To determine whether the varicocele results from collateral circulation partially replacing an obstructed left renal vein, we developed a scintigraphic technique that provides not only information about the velocity of retrograde flow in the testicular veins but also quantitative information about the size of the varicocele. We report an association between idiopathic hypereosinophilic syndrome and obstruction of the hepatic veins (Budd-Chiari syndrome). Budd-Chiari syndrome was assessed by liver biopsy and hepatic phlebography and documented by computed tomography. Upper extremity deep venous thrombosis (DVT) is uncommon. In the city of Malmö, Sweden (240,000 inhabitants), 296 cases undergoing phlebography due to a suspicion of upper extremity DVT, during 1971-1986 were analysed. 165 arm phlebograms did not reveal any thrombi (56%). In 11 cases (4%) external compression of the vein was found. Thrombi in the axillary or subclavian vein were found in 120 cases (40%) and were classified as primary in 73 cases and secondary in 47 cases. Only seven cases of effort thrombosis were found. Out of 1229 patients operated on according to Harrington there were eight with clinical deep venous thrombosis (DVT) confirmed by phlebography or at autopsy. One patient died from massive pulmonary embolism. All eight DVT were located proximally on the left side. In two cases the first symptom was abdominal pain. At follow-up two patients had slight complaints. One had edema in hot weather and one swelling of the left leg. Another patient had no complaints but venography showed pathological collateral veins. There may be a considerable risk of development of a postthrombotic syndrome.

The frequency of venous thrombosis during central vein catheterization was prospectively studied in a group of 51 patients (53 catheterizations) during a 10-month period. 12 catheterizations (23%) resulted in thrombosis as revealed by phlebography. Of these, 11 were due to catheters whose site of entry was the upper limb. No other favoring factors (osmolality of the infused fluids, length or catheterization etc.) were detected. Percutaneous transfemoral occlusion of the testicular vein is a new alternative in the treatment of idiopathic varicocele. Embolization with the Gianturco coil was done on 27 patients. The occlusion was performed immediately after diagnostic phlebography. The results of followup for 1 year are available for 18 patients.
