Radiomics-Based Differential Diagnosis of Radicular Cysts and Apical Granulomas on CBCT Images Using RadC-CNN Architecture.

Background/Objectives: This study aims to evaluate the diagnostic performance of radiomic features derived from cone-beam computed tomography (CBCT) images in differentiating radicular cysts (RC) from periapical granulomas (PG). The study also compares the performance of traditional machine learning (ML) algorithms with a novel deep learning (DL) model, Radiomics Cyst Convolutional Neural Network (RadC-CNN). Methods: CBCT images of 98 patients (55 RC, 43 PG), confirmed by histopathological diagnosis, were retrospectively analyzed. Lesions were semi-automatically segmented in 3D Slicer, and 48 …

[A case of successful application of platelet-rich fibrin to eliminate a large residual cavity after enucleation of a radicular cyst].

A clinical case of successful application of platelet-rich fibrin (PRF) to eliminate a large residual cavity after enucleation of a radicular cyst is described. The method is based on the use of biological substances that produce growth factors that stimulate reparative osteogenesis. A similar source of growth factors is PRF. According to the literature, it is able to reduce inflammation of periodontal tissues, preserve the location of the alveoli and eliminate defects in the alveolar bone, thereby enhancing bone regeneration and ensuring its good stability. PRF is easy to manufacture, does no…

Conservative management of large radicular cysts associated with non-vital primary teeth: a case series and literature review.

Radicular cysts are the most common odontogenic cystic lesions of inflammatory origin. Large radicular cyst is comparatively less frequently associated with primary teeth. They represent only 0.5-3.3% of the total number of cysts in primary dentition. Radicular cysts arising from deciduous teeth are reported to occur in age range of 3-19 years with a male predominance. Although large radicular cysts are treated by enucleation with extensive removal of bone and vital teeth, marsupialization can be preferred as a conservative approach to reduce the morbidity. The purpose of this article is to re…

[A technic for performing tooth-preserving operations in treating radicular cysts and chronic periodontitis of the permanent teeth in children and adolescents].

A high incidence of complications developing during and after removal of teeth with chronic periodontitis and radicular cysts has made the dentists resort to conservative surgical methods of treatment of such conditions. These methods are replantation, resection of radical apex, hemisection, radicular amputation, granulectomy, crown-radicular separation, crown-radicular amputation, hemireplantation, and inter-radicular granulectomy. The technique of such treatment is described in detail. Conservative surgical methods of treatment include the following stages: (1) conservative therapy; (2) surg…

Surgical Management of Large Radicular Cyst Associated with Mandibular Deciduous Molar Using Platelet-rich Fibrin Augmentation: A Rare Case Report.

Radicular cysts arising from deciduous teeth are rare and usually cause a large bony defect. Autologous platelet-rich fibrin (PRF) is an easily available healing biomaterial in oral surgical defect with the new perspective of accelerated healing of a large bony defect. The present case is of unusually large radicular associated with neglected carious mandibular deciduous second molar in 10-year-old girl and its surgical management with PRF augmentation as a healing biomaterial in the bony defect. One-year follow-up showed uneventful healing and eruption of succedaneous tooth. Healing was relat…

Tooth Transplantation and a Root Submergence Technique After Enucleation of Multiple Continuous Radicular Cysts for Implant Treatment: A Case Report.

When a radicular cyst-involved tooth is challenging to save, tooth extraction and cyst enucleation are required. Since only thin facial bone is present on the coronal side of cystic defects, bundle bone loss and continuity of the cystic cavity after tooth extraction may result in extensive vertical bone defects. To prevent major bone augmentation, the following procedure was performed: the left maxillary incisor (#9), which exhibited apical lesions was extracted, while the right maxillary incisor (#8), despite having a healthy periodontal ligament but being difficult to save, was transplanted …

Preserving the vitality of teeth adjacent to a large radicular cyst in periapical microsurgery: a case report with 4-year follow-up.

BACKGROUND: Radicular cysts may enlarge considerably, cause extensive bone destruction, and jeopardize the integrity of the associated vital teeth. The different treatment approaches are aimed mainly at eliminating the cystic epithelial membrane while reducing the risk of injury to vital structures. Contrary to other treatment modalities, preapical surgery offers an unequivocal single occasion resolution for the patient. However, it has been associated with higher risk of collateral damages. CASE PRESENTATION: A patient presented with a large radicular cyst originating from a maxillary latera…

Recovery of Pulp Sensibility After the Surgical Management of a Large Radicular Cyst: A Case Report with a 4.5-Year Follow-up.

In addition to pathogenic teeth associated with cysts, the roots of adjacent teeth are often included in the cystic cavity. Whether these teeth require elective endodontic treatment followed by cystic enucleation remains unclear. In the case presented herein, we aimed to preserve the pulp of the teeth included in the cystic lesion. Unfortunately, the sensibility of the included teeth was negative after endodontic surgery, including cystic enucleation. However, the sensibility recovered after 1 year and was maintained throughout a 4.5-year follow-up. Therefore, we suggest that elective endodont

T-helper cell type 17/regulatory T-cell immunoregulatory balance in human radicular cysts and periapical granulomas.

INTRODUCTION: Cysts and granulomas are chronic periapical lesions mediated by a set of inflammatory mediators that develop to contain a periapical infection. This study analyzed the nature of the inflammatory infiltrate, presence of mast cells, and in situ expression of cytokines (interleukin [IL]-17 and transforming growth factor [TGF]-beta), chemokines (macrophage inflammatory protein [MIP]-1beta and monocyte chemotactic protein [MCP]-1), and nuclear transcription factor (FoxP3) in human periapical granulomas and cysts compared with a control group. METHODS: Fifty-five lesions (25 periapica
