Combined NMDA Inhibitor Use in a Patient With Multisubstance-induced Psychotic Disorder.

UNLABELLED: : Novel psychoactive substance use is a major social concern. Their use may elicit or uncover unpredictably as yet undescribed clinical pictures. We aimed to illustrate a multisubstance use case indistinguishable from paranoid schizophrenia, so to alert clinicians on possibly misdiagnosing substance-induced psychotic disorders. CASE REPORT: We describe a case of a 32-year-old man who started at 18 years with cannabinoids and ketamine, and is currently using N-methyl-D-aspartate (NMDA) antagonists. At age 23, he developed social withdrawal after being assaulted by a stranger, but d…

Case report of comorbid alcohol-induced psychotic disorder and Madelung's disease.

Madelung's disease, also known as benign symmetric lipomatosis (BSL), multiple symmetric lipomatosis (MSL), fatty neck syndrome or Launois-Bensaude syndrome, is a rare disease characterized by the presence of multiple, symmetric, loose adipose tissues distributed around the neck, occipitalis, shoulder, back or chest. The fat masses are non-encapsulated and therefore can move freely between adjacent areas. This disease is most commonly seen among middle-aged Caucasian men of Mediterranean origins; it is rarely reported in Asia. Among individuals with Madelung's syndrome, 60 to 90% have a histor

The significance of possible selves in patients of an early intervention programme for psychotic disorders.

AIMS: To assess the possible importance of patients' perceptions of possible future selves in determining self-esteem and mood. METHODS: One hundred and two patients of an early intervention programme for psychotic disorders completed measures of perceived positive and negative current and possible future selves, as well as indices of self-esteem and mood. RESULTS: Measures of current and possible selves generally showed bivariate correlations with self-esteem, depression and anger/hostility. Perceived negative possible self was the most important independent predictor of self-esteem, depres…

[A case of affective disorder with psychotic symptoms as late manifestation of Huntington's Chorea].

We report about a woman of 60 years who received psychiatric inpatient treatment for an affective disorder with psychotic symptoms on several occasions. As time elapsed symptoms of dementia became more and more obvious. Despite a comprehensive workup with neuroimaging methods (SPECT, PET) the correct diagnosis of Huntington's Chorea was not attained until the characteristic movements appeared. Up till then pathologic movements had hardly occurred and there were no known cases of Huntington's Chorea in the family. This case is remarkable as the patient was not only treated with different antide

Parsing the relationship of stigma and insight to psychological well-being in psychotic disorders.

BACKGROUND: It has been postulated that the effects of the stigma of mental illness on the psychological well-being of patients is mediated through internalization of the stigma. On the other hand, there is reason to suppose that simple awareness of public stigma could also have an impact to the extent that an individual is aware of being ill. AIM: To investigate whether internalization of the stigma of having a psychotic disorder and an interaction between perceived public stigma and awareness of being ill make independent contributions to the prediction of psychological well-being in patien

Clinical staging in the pathophysiology of psychotic and affective disorders: facilitation of prognosis and treatment.

The prevailing utility, and indeed necessity, of clinical staging models applied in considerations of neuropsychiatric disease progressions is discussed from the perspectives of schizophrenia spectrum disorders and affective disorders, cannabis in schizopsychotic disorder, incidences of affect and psychosis, staging disorders in aging and the indices and prevalence of apathy. There would appear to be a strong current consensus that the pursuit of clinical staging of these and other brain disease states has contributed a systematic conceptual instrument to facilitate the better understanding, d

Polymorphisms in SLC6A4, PAH, GABRB3, and MAOB and modification of psychotic disorder features.

We tested four genes [phenylalanine hydroxylase (PAH), the serotonin transporter (SLC6A4), monoamine oxidase B (MAOB), and the gamma-aminobutyric acid A receptor beta-3 subunit (GABRB3)] for their impact on five schizophrenia symptom factors: delusions, hallucinations, mania, depression, and negative symptoms. In a 90 family subset of the Irish Study of High Density Schizophrenia Families, the PAH 232 bp microsatellite allele demonstrated significant association with the delusions factor using both QTDT (F=8.0, p=.031) and QPDTPHASE (chi-square=12.54, p=.028). Also, a significant association b

Treatment of psychotic symptoms in bipolar disorder with aripiprazole monotherapy: a meta-analysis.

BACKGROUND: We present a systematic review and meta-analysis of the available clinical trials concerning the usefulness of aripiprazole in the treatment of the psychotic symptoms in bipolar disorder. METHODS: A systematic MEDLINE and repository search concerning clinical trials for aripiprazole in bipolar disorder was conducted. RESULTS: The meta-analysis of four randomised controlled trials (RCTs) on acute mania suggests that the effect size of aripiprazole versus placebo was equal to 0.14 but a more reliable and accurate estimation is 0.18 for the total Positive and Negative Syndrome Scale

Alterations of the dopamine transporter in resting lymphocytes of patients with different psychotic disorders.

The aim of our study was to investigate and compare the dopamine (DA) transporter (DAT) in resting lymphocytes of 20 psychotic patients and 20 healthy control subjects, by means of both the binding parameters (Bmax and Kd) of 3H-WIN 35,428, and the reuptake parameters (Vmax and Km) of 3H-DA. The results showed that both the Bmax of 3H-WIN 35,428 binding and the Vmax of 3H-DA reuptake of the patients were significantly lower than those of healthy subjects, while the Kd or Km did not show any change. These findings, while indicating a reduced density of the lymphocyte DAT proteins, provide furth

The catatonia conundrum: evidence of psychomotor phenomena as a symptom dimension in psychotic disorders.

