The toe-nail dust produced in chiropodial practice causes allergic hypersensitivity and Trichophyton rubrum is the most common fungal cause of nail dystrophy. Use of drills and burrs to reduce the thickness of hyperkeratotic nails generates dust and chiropodists develop precipitins to T. rubrum. A survey into the prevalence of these antibodies in 11.2% of state-registered chiropodists is described and 14% of the profession estimated to have antibodies to T. rubrum. A report is presented on systemic anaphylactic reaction [SAR] of rabbits to bovine serum albumin and ferritin. Active anaphylactic sensibilization with a high yield of sensibilized animals was achieved by administering antigens to rabbits aged a few hours up to 3 months. A patient with regional enteritis had received iron dextran for treatment of iron deficiency. Subsequently he developed a large (3.1 g/100 ml) IgG-K serum spike which had precipitin activity against dextran sulfate but not a variety of other antigens. There has been no evidence of multiple myeloma and the spike gradually disappeared spontaneously over the course of 2 years. We speculate that the monoclonal protein may have developed as a response to the iron dextran injections under the immunologic stress of a chronic inflammatory disease.

Alterations in immunological response before and after chemotherapy were investigated in hamsters infected with A. ceylanicum. Four reference anthelmintics mebendazole, albendazole, levamisole and pyrantel pamoate and one newly synthesized anthelmintic compound 81-470 were used. A 62-year-old woman presented with a 3-month history of shortness of breath on exertion and dry cough. On examination, she was noted to have fine end-inspiratory crepitations over the upper zone of the lungs. Pulmonary function tests (PFTs) showed a restrictive defect. A survey of the frequency of sensitization to Aspergillus antigens was conducted in a group of asthmatics in Cleveland and compared with a group of asthmatics in London, using common antigens for testing purposes. The prevalence of oral yeasts and humoral precipitating antibodies to candida was estimated in 204 unselected diabetic patients (172 outpatients and 32 inpatients). This is a report on two patients with occupational hypersensitivity pneumonitis (HP) caused by spores of Hypsizigus marmoreus (Bunashimeji) and serial follow-up measurements of serum surfactant protein D (SP-D) levels. A 44-year old female farmer and pigeon-breeder presented with paroxysmal attacks of dyspnoea caused by recurrent subglottic laryngeal oedema. The symptoms disappeared under corticosteroid therapy but returned after discharge from hospital. The avian origin of the disease, suggested by a strongly positive search for avian precipitins, was confirmed when the attacks ceased after antigen exclusion. A realistic inhalation challenge test performed on two occasions with pigeon's feathers and droppings as antigens resulted, within 15 minutes, in a 4-hour long attack of paroxysmal laryngeal dyspnoea.

A review was made of the medical records of 49 patients from whose sputum mucoid Pseudomonas aeruginosa had been isolated over a 20-month period.
