Obesity: a sociological guide for health practitioners.

Obesity is a condition of excess body fat that has been identified as one of the most serious public health problems of the 21st century. Obesity is a leading preventable cause of death globally and increases the likelihood of cardiovascular disease, diabetes and various cancers. Obesity worldwide has tripled in the adult population over the past 40 years, with 13% of the world's adult population now obese, leading to calls for control of a global obesity epidemic. Causes of obesity can be delineated at individual, societal and systemic levels, highlighting the need for analysis that transcend

Improving how positive newborn screening results are communicated to parents of children with sickle cell disease.

Sickle cell disease, the most common inherited disorder at birth in the UK, has been included in the UK newborn screening programme since 2006. For parents, receiving the news that their newborn has a serious long-term condition can trigger reactions such as shock, disbelief and guilt. Guidelines on sickle cell disease provide clear screening pathways, but there is variation in how and by whom positive results are communicated to parents. The way in which this is done is crucial, not only for parents' acceptance of the diagnosis but also for their future therapeutic relationships with healthca

Experiences of giving and receiving care in traumatic brain injury: An integrative review.

AIMS AND OBJECTIVES: To synthesise the literature on the experiences of giving or receiving care for traumatic brain injury for people with traumatic brain injury, their family members and nurses in hospital and rehabilitation settings. BACKGROUND: Traumatic brain injury represents a major source of physical, social and economic burden. In the hospital setting, people with traumatic brain injury feel excluded from decision-making processes and perceive impatient care. Families describe inadequate information and support for psychological distress. Nurses find the care of people with traumatic

Is there a role for pharmacists in multidisciplinary health-care teams at community outreach events for the homeless?

Homelessness is a significant public health problem. It is well-documented that people experiencing homelessness exhibit more serious illnesses and have poorer health than the general population. The provision of services and interventions by health-care professionals, including pharmacists, may make a simple yet important contribution to improved health outcomes in those experiencing homelessness, but evidence of roles and interventions is limited and variable. In Australia, the Queensland University of Technology Health Clinic connects with the homeless community by taking part in community

[Assessing common factors in psychotherapy: psychometric properties of a new time-economic instrument (WIFA-k)].

Research on common and differential factors in the therapeutic process is impeded by the lack of instruments suitable for assessing common change mechanisms. This study presents the psychometric properties of a newly developed time-economic instrument (WIFA-k), which was designed to assess common factors of psychotherapy as designed by Grawe. Within a multi-center study comparing the efficacy of cognitive therapy and psychodynamic therapy in the treatment of social phobia, 6 raters assessed 25 randomly selected, videotaped therapy sessions of each treatment approach, and evaluated common facto

Community health worker encounter forms: a tool to guide and document patient visits and worker performance.

OBJECTIVES: We explored the benefits of using community health worker (CHW) encounter forms to collect data on patient interactions and assessed the effectiveness of these forms in guiding and directing interactions. METHODS: A 1-page standardized encounter form was developed to document topics discussed during visits with diabetes patients. A portion of the form was designed to be used as a script to guide the interaction and assist patients in setting appropriate self-management goals. Data were also collected via CHW work logs and interviews with CHWs and their supervisors to validate find

Assessing your office for care of lesbian, gay, bisexual, and transgender patients.

Practitioners act as guide, protector, and confidant to their patients' most vulnerable health care concerns. Arguably, one of the most important times to consider the dynamics of a health care relationship is when treating culturally diverse populations such as lesbian, gay, bisexual, and transgender (LGBT) patients. This article outlines several recommendations for how physicians can begin the process of assessing their office and practice habits for supportive care of LGBT patients, including evaluating your belief systems, understanding risk factors associated with LGBT patients, modifying

Developing the therapeutic relationship: from 'expert' professional to 'expert' person who stutters.

UNLABELLED: This article looks back over the years and identifies some of the most influential thinkers, writers, and researchers who have had a profound effect on the way the therapy at the Michael Palin Centre for Stammering Children in London has evolved. It tracks the changes that have occurred in theoretical perspective, treatments offered, and the delivery of therapy. In particular this author is interested in the changing nature of the therapeutic relationship between professionals and people who stutter (PWS), and describes the way it has developed from the "expert professional" toward

Comparing homeopathic and general practice consultations: the case of problem presentation.

