Are global health commitments relevant in low-income countries? Considering discussions at Davos 2026.

At the 2026 World Economic Forum (WEF) Annual Meeting in Davos, health was positioned as central to economic resilience, productivity, and geopolitical stability. Four themes dominated the global health agenda: artificial intelligence (AI)-enabled health systems, investible health system resilience, women's health as an economic investment opportunity, and mental health as a macroeconomic priority. While these agendas reflect important global concerns, they are being advanced during a period characterised by shrinking development assistance, fiscal stress, debt pressures, and uneven institutio

Governance, coordination, and accountability in public health emergency preparedness and response: A scoping review.

Public health emergency preparedness depends not only on technical capacity but also on strong governance, coordinated multisector action, and clear accountability. However, evidence on how these systems function across different contexts remains fragmented. This scoping review synthesized global evidence on governance structures, coordination mechanisms, and accountability frameworks in public health emergency preparedness and response. Peer-reviewed and grey literature published between 2000 and 2025 was systematically searched in PubMed, Web of Science, Scopus, Embase, and CINAHL, as well a

Bridging crisis recovery and long-term transformation of the health system in Lebanon: evidence from key informant interviews and global lessons.

BACKGROUND: Since late 2019, Lebanon’s health system has faced immense strain an unprecedented financial and economic crisis, aggravated by the COVID-19 pandemic, and the Beirut Port explosion which damaged or destroyed a third of the capital, Beirut. This has created an urgent need for reform, heightened by the subsequent conflict (2023–2026) and its additional toll on health infrastructure and personnel. METHODS: A purposive sample of 27 key informants was chosen, based on their expertise in public health, hospital management, healthcare financing, and policy-making. Informants included poli

(Re)Tracing borders: A critical discourse analysis of the research on Latinx mental health impacts of U.S. immigration enforcement.

Social science research has documented the adverse mental health outcomes of U.S. immigration enforcement activity on Latinx immigrant individuals and families. Though informative, decolonial scholars have underscored the importance of contextualizing adverse mental health, particularly to explicitly link individual outcomes to the broader oppressive systems that produce them. For this article, I conducted a critical discourse analysis of 51 peer-reviewed journal articles to determine the extent to which social science researchers contextualize enforcement and its impacts, and to consider the

Mental health, social distress and political oppression: the case of the occupied Palestinian territory.

This paper presents a brief history of Palestinian mental health care, a discussion of the current status of mental health and health services in the occupied Palestinian territory, and a critique of the biomedical Western-led discourse as it relates to the mental health needs of Palestinians. Medicalising distress and providing psychological therapies for Palestinians offer little in the way of alleviating the underlying causes of ongoing collective trauma. This paper emphasises the importance of separating clinical responses to mental illness from the public health response to mass political

On refusing to become, or to remain, heterosexual.

This essay pushes back against mainstream queer and feminist discourses that assume an ontological distinction between lesbians and straight women. This distinction is a commonplace feature of a spate of recent books and films produced and marketed to mainstream feminist audiences. This idea of sexuality as ontology is key to the resurgence of biological determinism that has defined LGBT political advocacy in the twenty-first century. This biological determinism, or what we might call, following Lady Gaga, the "born this way" discourse, has pushed feminist discourse into a fatalistic heteropes

Where We Go From Here: Harnessing Queer Perspectives to Advance Practice in Ecology and Evolutionary Biology.

In this manuscript, we invite the field of ecology and evolutionary biology (EEB) to interrogate our relationship with society at large, consider the perspective of queer scientists, and take action to produce more accurate and objective science. Science and society continuously shape one another; this relationship determines what science is funded, which inquiries are pursued, and whose voices are included in the production and dissemination of knowledge. In EEB, many scientists have worked to address biases that originated from and were legitimized by societal norms. Here, we investigate thi

Beyond the autonomous individual: hydrosocial relations and the constitution of the relational hydrosocial self in an Andean conflict.

This article addresses a specific under-theorized problem in social theory: how subjectivity is constituted through relations in which ecological and nonhuman entities are not contextual but constitutive. Against dominant models that define the self as autonomous, bounded, and human-centered, it argues that subject formation can emerge through hydrosocial relations linking humans, water systems, territory, and nonhuman entities. Drawing on qualitative fieldwork in the Razuhuillca micro-watershed in the Peruvian Andes, the study combines in-depth interviews, walking ethnography, field observati

Malaria Elimination in Suriname, Timor-Leste and Georgia: Strategic Lessons for Sub-Saharan Africa.

BACKGROUND: Malaria remains a major cause of morbidity and mortality worldwide, with over 263 million cases and 597,000 deaths estimated in 2023, of which 94% occurred in Sub-Saharan Africa. Despite this heavy burden, several low- and middle-income countries have recently succeeded in eliminating local malaria transmission. OBJECTIVE: This article describes and synthesises malaria elimination experiences in Suriname, Timor-Leste and Georgia, all three certified malaria-free by the World Health Organization in 2025, to identify strategic lessons and contextual factors that may inform eliminati

Sweden's public health response to COVID-19: a qualitative study building on a realist approach.

BACKGROUND: Sweden's healthcare system, known for its equity and efficiency, faced criticism in the early months of the COVID-19 pandemic due to its high mortality rate compared to other Nordic countries. This study aims to explore the Swedish public health system's response to emerging respiratory infectious diseases, particularly COVID-19, using a realist approach. It analyzes contextual factors, interventions, mechanisms, and outcomes to provide a comprehensive understanding of the response, including debates around voluntary versus mandatory measures and the protection of vulnerable groups

Taking opioids in times of crisis: Institutional oversight, chronic pain and suffering in an integrated healthcare delivery system in the U.S.

