A health equity ECHO for clinicians of individuals with SCD.

SCD is the most common genetic disorder in the United States, affecting nearly 100 000 Americans, with the majority of affected individuals identifying as Black. Studies have shown that both children and adults with SCD face stigmatization resulting in low-quality care stemming from institutional racism and implicit bias by clinicians. Clinician biases often result from a lack of awareness of their own racial identity, failure to develop an anti-racist or social justice lens, and absence of skills to mitigate racist practices and policies. The Sickle Treatment and Outcomes Research in the Midw…

The African American Historical Loss and Trauma Response Scales: Development and initial psychometric evaluation.

Racism in the United States has constituted transgenerational legacies of trauma for people of African descent. Scholars have highlighted the immense emotional and psychological consequences of race-based traumatic stress; yet a complete understanding of the impact of historical trauma for African Americans is hampered by the lack of instrumentation for use in quantitative methodologies. We thus sought to develop psychometrically sound instruments to better understand impacts derived from the injury of enslavement and ongoing racial traumas and the resulting emotional impacts of historical los…

HIV-related stigma in pregnancy and early postpartum of mothers living with HIV in Ontario, Canada.

HIV-related stigma is associated with many psychological challenges; however, minimal research has explored how perceived HIV-related stigma … racteristics, perceived stress, depression symptoms, social isolation, social support and perceived racism in the third trimester and 3, 6 and 12 months postpartum. Multivariable linear regression was employed to explore the relationship between HIV-related stigma and multiple independent variables. HIV-related stigma generally increased from pregnancy t …

"You can't whoop autism": Intersectional stigma in the Black community.

Black caregivers of autistic children encounter numerous barriers to accessing diagnoses and services, leading to adverse outcomes due to delayed diagnosis and limited access to services. The presence of stigma within the Black community, combined with interpersonal and institutional racism, may contribute to these ongoing inequalities; however, few studies address these forms of discrimination together. This study is based on 55 in-depth interviews conducted with caregivers of Black autistic children receiving services at a public autism clinic in the U.S. South. Using grounded theory analysi…

Black health professional families: assessment of strengths and stability.

Despite pernicious racism, most black health professionals achieve professional success and most have stable marriages. A 19-item questionnaire was designed to assess this hypothesis following a lecture on the strengths and weaknesses of black health professional families to physicians, dentists, and their spouses (n = 38) at the 1990 annual meeting of the combined Old North State Medical and Dental Societies of North Carolina. Surprisingly, these families seemed eager to disclose feelings about themselves, their professions, and their marriages. Major preliminary findings were that these heal…

Race-based experiences of ethnic minority health professionals: Arab physicians and nurses in Israeli public healthcare organizations.

UNLABELLED: Increasing workforce diversity was found to contribute to the narrowing of disparities in health. However, racism toward ethnic minority health professionals has not been adequately researched. In Israel, public healthcare organizations that serve a mixed Jewish-Arab population employ Arab minority healthcare professionals. Instances of prejudice and manifestations of racism toward them, which frequently surface in public discussion and the media, have unfortunately gained little scholarly attention. We used the intergroup contact approach and the theory of the social process of ev…

Using community-based participatory research methods to inform care for patients experiencing homelessness: An opportunity for resident education on health care disparities.

People experiencing homelessness (PEH) suffer higher burdens of chronic illnesses, have higher rates of emergency medicine (ED) use and hospitalization, and ultimately are at increased risk for premature death compared to housed counterparts. Structural racism contributes to a disproportionate burden of homelessness among people of color. PEH experience not only significant medical concerns but also complex social needs that need to be addressed concurrently for effective healing, issues that have been magnified by the COVID-19 pandemic. As health disparities and structural racism intersect am…

Still I rise: The need for self-validation and self-care in the midst of adversities faced by Black women with HIV.

OBJECTIVES: Psychosocial factors of trauma and abuse, racial discrimination, HIV stigma, and gender-related stressors (e.g., prioritizing others' needs) have been associated with antiretroviral treatment (ART) nonadherence and poor viral suppression among Black women living with HIV (BWLWH). To inform the development of an intervention addressing these psychosocial factors to improve ART adherence, the authors sought the insight of BWLWH. METHOD: Qualitative semistructured interviews were conducted with 30 BWLWH to gather information on their experiences with trauma, racism, HIV stigma, gende…

How to Be An Antiracist Hand Surgery Educator.

Racism that unjustly marginalizes black people in the United States is not a new concept. It underlies nearly every aspect of American history, leading to the systemic racism that is ingrained in our society today. With the recurrent theme of people of color having worse health outcomes than the majority community, it is important for hand surgeons to employ cultural competence and antiracist education to provide better care and support for patients, staff, students, and colleagues. In this article, we will provide a definitional framework, clinical examples, and practical pearls to promote ch…

An Intersectional Examination of How Class, Marital Status, and Age Shape Discrimination Among Black Women Who Are Postpartum.

