A patient is described in whom two different sites of cannulation of central veins produced markedly different pressures with potentially dangerous consequences. Falsely elevated pressures recorded from a catheter inserted via the subclavian vein were related to the patency in the arm of an arteriovenous shunt. The likely cause of this phenomenon is discussed.

Blood pressure is an inherently variable phenomenon. In addition to this the level rises in the clinical setting; a response which is extremely variable. Problems therefore arise in the diagnosis and treatment of hypertension. Studies have shown that blood pressure can reliably be measured at home either with a stationary or ambulatory apparatus. The readings which are reproducible are generally lower than those obtained in the clinic. Ambulatory recordings are a more reliable guide to prognosis and to the response to hypotensive agents. In order to create awareness of a potential adverse effect of extraoral orthodontic appliances, we report the development of alopecia areata in two patients. Both patients experienced the onset of alopecia areata directly under sites of pressure from thier "headgear." In one patient the hair loss began 2 weeks after she began using her appliance. The other girl noticed patchy alopecia 2 to 3 weeks following an increase in the pressure and duration of use of an extraoral appliance. An account of the significance of intracranial pressure is followed by a review of the various sites in which it can be recorded (spinal canal, lateral ventricles, subarachnoidal spaces, operation cavity, other sites). Monitoring of ventricular pressure is discussed in detail; this deals with the classic site and its technique has become part of clinical practice. The utilisation and interpretation of the data in the form of pressure levels and their changes in the course of time are also discussed. The clinical indications for monitoring are noted.
