[Uses and risks of hemodynamic monitoring by inflow catheter in acute myocardial infarct].

Using hemodynamic monitoring by flow-directed heart catheterization in acute myocardial infarction, left ventricular dysfunction can be analyzed and quantified. Differential therapy concerning substitution of volume and administration of drugs with influence on pre- and after-load is facilitated. Since the introduction of the Swan-Ganz flow-directed catheter the monitoring has been technically improved with the additional advantage of lower incidence of catheter-induced arrhythmias and the possibility to measure the pulmonary capillary wedge pressure. The Swan-Ganz thermodilution catheter furt

[Recurrent necrosis as a complication of the subacute period of myocardial infarct].

Recurrent myocardial infarction was observed in 105 among 656 myocardial infarction patients (16.1%). The recurrence developed more often after macro-focal infarction, especially when complicated by cardiogenic shock and/or pulmonary oedema. The clinical pattern of a recurrence was characterized by a significant increase of the incidence of the arrhythmic and gastralgic variants of myocardial infarction. Among those surviving acute myocardial infarction for over 72 hours recurrent necrosis was the leading cause of death in 78%. A faster hospital rehabilitation of the myocardial infarction pati

Simultaneously Occurring Ischemic Stroke, Leg Artery Occlusion, and Pulmonary Embolism Induced by a Uterine Myoma.

OBJECTIVES: Paradoxical embolism from venous thrombosis through the patent foramen ovale is a rare but well-known cause of stroke in young adults. Here, we report a case of simultaneous middle cerebral artery infarction, multiple occlusions of the leg arteries, and pulmonary thromboembolism from the venous thrombus, all due to compression of the external iliac vein by a uterine leiomyoma. MATERIALS AND METHODS (CASE PRESENTATION): A 44-year-old woman presented with left hemiparesis and central-type left facial palsy. She denied a history of hypertension, diabetes mellitus, previous cerebral i

Acute Mitral Regurgitation with Unilateral Left-Sided Pulmonary Edema: A Complication of STEMI Treated Successfully with a MitraClip Procedure.

A 67-year-old man with inferior wall ST-segment elevation myocardial infarction underwent Impella-assisted percutaneous coronary intervention complicated by unilateral left-sided pulmonary edema and cardiogenic shock due to severe mitral valve regurgitation. Surgery was deferred due to hemodynamic instability and a high risk of mortality, so he underwent a MitraClip procedure. Mitral regurgitation (MR) is a catastrophic mechanical complication of myocardial infarction that leads to the development of pulmonary edema, cardiogenic shock, and death. After the procedure, the patient significantly

[A case report of right-sided cardiac and pulmonary thromboembolism treated by emergent operation].

A forty-four-year-old man with a clinical diagnosis of diabetes melitus and severe obesity (height 170 cm, weight 108 kg) was admitted to the hospital on 12th January 1995 because of acute myocardial infarction, and on 21st January, he was referred to our hospital with sudden onset of shock, bradycardia, loss of consciousness in spite of having recovered well from myocardial infarction. The echocardiography and pulmonary arteriography revealed a pulmonary embolism and a tumor in the right atrium. Administration of tissue plasminogen activator (TPA) was not sufficiently effective. An emergency

[A clinical and pathological study of 46 cases of sudden and unexpected death].

A series of 46 autopsied adult cases of sudden and unexpected natural death were investigated. In this study, sudden and unexpected death was defined as any death occurring with 24 hours of onset of symptoms in a person with or without probable cause of death suggested by medical history. The cases included 31 males and 15 females aged 26 to 85 years (mean 66.6 years). Age distribution peaked in seventies. The lesions causing sudden and unexpected death according to the most frequent organ systems were, diseases of the heart (acute myocardial infarction with or without old infarct, 20; old myo

Cocaine: A Provoking Risk Factor in Venous Thromboembolism.

Cocaine is a highly addictive recreational drug that is a well-known cause of a variety of disease processes such as stroke, myocardial infarction, and even sudden cardiac death. In current literature, venous thrombosis secondary to cocaine abuse remains under-examined, while the harmful effects of the drug within the arterial vasculature are well-studied and understood. Our case presents a patient who was found to have a large pulmonary embolism and pulmonary infarction after several days of cocaine abuse. This report serves to raise awareness of a potentially life-threatening effect of this …

[Allergic granulomatous angiitis associated with cerebral infarction, myo-pericarditis and acute respiratory failure due to eosinophilic pneumonia].