INTRODUCTION: Intersectionality posits that multiple social categories (eg, race, gender, class) interact to create systems in which people experience discrimination differently. Among Black women, limited work has explored intersections beyond race, pregnancy status, and gender. This study compares experiences of racism and discrimination by multidimensional social category, inclusive of socioeconomic status (SES), marital status, and age, among Black women who are postpartum. METHODS: We analyzed cross-sectional survey data from 197 self-identified Black women aged 18 years or older with a …

How colorism gets "under the skin": The role of ethnic-racial discrimination and diurnal cortisol in the physical health of African American and Latino young adults.

Empirical research has identified ethnic-racial discrimination as a fundamental driver of health disparities and a key contributor to poorer physical health outcomes among African American and Latino populations. Colorism-bias that generally privileges lighter skin over darker skin-has also been linked to adverse health outcomes; however, little to no research has explored how the intersection of colorism and racism "gets under the skin" to influence physical health, particularly among young adults. Grounded in an intersectional framework, the present study recognizes that multiple systems of …

Psychosocial Stressors and Heart Failure Risk Among Black Women in the US.

IMPORTANCE: Black women are disproportionately susceptible to developing heart failure and display more severe morbidity and mortality compared with White women. Factors contributing to excess heart failure risk among Black women remain incompletely understood. Dear Health Promotion Scholar: Letters of Life From, for, and About Black Women in Academia.

Black women are change agents actively working within their power to combat systemic racism in academia, while constantly battling injustices. Understanding lived realities and experiences of racial ethnic minority women as "outsiders within" is crucial for confronting long-standing racism embedded within academic spaces. Institutions may be outwardly addressing racial injustice, and perpetuating injustices internally, whether known or unknown. Using a relational dialectics framework and letter writing style, the purpose of this commentary is to describe the complexities present in experiences…

Inequities in Anticipatory Stress of Police Brutality and Depressed Mood Among Women.

BACKGROUND: Police brutality towards racially minoritized populations is structural racism. Even though most of the research on the health impacts of police brutality centers the experiences of men, women are also harmed by this structural violence. OBJECTIVES: We identify factors associated with the anticipatory stress of police brutality among women and examine its relationship with depressed mood across ethno-racial categories. METHODS: Data came from the cross-sectional Survey of the Health of Urban Residents in the United States (N = 2796). Logistic regressions were used to identify fac…

Racial trauma and its health impacts in Black people in the United States and Canada: a scoping review on conceptualizations, definitions and manifestations.

Despite growing recognition of racism as a public health crisis, racial trauma remains inconsistently defined and under researched. This scoping review investigates how racial trauma is conceptualized in empirical literature, focusing on its manifestations and long-term health impacts among Black individuals and communities in the United States and Canada. Guided by the Joanna Briggs Institute methodology for conducting scoping reviews, we systematically searched ten multidisciplinary databases and identified 29 empirical studies published between 2000 and 2025. Most included studies were cond…

Is medical education systemically racist?

After the murder of George Floyd, many professions, organizations, and institutions have begun to confront the long and persistent legacy of racism in the United States. Within that context, it is critically important for the medical education community to address the question of whether medical education is systemically racist, and if so, what should be done to address this problem. In this commentary, this author seeks to answer these questions, primarily by focusing on the role of standardized multiple-choice examinations in determining who gains admission to medical school and which medica…

Impact of Racial and Ethnic Disparities on Patients With Dilated Cardiomyopathy: JACC Focus Seminar 7/9.

Significant race- and ethnicity-based disparities among those diagnosed with dilated cardiomyopathy (DCM) exist and are deeply rooted in the history of many societies. The role of social determinants of racial disparities, including racism and bias, is often overlooked in cardiology. DCM incidence is higher in Black subjects; survival and other outcome measures are worse in Black patients with DCM, with fewer referrals for transplantation. DCM in Black patients is underrecognized and under-referred for effective therapies, a consequence of a complex interplay of social and socioeconomic factor…

Historical Redlining Impacts Contemporary Environmental and Asthma-related Outcomes in Black Adults.

Rationale: Environmental threats and poorly controlled asthma disproportionately burden Black people. Some have attributed this to socioeconomic or biologic factors; however, racism, specifically historical redlining, a U.S. discriminatory mortgage lending practice in existence between the 1930s and the 1970s, may have actuated and then perpetuated poor asthma-related outcomes. Objectives: To link historical redlining (institutional racism) to contemporary environmental quality- and lung health-related racial inequity. Methods: Leveraging a broadly recruited asthma registry, we geocoded 1,034 …