BACKGROUND: Opioid treatment for chronic pain has garnered heightened public attention and political pressure to control a devastating public health crisis in the United States (U.S.). Resulting policy changes, together with ongoing public and political attention, have pushed health care systems and providers to lower doses or deprescribe and taper patients off opioids. However, little attention has been paid to the impact of such practice changes on patients who had relied on opioid treatment to manage their chronic pain. The aim of this article is to explore experiences with opioid-related c

Emerging infectious diseases: vulnerabilities, contributing factors and approaches.

We live in an ever more connected global village linked through international travel, politics, economics, culture and human-human and human-animal interactions. The realization that the concept of globalization includes global exposure to disease-causing agents that were formerly confined to small, remote areas and that infectious disease outbreaks can have political, economic and social roots and effects is becoming more apparent. Novel infectious disease microbes continue to be discovered because they are new or newly recognized, have expanded their geographic range, have been shown to caus

Implementation Determinants of Integrated Tuberculosis and Diabetes Care in South Asian Association for Regional Cooperation (SAARC) Countries: A Systematic Review.

BACKGROUND: Tuberculosis (TB) and diabetes mellitus (DM) represent a growing syndemic in low- and middle-income countries (LMICs), particularly across South Asia. The bidirectional relationship between these diseases exacerbates health outcomes and increases system burdens. Although the World Health Organization has advocated for integrated management of TB and DM, implementation remains inconsistent across the SAARC region. This systematic review aims to identify and analyse implementation determinants of integrated TB and DM care in SAARC countries. METHODS: We conducted a systematic review

Early Warning Systems and Emergency Declarations During Brazil's Largest Dengue Outbreak.

OBJECTIVES: Early warning systems (EWSs) are essential for alert, detection, and response to potential public health emergencies. Brazil's 2024 dengue epidemic, the largest ever recorded, offers a critical case for assessing correspondence between timely epidemic risk detection and health governance responsiveness. METHODS: We collected data on dengue cases, deaths, and public health emergency declarations (PHEDs) at federal, state, and municipal levels. We evaluated the timeliness of 2024 emergency declarations by retrospectively mapping observed risk levels across all states and capitals, u

Teenage pregnancy in the United States.

One out of every 10 women aged 15-19 becomes pregnant each year in the United States. Of these pregnancies, five out of every six are unintended--92 percent of those conceived premaritally, and half of those conceived in marriage. The teenage pregnancy rate is high because only a minority (one in three) of sexually active young women always use contraceptives, and only one in two of these women rely on the most effective methods. The two most common reasons given by adolescents for not using contraceptives are believing that the risk of pregnancy is small, and failing to anticipate intercourse

Unmet Health Needs Among Men who Have Sex With Men in an Era of Rising Authoritarianism: An Increasing Concern for Public Health.

Men who have sex with men (MSM) continue to experience severe health disparities despite advancements in biomedical prevention strategies such as pre-exposure prophylaxis (PrEP) and harm reduction initiatives. Systemic stigma, health care inequities, and structural neglect contribute to disproportionately high rates of HIV, sexually transmitted infections (STIs), substance use, and mental health challenges among MSM. The rise of authoritarian political movements in the United States exacerbates these disparities by restricting LGBTQIA+ health care access, defunding HIV prevention programs, and

Liberation Medicine in Authoritarian Contexts: The Brutality of U.S. Mass Incarceration.

This article engages Führer and Vorhölter's (2025) reinvigoration of Nancy Scheper-Hughes's utopian call for liberation medicine in their special issue of Culture, Medicine, and Psychiatry. They offer a practical reconceptualization of liberation medicine as "concrete utopias" in the "medical undercommons". Inspired by Scheper-Hughes's humble call for "good-enough" ethnographic engagement in her documentation of a successful solidarity movement of single-mother agricultural laborers in Northeastern Brazil, I highlight ethnographically a (fragile) concrete utopian initiative: a peer-support pro

The implementation process of decentralization of health services: a qualitative perspective from Sudan.

BACKGROUND: Health system reform initiatives have increasingly embraced decentralization as a key trend. The implementation process and its outcomes are influenced by a myriad of factors, including economic forces, political dynamics, and ideological factors. Comprehending and carefully examining the implementation phase of decentralization and its consequences to achieve desired outcomes is crucial. Notably, this phase is often considered the weakest aspect of policy reforms, especially in developing countries. Therefore, this study aimed to explore and analyze the implementation phase of dec

Estimating mortality in conflict settings: expert perspectives on methods, barriers, and ethical challenges.

BACKGROUND: Estimating mortality in conflict settings is essential for effective humanitarian response yet remains fraught with methodological, political, and ethical challenges. Conventional techniques such as retrospective surveys, prospective surveillance, and multiple-source analysis, often collapse under insecurity, displacement, or censorship. Emerging approaches, including key-informant reporting, small-area estimation (SAE), satellite-based remote sensing, and social-media analytics, offer new possibilities but raise questions about bias, data ownership, and the ethics of digital surve

Towards a new compact for health financing in Lao People's Democratic Republic.

Inadequate or ineffective health financing poses a significant challenge to the Government of Lao People's Democratic Republic's goal of achieving universal health coverage by 2030. Here we explore possible application in the Lao context of the Center for Global Development's New Compact approach, including locally-led evidence-informed prioritisation, domestic financing and consolidated supplementary aid. Using WHO's political economy analysis guide we identify opportunities and barriers and propose a road map towards implementation of the New Compact. This includes firstly, increasing local