To provide a rational basis for reconceptualizing catatonia in Diagnostic and Statistical Manual of Mental Disorders (Fifth Edition), we briefly review historical sources, the psychopathology of catatonia, and the relevance of catatonic schizophrenia in contemporary practice and research. In contrast to Kahlbaum, Kraepelin and others (Jaspers, Kleist, and Schneider) recognized the prevalence of motor symptoms in diverse psychiatric disorders but concluded that the unique pattern and persistence of certain psychomotor phenomena defined a "catatonic" subtype of schizophrenia, based on intensive

An exploratory analysis of neurocognition in methamphetamine-induced psychotic disorder and paranoid schizophrenia.

OBJECTIVE: This exploratory study compared the neurocognitive performances of individuals with methamphetamine-induced psychotic disorder (MA psychosis) and paranoid schizophrenia. BACKGROUND: MA psychosis presents very similarly to paranoid schizophrenia and the relationship between the 2 disorders is not well-understood. Neurocognitive functioning is one potential factor for comparison, as deficits are documented in both schizophrenia and methamphetamine dependence. METHOD: Participants were selected through chart review. Neurocognitive test scores for participants with paranoid schizophre

Risk factors for the emergence of psychotic disorders in adolescents with 22q11.2 deletion syndrome.

OBJECTIVE: The 22q11.2 deletion syndrome is the most common known genetic risk factor for the development of schizophrenia. The authors conducted a longitudinal evaluation of adolescents with 22q11.2 deletion syndrome to identify early risk factors for the development of psychotic disorders. METHOD: Sixty children, 31 with 22q11.2 deletion syndrome and 29 comparison subjects with idiopathic developmental disability matched for age and IQ, underwent a baseline evaluation between 1998 and 2000; of these, 51 children (28 and 23 in the two groups, respectively) underwent follow-up evaluation betw…

Characteristics of subgroups of individuals with psychotic illness and a comorbid substance use disorder.

OBJECTIVES: The co-occurrence of severe mental illness and substance use disorder, or dual diagnosis, is prevalent and is associated with significant clinical and social problems. Most studies have treated persons with a dual diagnosis as a homogeneous population, grouping together different substances of misuse. This study investigated whether subgroups defined by their main substances of misuse were heterogeneous. The primary hypothesis was that users of stimulants, such as cocaine or amphetamines, would be characterized by especially high rates of inpatient admission, violence, and self-har

The CBT of delusional disorder: the relationship between schema vulnerability and psychotic content.

This paper reports the process, interventions, and outcome of the cognitive-behavioural therapy of a patient who had a diagnosis of delusional disorder. The clinical features, process of therapy, and successful outcome are all discussed in relation to explanatory information processing models of delusion development and maintenance, and recent developments in formulation-driven CBT of delusions. The implications for therapy, service development, and training are discussed. Beliefs that mediate the relationship between core schemas and delusions may form a helpful focus for early therapeutic en

Nosological implications of psychotic symptoms in patients with established posttraumatic stress disorder.

OBJECTIVE: We describe three patients with chronic posttraumatic stress disorder (PTSD) who later developed psychotic symptoms. The nosological implications are discussed. CLINICAL PICTURE: All three patients had no prior history of psychotic illness. The content of the psychotic symptoms was restricted to core features of the traumatic experiences which initially led to PTSD. TREATMENT: The patients received low-dose antipsychotic medications in addition to other treatments for PTSD and/or depression. OUTCOME: All three patients developed severe extrapyramidal side effects, particularly ak

Psychotic features in chronic posttraumatic stress disorder and schizophrenia: comparative severity.

Psychotic features are frequent in combat veterans with chronic posttraumatic stress disorder (PTSD), may correlate with severity of PTSD symptoms, and may reflect a distinct subtype of the disorder. These psychotic features include auditory and visual hallucinations and delusional thinking that is usually paranoid in nature. Psychotic features may be under-recognized in chronic PTSD because patients are reluctant to report these symptoms and because they may not have overt changes in affect or bizarre delusions characteristic of other psychoses, e.g., schizophrenia. To further assess these ph

Importance of psychotic features to long-term course in major depressive disorder.

OBJECTIVE: Most efforts to describe the prognostic significance of psychotic features in depression have been limited to single assessments 1 year or less after the initial evaluation. However, the various biological and treatment response differences between patients with psychotic and nonpsychotic depression suggest that prognostic differences may be very long-term. METHOD: The 787 patients described here entered the study as they sought treatment at one of five academic medical centers; they had either RDC major depressive disorder or schizo-affective depression (other than the mainly schi

Therapy of a case of psychotic personality organization manifesting gender identity disorder.

The author relates his experience with a case of psychotic personality organization manifesting transsexualism spanning about 10 years. This paper presents the case and discusses the psychodynamics together with the course of therapy. The patient was a case of primary transsexualism who fell into hallucinatory-delusional states each time he seriously tried to become a woman. His transsexual wish was identification with his idealized grandmother image, for defense of his helpless and vulnerable self, and an attempt to reestablish his self which was facing a crisis of disintegration due to disor

Expressive pragmatic language in mood and psychotic disorders: a systematic review and meta-analysis.

Pragmatic language impairments-difficulties using language effectively in social contexts-are common in adults suffering from severe mental illnesses (SMIs) such as schizophrenia spectrum disorders (SSD), major depressive disorder (MDD), and bipolar disorder (BD). These impairments hinder social functioning and recovery but have been explored most widely using comprehension tasks, with pragmatic production being poorly described. We undertook a systematic review and meta-analysis of studies assessing expressive pragmatic language in adults with SMIs versus healthy controls. 18 items were teste
