Extra-articular ganglion cysts arising from the gastrocnemius tendon near popliteal vessels can cause pain and claudication. Open resection of this kind of cyst has been described frequently because the vessels can be well protected with a retractor. However, it's a challenge to remove cysts that are near vessels under arthroscopy, because a retractor cannot be used in arthroscopic surgery. This article will report a method of arthroscopic resection for extra-articular ganglion cysts near popliteal vessels.

Unilateral calf atrophy may result from several medical conditions, such as lumbar radiculopathy, asymmetric myopathy/dystrophy, a Baker's (popliteal) cyst leading to tibial nerve compression, and disuse atrophy. We present a case series of four patients with unilateral calf atrophy, including chronic neurogenic atrophy (benign focal amyotrophy, one patient), tibial nerve compression at the popliteal fossa by a Baker's cyst (one patient), and disuse atrophy (two patients). All four patients underwent electrodiagnostic (EDX) studies, and two of them had denervation changes of the gastrocnemius.

Phlebography is frequently requested to confirm or exclude deep venous thrombosis in patients presenting with a painful, swollen calf or leg. We have noted a small proportion of those with negative phlebograms to have compression or lateral deviation of the popliteal vein. These patients were submitted to contrast arthrography. Eighty per cent of this selected group were found to have dissecting or ruptured Baker's cysts; almost a third of these were dissections upwards into the thigh. A positive diagnosis in these patients is important as management differs from deep vein thrombosis. The opti

Calcified bodies may arise in a joint due to trauma, arthropathy resulting in joint destruction or synovial osteochondromatosis. These calcified loose bodies may pass into a popliteal cyst though posterior joint-bursal communications, or can arise in a popliteal cyst de novo on account of chondrometaplasia. A case history illustrates the differential diagnostic considerations. X-ray combined with ultrasound investigation will exclude soft tissue tumors and vascular lesions.

Adventitial cystic disease (ACD) is an uncommon and rare vascular entity characterized by a highly viscous and mucinous cyst located in the adventitia of an artery. Although first described in the external iliac artery, the disease is more frequently sited in the popliteal artery. ACD characteristically occurs in young adults, mostly male, with intermittent claudication as the initial presenting symptom. As the cyst enlarges, it leads to vascular compression with stenosis or occlusion. Because cystic formation of the popliteal artery is uncommon, the disease is often unrecognized. We report a

INTRODUCTION: Popliteal cysts (Baker cysts) causing compressive neuropathy of combined tibial and common peroneal nerves is an exceptional entity. Especially, an isolated multi-septate unruptured cyst (usually posteromedially) dissecting posterolaterally, thereby causing compression of multiple components of the popliteal neurovascular bundle as seen in this case report is unique in clinical field and in the literature. Awareness and early diagnosis of such cases along with a prudent technique will preclude permanent impairment.

Cystic adventital disease is a very rare lesion of peripheral arteries. The cysts usually occur in the popliteal arteries, they contain gelatinous substance and by eccentric compression of the lumen of the artery may cause calf claudication. We report a case of decompression of the popliteal artery by percutaneous ultrasound guided adventitial cyst aspiration in an 80-year-old patient complaining of intermittent claudication. The procedure resulted in normal flow in the popliteal artery and resolution of symptoms. Such procedure is efficacious, safe and can be performed on an outpatient basis.

Although synovial cysts are most commonly associated with rheumatoid arthritis and osteoarthritis, they may occur in many other conditions. The clinical manifestations of these cysts are numerous and may result from pressure, dissection or acute rupture. Vascular phenomena occur when popliteal cysts compress vessels, and result in venous stasis with subsequent lower extremity edema or thrombophlebitis. Rarely, popliteal cysts may cause arterial compromise with intermittent claudication. Neurological sequelae include pain, paresthesia, sensory loss, and muscle weakness or atrophy. When synovial…

Baker's cysts are rare cause of peripheral nerve entrapment and only a few cases of tibial nerve entrapment resulting from the popliteal cyst in the calf muscle have been reported in the literature. We present a case of rheumatoid arthritis complicated by a Baker's cyst with a tibial nerve entrapment. It is important to diagnose a Baker's cyst early and to differentiate it from thrombophlebitis, a popliteal aneurysm, tumor or muscle tear to effect optimal therapy and to obviate a potential neuropathy. Prompt recognition of these cases may save the patients unnecessary procedures and delay in t…

A review of 587 knee arthrographies revealed filling of lymph vessels in 12 cases. They occurred in connection with arthritis (4 rheumatoid), osteoarthritis, ruptured meniscus, ruptured popliteal cyst and after operation. Lymphatic filling was not related to the leakage of contrast through a ruptured popliteal cyst or to inadvertent extra-articular injection. Filling of lymphatics is more likely to occur in examinations with a positive contrast medium when a larger amount of medium is used.