Both general practice and homeopathic consultations are organized around the key task of treating patients' health-related problems. Despite their different theories of healing, interactions between professionals and patients in both share many features, though there are also clear differences in the ways in which patients and professionals go about the process of problem solving. This paper compares the ways in which a specific activity, the delivery and reception of the reason for the visit, is managed in these two institutional environments. Through the comparison, it discusses ways in whic

The online clinical case study group: an e-mail model.

Online peer supervision and case study groups are an effective method for clinicians to share experiences and support each other in their work. This article describes some theoretical and practical ideas about how to set up and manage such a group using an e-mail list. It offers some suggests for helping members acclimate to the group, present their cases, and pace their responses to the case discussions. The unique group dynamics of online text communication are explored, as well as the important ethical issues that must be considered in these types of online groups. The article concludes by

[Ethical problems in forensic-psychiatric assessment].

Forensic psychiatry as a subspecialty variously differs from general psychiatry. Therefore psychiatrists working in forensic psychiatry are confronted with ethical problems of other kind. In the following several ethical problems connecting with forensic psychiatric examination are pointed out. Finally the question is discussed, whether there is a need of special ethical guidelines for forensic psychiatry.

Treatment of physically abused adolescents.

It is clear that a number of factors are emerging as necessary criteria for helping physically abused adolescents: The need to establish specific residential treatment units for treating physically abused adolescents. The importance of educational programming to develop community awareness of the seriousness of this issue. The growing need for an awareness is conventional treatment environments to recognize the physically abused adolescent. The need to develop a sensitive and therapeutic approach for such children.

Different clinical perspectives of good and poor therapy sessions.

This study investigated differences in causal attributions made by student clinicians taking actor and observer roles in good and poor therapy sessions. One hundred thirty-four graduate student clinicians in speech-language pathology were asked to imagine a hypothetical good or poor therapy session in which they took either the role of a clinician working with a client in a session or that of a clinician observing the session. To account for the session's hypothesized outcomes, clinicians taking the actor role cited client causes more frequently than other causes while clinicians taking the ob

Implications of object-relations theory for the behavioral treatment of agoraphobia.

In this paper I review recent developments in analytic object-relations theory and show how such a frame of reference can formulate and contribute useful clinical insights for the implementation of in-vivo exposure treatment. Since agoraphobic patients tend to relate to significant others, including therapists, in either a safety-producing or potentially anxiety-provoking fashion, knowledge of developmental object-relations theory can aid the behavior therapist in avoiding the enactment of one or the other side of the split. In-vivo exposure, is structured to guard against such enactments. How

Touch in the counselling relationship: an exploratory study.

There is a continuum of views on the extent to which touch enhances the treatment process of patients. Health professionals must deal with the use of touch in some way, even if their final decision is not to use it. Health professionals were interviewed about personal touch as a part of their professional relationships with patients. Even though the subjects using touch differed in health professions and primary theoretical orientations, a commonality was the belief in a strong person-to-person relationship as an integral part of the helping process. Instruction concerning touch appears to be

Engagement before alliance.

An approach to the psychotherapy of ego-depleted states or with morbid disease involves the commitment of the therapist far beyond a distant psychoanalytic stance. Engagement must occur before a therapeutic alliance and exploration of underlying issues can begin. The patient in a helpless state needs the therapist's concern (both by frequent visits and telephone), active participation with family, and occasionally medication. The 'all out' effort allows the patient to borrow some of the therapist's strength. Within this approach, one may expand or contract goals; many patients went on to furth

The dissatisfied patient with chronic pain.

Patients with chronic pain may become dissatisfied because of their lack of improvement. Thirteen such patients, encountered in a pain-management program, registered formal complaints of their dissatisfaction. From their histories they were found to be the most chronic and treatment-refractory patients encountered, with problems of medication dependency, accident proneness, and dissatisfaction with previous treatment efforts. During hospitalization they opposed psychologic approaches, further manifested their dependency on medication, and some of them had circumscribed delusions. The pain-mana
