Couple Relationships Are Associated With Increased REM Sleep-A Proof-of-Concept Analysis of a Large Dataset Using Ambulatory Polysomnography.

Background/Objectives: Rapid Eye Movement (REM) sleep is associated with memory consolidation and several health effects including stress response, mental health, and longevity. Recently, it has been shown that regularly co-sleeping couples have increased and stabilized REM sleep when co-sleeping as compared to sleeping individually. However, it remained unclear whether this is due to a specific effect of altering the usual sleeping environment by partner deprivation or due to a generalizable REM-sleep promoting effect of couple relationships. The present study aims to clarify this ambiguity.

Management plan to reduce risks in perioperative care of patients with obstructive sleep apnoea averts the need for presurgical polysomnography.

INTRODUCTION: Obstructive sleep apnoea (OSA) is associated with increased perioperative morbidity and mortality. Patients at risk of OSA as determined by pre-anaesthesia screening based on the American Society of Anesthesiologists checklist were divided into 2 groups for comparison: (i) those who proceeded to elective surgery under a risk management protocol without undergoing formal polysomnography preoperatively and; (ii) those who underwent polysomnography and any subsequent OSA treatment as required before elective surgery. We hypothesised that it is clinically safe and acceptable for pati…

Assessing sleep in opioid dependence: a comparison of subjective ratings, sleep diaries, and home polysomnography in methadone maintenance patients.

OBJECTIVES: Comparisons of subjective and objective sleep measures have shown discrepancies between reported sleep and polysomnography (PSG) in non-drug dependent individuals with and without insomnia. Sleep may affect behavioral and physiologic aspects of drug abuse and dependence; patients in methadone maintenance therapy (MMT) for opioid dependence frequently report sleep problems. Whether subjective sleep reflects objective sleep in MMT patients is unknown. We undertook these analyses to establish the correlations among subjective and objective sleep measures in MMT patients. METHODS: We …

A pilot study to assess a novel screening questionnaire for central sleep apnea in patients undergoing overnight diagnostic polysomnography.

PURPOSE: A novel central sleep apnea (CSA) screening questionnaire, SCOUTS-BAG-HARMS (SBH), was developed and assessed with polysomnography (PSG)-confirmed "any CSA", defined as a central apnea index (CAI) ≥ 5 per hour. Unlike the STOP-BANG questionnaire, which screens for obstructive sleep apnea (OSA), SBH incorporates CSA-specific questions in three sections (SCOUTS), including silent apneas, demographics (BAG), including body mass index, and comorbidities (HARMS), including heart failure. METHODS: In this pilot study, patients undergoing a PSG at our sleep lab between January and June 2023…

Cost-utility analysis of a tiered diagnostic approach combining a screening model and polysomnography in pediatric obstructive sleep apnea.

STUDY OBJECTIVES: Obstructive sleep apnea (OSA) is a common disorder in the pediatric population, primarily diagnosed through polysomnography (PSG). However, PSG can be expensive and is often limited in availability. This study aimed to develop a cost-effective diagnostic strategy by integrating a screening model with PSG. METHODS: A retrospective analysis was conducted on children suspected of OSA. Screening models were initially constructed with machine learning techniques. Cost-utility analyses compared 3 diagnostic strategies: (1) PSG alone, (2) the screening model alone, and (3) the scre…

Performance of the Fitbit Charge 2 and Galaxy Watch 2 compared with polysomnography in assessing patients with obstructive sleep apnoea.

Wearable devices may be used to evaluate sleep in patients with obstructive sleep apnoea (OSA). This study compared the use of two wearable devices (the Fitbit Charge 2 (FC2) and Galaxy Watch 2 (GW2)) with that of polysomnography (PSG) for assessing the sleep time of OSA patients. A total of 127 consecutive patients with OSA underwent overnight PSG while wearing the FC2 and GW2 on their nondominant wrist. We compared the total sleep time (TST) determined by the devices with that obtained by PSG with paired t tests, Bland‒Altman plots, and interclass correlations. Moreover, we evaluated the tim…

Applicability of Actigraphy for Assessing Sleep Behaviour in Children with Palliative Care Needs Benchmarked against the Gold Standard Polysomnography.

In children with life-limiting conditions and severe neurological impairment receiving pediatric palliative care (PPC), the degree to which actigraphy generates meaningful sleep data is uncertain. Benchmarked against the gold standard polysomnography (PSG), the applicability of actigraphy in this complex population was to be assessed. An actigraph was placed on N = 8 PPC patients during one-night polysomnography measurement in a pediatric tertiary care hospital's sleep laboratory. Patient characteristics, sleep phase data, and respiratory abnormalities are presented descriptively. Bland-Altman…

The importance of audio-video documentation during polysomnography for diagnosis of catathrenia in a 6-year-old child: case report.

Catathrenia is a rare disorder classified as a respiratory one and characterized by expiratory groans during sleep. We report a case of catathrenia in a 6-year-old male patient, with documented video and audio polysomnography. The diagnosis of catathrenia is made through a detailed analysis of video and audio recordings during the examination. Although there is no association with the risk of physical damage known today, catathrenia presents itself as a source of anxiety among patients and their families. The differentiation of catathrenia with central apnea, obstructive apnea, primary snoring…

Utility of polysomnography and video swallow studies in the management of pediatric patients with congenital idiopathic bilateral vocal fold dysfunction.

OBJECTIVES: Congenital idiopathic bilateral vocal fold dysfunction (BVFD) is an uncommon cause of neonatal stridor and respiratory distress postnatally. Approximately 50% of affected neonates or infants will historically require tracheostomy for this condition. Timing and candidacy for tracheostomy in BVFD patients is often subjective and poorly understood. Polysomnography (PSG) and video swallow studies (VSS) may be helpful in the management of patients with BVFD prior to tracheostomy by quantifying their degree of upper airway obstruction during sleep and feeding dysfunction while awake. ME…

Utility of screening questionnaire, obesity, neck circumference, and sleep polysomnography to predict sleep-disordered breathing in children and adolescents.

BACKGROUND: Polysomnography (PSG) remains the gold standard for diagnosing obstructive sleep apnea (OSA) and sleep-disordered breathing in children. Yet, simple screening tools are needed as it is not feasible to perform PSG in all patients with possible OSA. AIM: The study adapted questions from the Pediatric Sleep Questionnaire-Sleep-Related Breathing Disorder (SRBD) Questionnaire to develop a predictive scale for OSA identified on PSG. We also tested whether adding anthropometric measurements (body mass index and neck circumference) improved prediction of OSA. METHODS: After IRB approval,…

Resolution of airflow obstruction on polysomnography after laryngotracheal reconstruction with anterior tracheal wall suspension in a patient with DiGeorge Syndrome.

DiGeorge Syndrome (DGS) may be associated with airway abnormalities including laryngomalacia and suprastomal collapse of the trachea (SCT), which may lead to sleep disordered breathing (SDB). We present a 4-year-old boy with DGS, SCT, and SDB by polysomnography (PSG) while the tracheostomy tube was capped. The patient underwent anterior tracheal wall suspension (ATWS) with concurrent tracheostomy decannulation. Following the repair, the patient experienced improved airway patency visually and by PSG with resolution of obstructive sleep apnea and hypoventilation. ATWS is an effective method to …
