A case is presented of a 49-year-old man with psoriasis limited to the nails for 12 years. The patient had no evidence of psoriasis elsewhere. He had been treated with a year-long course of griseofulvin and 2 months of weekly fluconazole without effect before the diagnosis of psoriasis was made by biopsy of the lateral nail fold. Options for the treatment of nail psoriasis are reviewed.

The pathogenesis and treatment of psoriasis are reviewed. Psoriasis is characterized by defects in the normal cycle of epidermal development that lead to epidermal hyperproliferation, altered maturation of skin cells, and vascular changes and inflammation. The condition typically manifests as areas of thickened, flaky, silvery white and reddened skin that may hurt, itch, and bleed. Biochemical markers of psoriasis are changes in levels of keratins, keratinocyte transglutamase, migration inhibitory factor-related protein, skin-derived antileukoproteinase, involucrin, small protein rich protein …

BACKGROUND: Guttate psoriasis is a distinctive acute form of psoriasis which characteristically occurs in children and young adults. Very little specific evidence-based guidance is available in standard texts to help make rational decisions about treatment options.
OBJECTIVES: To assess the effectiveness of treatments for guttate psoriasis.
SEARCH STRATEGY: We searched the Cochrane Clinical Trials Register (Cochrane Library, Issue 3, 1999), Medline (1966- September 1999), Embase (1988-September 1999), Salford Database of Psoriasis Trials (to November 1999) and European Dermato-Epidemiology N…

A seven-year-old boy with neurofibromatosis who developed scalp psoriasis is presented. The clinical evaluation of the patient revealed multiple café au lait spots, axillary freckling, bilateral Lisch nodules and a psoriatic plaque on his scalp. Though there is no known direct relationship between neurofibromatosis and psoriasis, one is suggested in this patient, possibly related to a common genetic defect.

Psoriasis is a common and chronic skin disorder under active investigation around the world. Despite this, determination of its genetic basis, role of the immune system in the disease pathophysiology and development of effective therapy, have been hampered severely by the absence of any spontaneous psoriatic skin disease in animals. Furthermore, until recently, validated animal models designed to create psoriasis were unavailable to investigative skin biologists and clinical scientists. However, there is at least one animal model which has been established and validated; it uses human skin eng…

IgA nephropathy (IGAN) was first described by Berger and Hinglais in 1968. It is now the most common glomerular disease worldwide. IGAN has been associated with several diseases. Its association with psoriasis has been rarely described. We report a case of IGAN with crescentic changes, associated with psoriasis vulgaris. We review the literature on the association of IGAN with psoriasis and discuss the likely pathogenetic linkage.

Although the relationship between streptococcal tonsillitis and acute guttate psoriasis is well recognized, its relationship to chronic forms of psoriasis is less established. In order to explore this further, the authors questioned 35 patients with severe psoriasis and 35 age-matched eczema controls about their history of significant sore throats (i.e. requiring a GP visit) and any resultant worsening of the skin condition. A third of the psoriasis patients reported recurrent sore throats which worsened their skin condition. This was true of only one (3%) of the 35 eczema controls. The author…

Pustular psoriasis and erythrodermic psoriasis are generally less stable, more challenging to treat, and more severe for the patient than classical plaque-type psoriasis vulgaris. The risk for systemic infection and overwhelming sepsis in these forms of psoriasis emphasizes the potential life-threatening impact of psoriasis. Careful attention to management strategies enables the astute clinician to help the patient improve, despite the challenges these difficult forms of psoriasis present.

This article reviews the use of systemic retinoids in the treatment of psoriasis. Various retinoids are discussed in the treatment of generalized pustular psoriasis, exfoliative psoriasis, and plaque psoriasis, both in combination therapy and monotherapy. Dosing outlines, side effects, and recommended monitoring guidelines are reviewed.

Psoriatic skin disease is common; it occurs at all ages and co-exists with joint disease in approximately 10% of cases. All areas of skin, s … sufferers live normal lives and, with the exception of severe erythrodermic or generalized pustular psoriasis, there is no mortality. Morbidity, particularly social and occupational, is more of a problem than is often acknowledged.

We treated childhood psoriasis with methotrexate (MTX) in seven children (4 boys, 3 girls) over 7.5 years. Their ages and duration of disease varied from 3.5 to 16 years (mean 12.14 yrs) and 4.8 months to 5 years (mean 2.2 yrs), respectively. Psoriatic erythroderma was seen in three patients, generalized pustular psoriasis in two, recalcitrant psoriasis and psoriatic arthropathy in one each. Pre-MTX liver biopsy performed in 4 children showed grade I changes. Methotrexate was given in a single weekly oral dose of 3.75 to 25 mg (mean 16.6 mg). The duration of treatment necessary to control the …

The demonstration of activated T lymphocytes, HLA-DR+ I-CAM 1+, gamma IP-10+ keratinocytes, and increased levels of lymphokines in active plaques suggests that immunologic mechanisms may play a role in the pathogenesis of psoriasis. Epidermal hyperplasia and inflammation in psoriasis may be linked by those cytokines many of which are produced by both keratinocytes and leukocytes. Epidermal acanthosis and keratinocyte mitoses have been observed in delayed-type hypersensitivity reactions and after the intradermal injection of gamma interferon. Gamma interferon and its induced proteins have been …

Parapsoriasis is not a disease entity. It is an auxiliary term introduced in 1902 to group several dermatoses with a faint similarity to psoriasis. The historical development leading to the creation of the term parapsoriasis is outlined and the history of psoriasis is briefly reviewed.

5 new cases of geographic stomatitis are presented and added to a review of 32 similar cases reported in the literature up to 1987. Furthermore, a series of 70 consecutive patients with psoriasis were examined for oral mucosal lesions. Based upon the findings of the literature and the results of the present study, there seems insufficient proof to consider geographic stomatitis an oral manifestation of psoriasis.

A pair of monozygotic developed a similar pattern of psoriatic lesions in of the distribution of lesions and the time of onset of the disease, in spite of living in different living conditions. This suggests a strong influence of genetic factors. Clinically, one had guttate psoriasis and the other, the nummular psoriasis. The - possible mode of inheritance is autosomal recessive trait.