A 62-year-old man with intermittent claudication but normal foot pulses at rest had no atheroma shown on femoral arteriography. There was evidence of external compression of the artery at the level of a popliteal cyst which was demonstrated by arthrography on films taken after sufficient exercise to be in communication with the knee joint. At operation the cyst was continuous with changes of cystic adventitial disease of the popliteal artery. Both the main cyst and th cystic changes in the artery were removed. Postoperatively the patient's claudication disappeared. Evidence is presented that s…

OBJECTIVE: To determine the prevalence of popliteal cyst (Baker's cyst) in rheumatoid arthritis (RA), through the use of a very sensitive and non-invasive method, high resolution ultrasonography. The present is the first such report in the literature.
METHODS: Ninety-nine unselected consecutive patients with RA, after undergoing routine clinical and laboratory evaluation, had knee radiographs and ultrasound examinations of both knees, the popliteal fossae and calves, using an Ultramark 9ATL apparatus with a 3 MHz curved array and 10 MHz linear array heads and color doppler ability.
RESULTS: …

Rupture of simple (extraneural) cysts such as popliteal cysts (Baker's cysts) is a well-known occurrence. The purpose of this report is to introduce the similar occurrence of extraneural rupture of peroneal and tibial intraneural cysts in the knee region, describe the associated magnetic resonance imaging (MRI) findings, and identify risk factors. There was MRI evidence of rupture in 20 of 38 intraneural cases reviewed, mainly in the region of the fibular head and popliteal fossa. Ruptured intraneural cysts and simple cysts share these MRI findings: T2 hyperintense fluid within surrounding int…

BACKGROUND: Tibial mononeuropathy due to compression by a synovial cyst is uncommon with fewer than 10 cases reported in the literature.
METHOD: Case Study.
RESULTS: Tibial motor mononeuropathy with focal conduction block between the popliteal fossa and ankle was identified by electrophysiological testing. Magnetic resonance imaging and surgical exploration identified a synovial cyst compressing the nerve just distal to the popliteal fossa.
CONCLUSIONS: The combination of electrophysiological localization and imaging in cases of unexplained tibial mononeuropathy may identify an unexpected s

A 40-year-old female arrived with persistent posterior right knee pain, swelling in the popliteal, infrapatellar, anterior calf areas and difficulty walking due to joint stiffness. Multiple hypoechoic collections with internal echoes and debris were discovered in the anterior calf region using ultrasound imaging, which extended from a thick-walled infrapatellar hypoechoic collection with peripheral vascularity. A significant popliteal fossa cyst of comparable appearance was also observed. The results were consistent with a ruptured infrapatellar bursa sac and a popliteal fossa (Baker's cyst).

The ruptured popliteal synovial cyst is a common complication of chronic knee arthritis. In contrast, non-popliteal synovial rupture is less well recognized and may present a diagnostic dilemma. We report an 81-year-old woman who presented with chest wall pain and ecchymosis. Ultrasonography of the shoulder region readily diagnosed a dissecting parasynovial cyst. She developed the unusual complication of contralateral recurrence. Literature review revealed a small but important set of non-popliteal synovial ruptures in the regions of the shoulder, elbow, wrist, spine, hip, knee, and ankle. Loc

Popliteal cysts, also termed Baker's cysts, are clinically common cystic lesions in the popliteal fossa. Typically, the contents of a ruptured cyst tend to spread into the myofascial interfaces in any direction, most commonly inferomedially or into a palpable superficial position. However, to our knowledge, reports of Baker's cysts dissecting into the deep intermuscular septum of the lower calf are extremely rare. We present here the details of the successful treatment through arthroscopy combined with lower calf incision of a patient who sustained hematoma of the knee and lower calf secondary…

The authors present three clinical cases of venous compression of the popliteal area. They are two popliteal cysts and one tibial exostosis. A relatively frequent popliteal artery aneurysm and a few iatrogenic cases, complete the list of possible differential diagnoses. Ultrasonography seems to present definite advantages concerning its use and aiding phlebography, arteriography and arthrography in detecting a popliteal "mass" causing a proximal venous stasis.

Sixty-two patients with popliteal or calf synovial cysts defined by arthrography are presented, of whom 34 had the pseudothrombophlebitis (PTP) syndrome. While the clinical manifestations of PTP may closely mimic thrombophlebitis, including the presence of calf pain, swelling and warmth, and a positive Homans' sign, the helpful descriminating features include the presence of inflammatory joint disease (91%), concomitant pain and swelling of the knee (94%), a demonstrable knee effusion (91%) and the absence of deep venous tenderness or cord. Except for seven patients with large intact Baker's c
