. Advances in neural interfaces for artificial …

Arthur Rimbaud: "The Man with Wind Soles" - Riders' Osteosarcoma with Postamputation Stump Pain.

The famous poet Arthur Rimbaud (1854-1891) stopped writing poetry at 21 years and subsequently had a rather adventurous life mainly in the Arabic peninsula and Ethiopia. He died at 37 years, only a few months after the amputation of his right lower limb due to a developing tumor in the knee, which probably was an osteosarcoma in the lower third of the femur. His letters to his sister Isabelle suggest that he suffered from severe stump pain rather than phantom limb, but since he lived only shortly after surgery (he developed extensive carcinomatosis), one does not know whether a full phantom wo…

Early psychological aspects of severe hand injury.

We investigated the incidence and nature of psychological symptoms occurring during the first two months after severe hand injuries. 94% of patients had significant symptoms at some point early in rehabilitation, including nightmares (92%), flashbacks (88%), affective lability (84%), preoccupation with phantom limb sensations (13%), concentration/attention problems (12%), cosmetic concerns (10%), fear of death (5%), and denial of amputation (3%). Two months later, flashbacks (63%) remained pronounced. Nightmares (13%), affective lability (48%), concentration/attention problems (5%), fear of de…

Phantom phenomena - Their perceived qualities and consequences from the patient's perspective.

Background and aim The curious phenomenon phantom limbs early became the object of research, and its underlying mechanisms have been discussed over the years. The complex nature of phantom phenomena makes interpretation of the results ambiguous, regarding both prevalence and the accompanying suffering. There is a lack of knowledge about how amputees experience the meaning and consequences of phantom phenomena. The present aim, therefore, was to investigate how individuals, in an interview situation, described the qualities of possible perceived phantom phenomena, and how their experience affec…

We compared motor imagery performance of normally limbed individuals with that of individuals with one or both hands missing since birth (i. … d, but still modulated by different rotation angles. They are further compatible with the view that phantom limbs in hand amelia may constrain motor imagery as much as do amputation phantoms.

[Systematic review of the effectiveness of local anaesthetics in the treatment of neuropathic pain or phantom pain].

BACKGROUND: Chronic pain is still a relevant medical and socioeconomic problem. The treatment focuses not only on pain reduction but also on functional treatment goals. Neuropathic pain includes biological, social and psychological aspects. In September 2023, the updated S3 guidelines for the management of peripheral nerve injuries were published. Multimodal pain management strategies encompassing systemic and local pharmacological, physiotherapeutic and occupational therapeutic interventions, are part of the guidelines. A central question addressed the widely debated treatment option using pe

Motor control drives visual bodily judgements.

The 'embodied cognition' framework proposes that our motor repertoire shapes visual perception and cognition. But recent studies showing normal visual body representation in individuals born without hands challenges the contribution of motor control on visual body representation. Here, we studied hand laterality judgements in three groups with fundamentally different visual and motor hand experiences: two-handed controls, one-handers born without a hand (congenital one-handers) and one-handers with an acquired amputation (amputees). Congenital one-handers, lacking both motor and first-person v

A patient-centred evaluation of phantom skin wetness as a sensory symptom in people with multiple sclerosis.

BACKGROUND: A noticeable but unknown proportion of people with multiple sclerosis (pwMS) report the sudden experience of wetness on a dry skin site, i.e., phantom wetness. Yet, we lack patient-centred investigations on the prevalence and subjective experience of this uncomfortable symptom. OBJECTIVES: To assess the prevalence of phantom wetness in pwMS, its association with individual factors, and subjective experience. METHODS: 757 pwMS completed an online survey assessing the frequency and subjective experience of phantom wetness. We calculated descriptive statistics and odd ratios and per

Phantom phenomena (phantom arm) following cervical root avulsion. Effect of dorsal root entry zone thermocoagulation.

We report on nine male patients with cervical root avulsions and brachial plexus injuries following traffic accidents. These non-amputees (m … hile one patient experienced pain relief of 20% to 50%. Reviewing the literature it is assumed that phantom limb following injury to the brachial plexus indicates cervical root avulsion. In such cases Horner's syndrome is a good indication for lower cervical root avulsion (C8-Th2).

Pre-emptive analgesia for chronic limb pain after amputation for peripheral vascular disease: a systematic review.

BACKGROUND: Chronic stump and phantom limb pain after amputation for critical ischemia significantly affect patients' quality of life and pose challenging clinical problems. Pre-emptive analgesia attempts to prevent chronic postoperative pain by minimizing painful stimuli before and during surgery. METHODS: This systematic literature review aimed to evaluate the evidence supporting the use of pre-emptive analgesia in minimizing the risk of chronic stump and phantom pain after lower limb amputation for critical ischemia of peripheral vascular disease. RESULTS: A total of 11 studies have been …

Effectiveness of pre-emptive targeted muscle reinnervation on post-amputation pain in patients undergoing above knee amputation: A randomized controlled trial.

BACKGROUND: Postamputation pain in amputees is a major cause of morbidity. Studies have highlighted the impact of preemptive surgical intervention of the amputated nerves for treatment of postamputation pain. The study aimed to analyze the role of targeted muscle reinnervation (TMR) at the time of limb loss in addressing both residual limb pain and phantom limb pain. METHODS: A randomized controlled trial (RCT) in acutely injured patients undergoing above-knee amputation (AKA) (n = 97) were randomized into two groups with equal allocation, i.e., group A with TMR (intervention) and group B wit…

Visual responsiveness in sensorimotor cortex is increased following amputation and reduced after mirror therapy.

Phantom limb pain (PLP) following amputation, which is experienced by the vast majority of amputees, has been reported to be relieved with daily sessions of mirror therapy. During each session, a mirror is used to view the reflected image of the intact limb moving, providing visual feedback consistent with the movement of the missing/phantom limb. To investigate potential neural correlates of the treatment effect, we measured brain responses in volunteers with unilateral leg amputation using functional magnetic resonance imaging (fMRI) during a four-week course of mirror therapy. Mirror therap…

"Phantom" sensations following intraspinal injury.

The sensations associated at times with neural lesions in the spinal canal may have the following features and phases: 1) early anosognosia, i.e. lack of awareness of disability; 2) initial stimulation; 3) illusion of amputation; 4) disappearance of that illusion and 5) grotesque sensations. Detailed features of the phantoms such as changes in their size, position, and vividness as well as tactics evoking changes in the sensations are recorded. The special situations are described in which paraplegia is accompanied by actual amputation, or there is a phantom of an upper limb after avulsion of

[Chronic pain : Perception, reward and neural processing].

Many chronic pain syndromes are characterized by enhanced perception of painful stimuli as well as alterations in cortical processing in sensory and motor regions. In this review article the alterations in muscle pain and neuropathic pain are described. Alterations in patients with fibromyalgia and chronic back pain are described as examples for musculoskeletal pain and also in patients with phantom limb pain after amputation and complex regional pain syndrome as examples for neuropathic pain. In addition to altered pain perception, cumulative evidence on alterations in the processing of rewar…

Cortical origin of pathological pain.

Pain without accompanying tissue pathology poses a classic puzzle, presented in extreme form by phantom pain in a non-existent amputated limb. A clue to the origin of such pain is given by the recent discovery of a region of cortex active in response to incongruence between motor intention, awareness of movement, and visual feedback. Phantom-limb sensation, and repetitive strain injuries or focal hand dystonias in writers, musicians, or keyboard operators, are accompanied by plastic changes in sensorimotor cortex and by pathological pain. Disorganised or inappropriate cortical representation o
