The contributions of Sir Russell Brock to the early development of cardiac surgery are multiple. However, one of the most telling was his description of 3 cases of pulmonary stenosis in 1948 treated by direct transventricular valvulotomy. His subsequent treatise, The Anatomy of Congenital Pulmonary Stenosis, published in 1957, outlined this direct approach to cardiac lesions, a philosophy commonly adopted in modern care of cardiac lesions and a major contribution to cardiac surgical thinking.

The pathophysiology associated with tendon and nerve entrapment due to idiopathic tenosynovitis of the hand and wrist is discussed. The basis of clinical symptoms, nonoperative and operative treatment, complications, and prognosis relating to stenosing tenosynovitis and carpal tunnel syndrome are reviewed. A systematic approach to developing a philosophy for operative treatment is based upon severity of symptoms and findings at surgery. Deformities of the median nerve are classified into grades that correlate with symptoms and influence selection of the surgical procedure.

The management of nasal trauma in children differs considerably from that in adults. In children, the evaluation is decidedly more difficult because of lack of patient cooperation and different anatomical features. The controversy regarding surgical manipulation of the pediatric nose has resulted in confusion as to the proper management of nasal trauma in this age group. On the basis of our experience with these problems, we have developed a philosophy of management; its principles and techniques are described herein.

An unusual case of biliary obstruction caused by a benign villous adenoma of the ampulla of Vater is reported and the literature reviewed. Of the reported cases, 75% have jaundice or symptoms of cholecystitis. Rarely, gastrointestinal bleeding or pancreatic duct obstruction is a presenting symptom. The tumors are frequently small and can be overlooked. Operative treatment varies from excision of the tumor to pancreatoduodenal resection depending on histological evaluation, intraoperative findings, and the surgeon's philosophy.

After examination of the factors affecting the validity of audiometric tests in schools, screening programs are seen as having little value as a preventive measure because of their failure to reach their primary purpose, the early discovery of hearing loss and ear disease. The philosophy of screening is challenged and an approach of total prevention proposed that puts much more emphasis on primary prevention measures such as the control of determinants of ear disease and hearing loss in school-children.

INTRODUCTION: Peri-prosthetic fractures (PPFs) are a serious complication of total hip arthroplasty (THA) associated with significant morbidity, mortality and re-operation similar to the rates seen following femoral neck fractures. When exploring the PPF rates around cemented stems, many studies combine the results of composite beam and taper-slip stems despite their different philosophies. This review looks specifically at PPFs with the use of cemented stems of the taper-slip philosophy to evaluate the demographics, PPF rate and fracture patterns in this patient cohort.

Knowledge pertaining to adult thoracolumbar spinal deformity and corrective surgery continues to advance. Much of this is due to recent advances in understanding in sagittal alignment. A twin pregnancy is reported in which a prolonged interval between delivery of twins was accomplished by the use of labor-inhibiting drugs and the placement of a cerevical cerclage. Infants weighting 690 and 1192 g were delivered from a mother with a normal uterus at 27 and 31 weeks' gestation, with survival of the second twin. Pathologic and sonographic findings in the case, philosophies of its management, and a review of the medical literature are presented. The case is apparently unique to obstetric literature.

Symptoms of patulous Eustachian tube (ET), particularly autophony, can overlap with other conditions, and can arise from a variety of causes. We review the pathophysiology of "speech hyper-resonance syndromes", and possible mechanisms, including resonances in the tympanic membrane, Eustachian tube and nasopharynx. Treatment can be directed at the eardrum or the ET depending on site of pathology. We review typical presentations, examination findings, and useful clinical tests to distinguish PET from other disorders, and our philosophy of management.
