High HIV incidence density among middle-aged and older populations in Luzhou, China: evidence from retrospective cohort study, 2018-2023.

HIV incidence is crucial for monitoring epidemics. This analysis of retrospective, programmatic HIV screening data aimed to estimate HIV incidence density within a repeatedly tested cohort, reflecting new infection detection among individuals who accessed testing services, and to explore associated demographic factors. We analyzed data from 1,863,802 individuals who accessed routine services across all city health facilities, tracking those initially seronegative. The overall incidence density was higher in males (1.21 per 1,000 person-years) than females (0.46), with peaks observed in males a

Detection of Toxoplasma gondii in wild bivalves from the Kerguelen and Galapagos archipelagos: influence of proximity to cat populations, exposure to marine currents and kelp density.

Oocysts of the protozoan Toxoplasma gondii are found in felid feces and can be washed into coastal waters, where they persist for months, attaching to algae and accumulating in invertebrates. We used wild bivalves to assess contamination of coastal waters of the Kerguelen and Galapagos archipelagos by this zoonotic parasite. Additionally, we leveraged the contrasting situations of these archipelagos to identify some potential drivers of contamination. In the Galapagos, with a cat density reaching 142 per km2, 15.38% of the sampled oysters (Saccostrea palmula) tested positive for T. gondii by q

High serum high-density lipoprotein-cholesterol is associated with memory function and gyrification of insular and frontal opercular cortex in an elderly memory-clinic population.

The issue of whether serum lipid marker values are cognitively and neurologically significant for elderly individuals attending a memory clinic has been controversial. We investigated the associations of serum lipid markers with the memory function and cortical structure in 52 patients aged ≥75 years who had attended our memory clinic based on their subjective memory complaints. None had a history of medication for hyperlipidemia. The Wechsler Memory Scale-Revised (WMS-R) was administered to all patients for the assessment of their memory function. Serum low-density lipoprotein cholesterol, hi

Influence of duration of total breast-feeding on bone mineral density in a Turkish population: does the priority of risk factors differ from society to society?

INTRODUCTION: Despite numerous studies on risk factors for osteoporosis the effect of breast-feeding on bone mineral density (BMD) is unclear. In this study our aim was to determine the influence of total duration of breast-feeding on BMD, and subsequent risk of osteoporosis. METHODS: A total of 1,486 postmenopausal women over the age of 40 were included in the study. Women with diseases or who were under drug treatments known to affect bone metabolism were excluded. The BMD of the lumbar spine and femoral neck were measured by dual-energy X-ray absorptiometry technique, using either the Norl

Association Between Baseline Mammographic Breast Density and the Incidence of Natural Menopause in Korean Women: A Nationwide Population-Based Study Using National Health Screening Data.

BACKGROUND: High mammographic breast density (MBD) reflects cumulative exposure to sex hormones. While it is well established that breast density declines during the menopausal transition, the association between baseline MBD and the subsequent timing of natural menopause remains underexplored. We investigated the relationship between baseline MBD and incident natural menopause in Korean women. METHODS: We conducted a retrospective cohort study using the Korean National Health Insurance Service database. A total of 428,998 premenopausal women aged 40-55 years who underwent breast cancer scree

Optimal Low-Density Lipoprotein Cholesterol Levels in Adults Without Diabetes Mellitus: A Nationwide Population-Based Study Including More Than 4 Million Individuals From South Korea.

BACKGROUND: Although the strong association between low-density lipoprotein cholesterol (LDL-C) and cardiovascular disease (CVD) is well-known, the threshold LDL-C level at which the risk of CVD begins to increase in individuals without diabetes mellitus (DM) remains unknown. We aimed to evaluate the association between incident CVD and serum LDL-C levels with or without statin use in individuals without DM. METHODS: We identified 4,182,117 individuals without previous CVD who underwent a health screening examination in 2009 and 2011 from the Korean National Health Insurance Cohort database.

Association of the serum myeloperoxidase/high-density lipoprotein particle ratio and incident cardiovascular events in a multi-ethnic population: Observations from the Dallas Heart Study.

BACKGROUND AND AIMS: Myeloperoxidase (MPO), a product of systemic inflammation, promotes oxidation of lipoproteins; whereas, high-density lipoprotein (HDL) exerts anti-oxidative effects in part via paraoxonase-1 (PON1). MPO induces dysfunctional HDL particles; however, the interaction of circulating levels of these measures in cardiovascular disease (CVD) has not been studied in humans. We tested whether serum levels of MPO indexed to HDL particle concentration (MPO/HDLp) are associated with increased CVD risk in a large multiethnic population sample, free of CVD at baseline. METHODS: Levels

Clinical concordance between the Gail and Tyrer-Cuzick breast cancer risk models in a Turkish population with high mammographic density prevalence: A cross-sectional study.

The Gail and Tyrer-Cuzick models draw on different input variables and can assign the same woman to different risk categories, yet whether this discordance alters clinical risk stratification in populations with high mammographic density prevalence has not been formally assessed. This study aimed to compare clinical concordance and density-driven risk reclassification between the 2 models in a Turkish screening cohort. In this prospective cross-sectional study, lifetime breast cancer risk was calculated using both models in 1365 women aged 35 to 75 undergoing routine screening at Ordu Universi

Mammographic Density as a Mediator for Breast Cancer Risk: Pooled Analysis of Four Population-Based Case-Control Studies on Women with Screening Mammograms before Age 50.

BACKGROUND: Multiple risk factors influence breast cancer risk, but the role of percent mammographic density (PMD) in mediating these associations remains unclear. METHODS: In this pooled analysis of women who had screening mammograms before age 50 years, we assessed whether PMD mediates associations between reproductive, structural, and lifestyle factors and breast cancer risk. Causal mediation analysis was used to estimate direct and indirect effects and the proportion mediated by PMD. RESULTS: PMD partially mediated 29% of the association between breast calcifications and breast cancer ri

Prediction Model for Low Bone Mineral Density in Cancer Survivors and Age-Matched Controls Using a Causal Bayesian Network: A Nationwide Population-Based Study in Korea.

BACKGROUND: With advances in cancer treatment, the number of cancer survivors has increased, bringing attention to long-term complications such as alterations in bone mineral density (BMD). Although survivors are at elevated risk for low BMD, prior studies have focused on specific cancer types and relied on traditional regression models, which are limited in capturing complex inter-variable relationships. This study aimed to examine the causal relationships among factors affecting BMD in cancer survivors and age-matched controls using causal Bayesian network (CBN) modeling. METHODS: Data from

Schmorl's nodes are associated with prevalent osteoporotic vertebral fracture and low bone mineral density: a population-based thoracic spine MRI study in older men and women.

BACKGROUND: Schmorl's node (SN) corresponds to nucleus pulposus herniation into the vertebral spongy bone with thickened trabeculae around the formed node. We hypothesize that a pathway may exist that: osteoporosis → weakened endplate → SN development ↔ endplate fracture of an osteoporotic vertebra. METHODS: For osteoporotic fractures in men (MrOS) and in women (MsOS) Hong Kong studies, at 14-year follow-up, thoracic spine magnetic resonance imaging (MRI) was sampled in 270 males (mean: 82.9±3.7 years) and 150 females (mean: 81.5±4.3 years).