We encountered a 23-year-old woman with allergic granulomatous angiitis (AGA) associated with cerebral infarction, myo-pericarditis, and acute respiratory failure due to extended eosinophilic pneumonia. She underwent emergency treatment at our hospital because of right hemiparesis and impaired consciousness. AGA was suspected because the patient had a history of bronchial asthma accompanied by pulmonary infiltrations with eosinophilia, and presented with diffuse pulmonary infiltrates, pericardial effusion, diffuse hypokinesis of myocardium, cerebral infarction and marked peripheral eosinophlia

Pulmonary vein thrombosis associated with a large hiatal hernia.

BACKGROUND: Pulmonary vein thrombosis is a rare and potentially life-threatening condition. Reported cases of pulmonary vein thrombosis commonly occur as a complication of pulmonary surgery. There is a paucity of literature describing its clinical manifestations and non-operative causes. OBJECTIVE: We report the unique case of pulmonary vein thrombosis associated with a large hiatal hernia in a patient initially presenting with renal infarction. CASE REPORT: A 68-year-old man initially presented with increasing intensity of left lower quadrant pain. Contrast-enhanced computed tomography of t

Acute Coronary Artery Thrombosis in a Patient With Non-Small Cell Lung Cancer.

Patients with cancer are at major risk for both venous and arterial thrombotic complications. Venous involvement of cancer-associated thrombosis encompasses deep vein thrombosis and pulmonary embolism. Arterial manifestations include mainly stroke and myocardial infarction. We present the case of a 59-year-old woman admitted to the hospital for chest pain of five hours duration. She had been diagnosed with advanced lung cancer one month before. Electrocardiogram showed ST-segment elevation in all leads except aVR, suggesting a myocardial infarction. Coronary angiography revealed thrombi in bot

Trousseau's syndrome associated with pulmonary pleomorphic carcinoma exhibiting aggressive features: A case report.

Trousseau's syndrome is characterized as an unexpected, cancer-related thrombotic event, such as a cerebral infarction or a deep vein thrombosis/pulmonary embolism. We describe the first reported case of Trousseau's syndrome with pulmonary pleomorphic carcinoma and aggressive features. A 74 year-old man presenting with a pulmonary mass, which was identified as pleomorphic carcinoma with extensive lymph node involvement, in the left lower lobe, underwent a left lower lobectomy. Immunohistochemical analysis revealed that neoplastic cells exhibited an extensive expression of tissue factors with a

Anomalous origin of left coronary artery from pulmonary artery associated with pulmonary hypertension.

This is a report on a 10-year-old child with anomalous origin of left coronary artery (LCA) from pulmonary artery (ALCAPA), severe pulmonary hypertension (PH), old myocardial infarction and poor intercoronary collateralization. It discusses the echocardiographic pitfalls in this particular setting and introduces a new echocardiographic view (posterior pulmonary cusp view) for visualization of the anomalous origin of LCA from the posterior pulmonary cusp (PC) in patients with ALCAPA from the PC of the pulmonary artery. We describe three echocardiographic pitfalls that can mislead the echocardio

The role of rib infarcts in the acute chest syndrome of sickle cell diseases.

The acute chest syndrome is a generic term for pulmonary complications of sickle cell diseases with heterogeneous etiologies that include pneumonia, vaso-occlusion of pulmonary arterioles, rib infarction, and fat embolism syndrome. My review summarizes these etiologies, the evidence, and pathophysiology supporting the hypothesis that infarction of segments of ribs by the same vaso-occlusive process responsible for the acute episodes of pain (characteristic of the sickle cell diseases) is often involved in the acute chest structure. Inflammation associated with the infarct then causes splinting

Incentive spirometry to prevent acute pulmonary complications in sickle cell diseases.

BACKGROUND: This study was designed to determine the incidence of thoracic bone infarction in patients with sickle cell diseases who were hospitalized with acute chest or back pain above the diaphragm and to test the hypothesis that incentive spirometry can decrease the incidence of atelectasis and pulmonary infiltrates. METHODS: We conducted a prospective, randomized trial in 29 patients between 8 and 21 years of age with sickle cell diseases who had 38 episodes of acute chest or back pain above the diaphragm and were hospitalized. Each episode of pain was considered to be an independent eve
