Persistent Postictal Central Apnea in Focal Seizures: Incidence, Features, and Imaging Findings.

BACKGROUND AND OBJECTIVES: Postconvulsive central apnea has emerged as a contributor to sudden unexplained death in epilepsy. The aim of this study was to evaluate the incidence and characteristics of postictal central apnea (PICA) in focal seizures. The secondary aim was to analyze morphometric features of the amygdala and other subcortical structures involved in autonomic control. METHODS: We prospectively enrolled consecutive patients admitted to the Epilepsy Monitoring Unit at Modena Academic Hospital (Italy) from April 2020 to December 2023. Inclusion criteria were as follows: (1) age ol…

The Intersection Between the Sternocleidomastoid and Splenius Capitis as the Anatomical Landmark to Facilitate Occipital Artery Harvest: A Retrospective Clinical Study.

BACKGROUND: Occipital artery (OA)-posterior inferior cerebellar artery (PICA) bypass is a challenging procedure and is not frequently performed owing to the difficulty of OA harvest. To facilitate harvest, the intersection between the sternocleidomastoid and splenius capitis (the OA triangle) is used as the anatomical landmark to identify the OA segment that carries the highest risk of damage. This clinical study aimed to demonstrate efficacy and safety of OA harvest using this landmark. METHODS: The study included 18 patients who underwent OA harvest using the OA triangle as a landmark for t…

Treatment of a partially thrombosed giant aneurysm of the vertebral artery by aneurysm trapping and direct vertebral artery-posterior inferior cerebellar artery end-to-end anastomosis: technical case report.

OBJECTIVE: The purpose of this article is to focus for the first time on the operative management of a direct vertebral artery (VA)-posterior inferior cerebellar artery (PICA) end-to-end anastomosis in a partially thrombosed giant VA-PICA-complex aneurysm and to underline its usefulness as an additional treatment option. METHODS: The operative technique of a direct VA-PICA end-to-end anatomosis is described in detail. The VA was entering the large aneurysm sack. Distally, the PICA originated from the aneurysm sack-VA-complex. The donor and recipient vessel were cut close to the aneurysm. Wher…

Trapping and revascularization for a dissecting aneurysm of the proximal posteroinferior cerebellar artery: technical case report and review of the literature.

OBJECTIVE AND IMPORTANCE: Subarachnoid hemorrhage caused by an isolated dissection of the proximal portion of the posteroinferior cerebellar artery (PICA) is a rare problem. The optimal treatment to use for patients presenting with this clinical scenario varies and therefore is controversial in the literature. We report a patient in whom this problem was treated effectively with trapping of the diseased segment and revascularization of the PICA. We report this case to review this rare topic and to present our perspective on the indications for and the effectiveness of trapping and revasculariz…

Bilateral Cerebellar Infarction Caused by Occlusion of a Bihemispheric Posterior Inferior Cerebellar Artery Variant: A Case Report.

Bilateral cerebellar infarction due to unilateral occlusion of the posterior inferior cerebellar artery (PICA) is extremely rare, particularly when a bihemispheric PICA variant is present. We report the case of an 81-year-old male with a history of hypertension and diabetes who presented with recurrent vomiting and gait disturbance, eventually progressing to a comatose state. MRI revealed bilateral cerebellar infarctions sparing the brainstem and ventricular enlargement consistent with obstructive hydrocephalus. Magnetic resonance angiography (MRA) demonstrated occlusion of the right PICA, whi…

Posterior Inferior Cerebellar Artery Infarction Originating at C1-2 after C1-2 Fusion.

Vertebral artery injuries associated with C1 lateral mass screw insertion rarely occur during C1-2 fusion. The posterior inferior cerebellar artery (PICA) is uncommonly located at the C1 lateral mass insertion position. A 71-year-old woman with atlanto-axial subluxation and cord compression underwent C1-2 fusion. Sixth nerve palsy and diplopia were detected postoperatively, and decreased consciousness occurred on postoperative day 4. Brain magnetic resonance image (MRI) and computed tomography (CT) revealed PICA infarction. In the preoperative CT angiography, the PICA originated between the C1…

Vertebral artery variations in thoracic aortic patients.

OBJECTIVES: Arterial variation is common in the vertebral artery, and simple occlusion of the left subclavian artery may result in brain infarction, especially when it terminates in the posterior inferior cerebellar artery (PICA). We report the results of preoperative vertebral artery evaluation by magnetic resonance angiography (MRA) and its impact upon operative strategy. METHODS: Among the 214 patients who underwent thoracic aortic surgery from 2009 through 2012, 159 patients with preoperative MRA were retrospectively analysed. Patients' age ranged from 35 to 88 (median 72), 122 were male …

Posterior inferior cerebellar artery aneurysms: incidence, clinical presentation, and outcome of endovascular treatment.

