AIMS: Although the United States incarcerates nearly two million people, the epidemiology of psychiatric disorders in correctional populations is not well understood, and no study has examined temporal trends in psychiatric disorder prevalences within a single correctional system. This study assessed how psychiatric disorder prevalences have changed in the Texas Department of Criminal Justice (TDCJ), the largest American state prison system housing post-conviction, sentenced individuals.
METHODS: This retrospective cohort study of TDCJ electronic medical record data from 1 January 2016 throug

PURPOSE: Substance use disorder is one of the most prevalent health issues among prison populations. In this regard, addiction treatment and harm reduction programs have been implemented in Iranian prisons since 2002. The purpose was to describe the practical experience of implementing harm reduction programs in Iran's prisons, emphasizing the impact of policy decisions on it.
DESIGN/METHODOLOGY/APPROACH: The study was qualitative study. In addition to analyzing 16 documents, the authors conducted face-to-face semi-structured interviews with 11 key informants who were asked questions about th…

During January-September 2024, a national active case-finding program in Peru's prisons screened >38,000 persons for tuberculosis (TB) using chest radiography with automated interpretation and rapid molecular tests. The program found high percentages of TB, rifampin-resistant TB, and asymptomatic infections, demonstrating the urgent need for systematic screening among incarcerated populations.

BACKGROUND: Provision of opioid agonist treatment (OAT) in custodial settings is resource-intensive and may be associated with diversion, non-medical use, and violence. A clinical trial of a new OAT, depot buprenorphine (the UNLOC-T study), provided the opportunity to obtain health and correctional staff perspectives regarding this treatment prior to widespread roll-out.
METHODS: Sixteen focus groups with 52 participants were conducted, including 44 health staff (nurses, nurse practitioners, doctors, and operational staff) and eight correctional staff.
RESULTS: Key challenges to providing OA

INTRODUCTION: Prison settings represent the highest concentration of prevalent hepatitis C cases in Australia due to the high rates of incarceration among people who inject drugs. Highly effective direct-acting antiviral (DAA) therapies for hepatitis C virus (HCV) infection are available to people incarcerated in Australian prisons. However, multiple challenges to health care implementation in the prison sector present barriers to people in prison reliably accessing hepatitis C testing, treatment, and prevention measures.
MAIN RECOMMENDATIONS: This Consensus statement highlights important con…

COVID-19 has been an unprecedented challenge in carceral facilities. As COVID-19 outbreaks spread in the US in early 2020, many jails, prisons, juvenile detention centers, and other carceral facilities undertook infection control measures such as increased quarantine and reduced outside visitation. However, the implementation of these decisions varied widely across facilities and jurisdictions. We explored how carceral decision makers grappled with ethically fraught public health challenges during the pandemic. We conducted semistructured interviews during May-October 2021 with thirty-two medi…

Supporting people placed under house arrest is a contemporary challenge. Between popular demand for risk control, judicial demand for recidivism prevention and hospital prerogatives, the articulation between care providers is often perilous. The advanced practice nurse plays a key role in the transition from inside to outside.

OBJECTIVES: To evaluate the effectiveness of opioid maintenance programs in treating opioid dependence in Spanish prisons.
MATERIAL AND METHOD: A narrative bibliographic review was carried out on the following databases: PubMed; Cochrane; IBECS; LILACS; HealthCare. All clinical trials were chosen as a preference, along with systematic review articles and some articles that were considered relevant for their content. The time period was limited to between January 2011 and November 2021. The languages chosen were English, Spanish and Catalan. Repeated articles and those that were not related to…

Crime rates in Australia have declined or been steady over the past decade yet prison populations are at all-time highs. Similarly, unemployment rates have been low but unemployment for those ex-prisoners seeking work is very high. In this paper, we draw on the findings of an Australia-wide survey of government-funded employment service providers who support working-aged Australians including ex-prisoners in their search for work. We find that the heterogeneity of the needs of ex-prisoners coupled with the frugality and inflexibility of government policies and practices frustrates the abilitie

This study aimed to identify the prevalence of women in prisons who have already had an HIV test inside prison and the factors associated with this test. A cross-sectional study was carried out with 1327 women in 15 prisons in 9 states in Brazil. Almost 60% (95% CI: 57.8-63.7) of women have already been tested for HIV in prison. The factors associated with this HIV test were age ≥41 years (OR = 1.9; 95% CI = 1.2-2.9), highest level of education (OR = 1.9; 95% CI = 1.3-2.8), having been arrested 3 or more times (OR = 1.9; 95% CI = 1.3-2.8), having received information about HIV/STI in the lifet…

IMPORTANCE: There is a large body of epidemiologic evidence that heat is associated with increased risk of mortality. One of the most effective strategies to mitigate the effects of heat is through air conditioning (AC); Texas regulates the internal temperature of jails to stay between 65 and 85 °F degrees, but these same standards do not apply to state and private prisons.
OBJECTIVE: To analyze whether heat during warm months is associated with an increased risk of mortality in Texas prisons without AC.
DESIGN, SETTING, AND PARTICIPANTS: This case-crossover study included individuals who di…

Many studies have demonstrated that prisons are hotspots of tuberculosis disease and transmission. Despite this, it remains unclear which interventions are most effective at controlling tuberculosis in prisons. The objective was to determine the study designs used to investigate tuberculosis control in prisons, and the efficacy of interventions undertaken. This systematic review included published studies which had the aim of reducing TB incidence or prevalence, or increasing the number of people screened for active pulmonary tuberculosis in incarcerated populations. 2,429 records were identif…