BACKGROUND AND PURPOSE: Results of endovascular treatment of PICA aneurysms are not well established. The purpose of this study was to report incidence, clinical presentation, and outcome of endovascular treatment in 46 patients with 47 posterior inferior cerebellar artery (PICA) aneurysms. MATERIALS AND METHODS: Of 2169 aneurysms treated between January 1995 and March 2007, 60 were located on the PICA (incidence, 2.8%). Forty-seven proximal PICA aneurysms in 46 patients were treated with endovascular techniques, 37 ruptured (79%) and 10 unruptured (21%). Four patients presented with lower cr…

Bilateral mirror posterior inferior cerebellar artery aneurysms: diagnostic caveat on catheter angiography.

Only three definitive cases of bilateral mirror proximal posterior inferior cerebellar artery (PICA) aneurysms have ever been reported. A fourth example is presented where each PICA aneurysm remained angiographically occult on contralateral vertebral artery angiography despite retrograde filling of the parent vessel PICA. Implications for clinical practice are discussed.

Treatment of vertebral artery dissecting aneurysm by aneurysm trapping and posterior inferior cerebellar artery reimplantation. Case report.

The author presents the case of a patient with a ruptured vertebral artery dissecting aneurysm in which the posterior inferior cerebellar artery (PICA) arose from the wall of the aneurysm. The aneurysm was treated by trapping and the PICA was anastomosed to the vertebral artery proximal to the dissection. This technique allows intraoperative obliteration of the aneurysm while maintaining normal blood flow to the PICA.

Clipping of Ruptured Aneurysm of Lateral Spinal Artery Associated with Anastomosis to Distal Posterior Inferior Cerebellar Artery: A Case Report.

BACKGROUND: Arteries that anastomose with the posterior inferior cerebellar artery (PICA) can harbor aneurysms. CASE DESCRIPTION: We present a case of a patient who suffered a subarachnoid hemorrhage as a result from an aneurysm on the left lateral spinal artery (LSA) that anastomosed to the PICA. The aneurysm was treated surgically, and the flow between the LSA and PICA was disrupted. The activated anastomotic network created a new anastomosis between the LSA and PICA, which was seen at 6 months' follow-up. CONCLUSIONS: Careful follow-up is warranted in patients who have an activated anasto…

A ground-based animal model of space adaptation syndrome.

We examined the effect and aftereffect of acute or chronic load of hypergravity produced by an animal centrifuge, on pica (that is, kaolin intake) in the rat as an index of motion sickness. Although the degree of pica initially induced by acute or chronic hypergravity was not different, the rate of decline of increased kaolin intake over poststimulus days was different. Pica after a 1-h load of 2g decreased rapidly. On the other hand, pica lasted 3 days after a 48-h load of 2g. These findings suggest that the aftereffects of chronic hypergravity application on pica are due to motion sickness i…

Angiographic analysis of the lateral intercostal artery perforator of the posterior intercostal artery: anatomic variation and clinical significance.

PURPOSE: Knowledge of the anatomic variations of the posterior intercostal artery (PICA) and its major branches is important during transthoracic procedures and surgery. We aimed to identify the anatomic features and variations of the lateral intercostal artery perforator (LICAP) of the PICA with selective PICA arteriography. METHODS: We retrospectively evaluated 353 PICAs in 75 patients with selective PICA arteriography for the following characteristics: incidence, length (as number of traversed intercostal spaces), distribution at the hemithorax (medial half vs. lateral half), and size as c…

Treatment of ruptured vertebral artery dissecting aneurysms distal to the posterior inferior cerebellar artery: stenting or trapping?

PURPOSE: The treatment of ruptured vertebral artery dissecting aneurysms (VADAs) continues to be controversial. Our goal was to evaluate the safety, efficacy, and long-term outcomes of internal trapping and stent-assisted coiling (SAC) for ruptured VADAs distal to the posterior inferior cerebellar artery (supra-PICA VADAs), which is the most common subset. METHODS: A retrospective review was conducted of 39 consecutive ruptured supra-PICA VADAs treated with internal trapping (n = 20) or with SAC (n = 19) at our institution. The clinical and angiographic data were retrospectively compared. RE…

Distal posterior inferior cerebellar artery aneurysms: clinical features and outcome of 80 patients.

OBJECTIVE: Aneurysms of the posterior inferior cerebellar artery (PICA) distal to its origin are rare. Beside their rarity, their treatment is challenged by a high proportion of fusiform aneurysms, torturous course of PICA, and often severe bleeding. Our aim is to represent the characteristics of these aneurysms and their treatment, as well as to analyze outcome. METHODS: We reviewed retrospectively 80 patients with PICA aneurysms who were treated at the Department of Neurosurgery, Helsinki, Finland. RESULTS: The 80 patients had altogether 91 distal PICA aneurysms. Subarachnoid hemorrhage oc…

MR angiography of peripheral posterior inferior cerebellar artery aneurysms.

OBJECTIVE: A small subset of posterior inferior cerebellar artery (PICA) aneurysms arise distal to the vertebral artery origin of the PICA. We present two such cases seen on conventional angiography and MR angiography (MRA). MATERIALS AND METHODS: The findings in two patients are presented. In one the aneurysm presented with subarachnoid hemorrhage. The second was found incidentally but later ruptured. RESULTS: There was good agreement between the appearance of the aneurysms on MRA and conventional angiography. CONCLUSION: Awareness of the possibility of peripheral PICA aneurysms and their …
