[Idionodal reciprocal tachycardia irresponsive to pharmacological prophylaxis. Description of a case successfully treated with radio frequency].

The case of a 63-year old female suffering from recurrent paroxysmal supraventricular tachycardia for 30 years is reported. In the past she was treated unsuccessfully with multiple drugs. Electrophysiological study showed dual nodal pathways and the inducibility of a junctional reciprocating tachycardia with RR and V-A intervals respectively 360-410 and 40 msec. Radiofrequency was applied for 30" in the nodal region during induced reciprocating tachycardia. The arrhythmia was interrupted after a few seconds and a 1st degree A-V block persisted for a few minutes. Repeated electrophysiological s

Radiofrequency Ablation Effectively Treated Focal Recurrence of Mesothelioma.

A 55-year-old man with malignant pleural mesothelioma underwent multimodality treatment comprising induction chemotherapy followed by extrapleural pneumonectomy and radiation therapy. After 2.5 years, focal recurrence occurred, with computed tomography revealing a tumor in the left cardiophrenic angle. Surgery was considered a problem for the patient because of the previous extrapleural pneumonectomy and difficult tumor location. Radiofrequency ablation was therefore performed; the course was uneventful, and there was no recurrence. Radiofrequency ablation should be considered an option to tre

Procedural Treatments as Adjuncts in the Management of Acne Vulgaris: A Narrative Review.

BACKGROUND: Acne vulgaris is a chronic inflammatory disorder of the pilosebaceous unit and one of the most prevalent dermatological conditions globally. Its multifactorial pathogenesis and variable clinical presentation necessitate a multimodal therapeutic approach. While pharmacological interventions remain first-line therapy, cosmetic and dermatologic procedures are increasingly utilized as adjunctive strategies to enhance clinical outcomes. This narrative review synthesizes current evidence on four commonly employed cosmetic modalities in acne management: chemical peels, mechanical peels, l

Efficacy of radiofrequency catheter ablation in treatment of elderly patients with supraventricular tachyarrhythmias and ventricular tachycardia.

Radiofrequency catheter ablation was performed in 100 men and 81 women, mean age 78 +/- 5 years, referred for ablation of atrial flutter, supraventricular tachycardia, and ventricular tachycardia, and for ablation of the atrioventricular junction with permanent pacemaker implantation in patients with atrial fibrillation and a rapid ventricular rate not controlled by drug therapy. A hematoma in 1 of 182 ablation procedures (<1%) was the only complication. Radiofrequency catheter ablation was successful in treating 63 of 70 patients (90%) with atrial flutter, in treating 60 of 66 patients (91%)

Catheter ablation for atrial flutter and fibrillation. An effective alternative to medical therapy.

Atrial flutter and atrial fibrillation are common arrhythmias that can be difficult to manage clinically. In many patients, these conditions are refractory to pharmacologic therapy because of drug failure or intolerance. Radiofrequency catheter ablation may be a reasonable alternative in patients with typical atrial flutter. The procedure has a high initial success rate and a low complication rate. However, recurrence after ablation is common, and a second treatment session may be needed. In selected patients with atrial fibrillation, radiofrequency ablation can be useful for rate control. How

Towards the optimization of management of hepatocellular carcinoma.

Hepatocellular carcinoma (HCC) is the fifth most common neoplasm in the world, closely correlated with viral hepatitis and liver cirrhosis. The vast majority of HCC patients present at a late stage and are unsuitable for surgery due to limited liver functional reserve. Tumors can involve major vessels or hilar structures, necessitating major liver resection and/or rendering liver resection unfeasible. A series of new technologies have been developed to optimise HCC management. Stem cell therapy improves impaired liver functional reserve prior to liver resection. Intravascular radiofrequency ab

Stereotactic hyperthermia for brain tumors.

We have developed a localized hyperthermia system using computed tomography-stereotactic surgery as malignant brain tumor therapy. In an experimental study, the temperature in the area of the cat brain within 20 mm from a radiofrequency electrode reached more than 43.7 degrees C. Intravenous administration of MCNU during hyperthermia caused a significant increase of MCNU content in the heated brain as compared with a control brain. In the clinical study, localized radiofrequent hyperthermia using stereotactic surgery was performed on 7 malignant deep-seated gliomas and 21 metastatic brain tumo

Interventional Uterine-Sparing Therapies for Leiomyomas: A Commentary on Myolysis.

Uterine leiomyomas (myomas, fibroids) are very common in premenopausal women and frequently are associated with abnormal uterine bleeding, bulk effects, and reproductive issues. In women who wish to retain their uterus and/or to enhance or retain fertility, interventional therapies have been introduced, including radiofrequency, laser (chromo), cryotherapy, and magnetic resonance (MR)-guided high-intensity focused ultrasound (MRgHIFU) myolysis. In formulating this commentary, all modalities of myolysis from systematic reviews, randomized controlled clinical trials, and observational studies th

Effects of Ultracavitation and Radiofrequency on Abdominal Adiposity.

BACKGROUND: The mechanism of fat reduction from radiofrequency occurs via the thermal stimulation of adipocyte metabolism, causing a lipase-mediated triglyceride enzyme degradation, apoptosis, and adipocyte rupture. Ultracavitation promotes fat reduction using adipose cell destruction through mechanical stimulus. The literature on the combination of these two physical agents, named ultrafrequency, is still scarce and requires consistent analysis of the effects of this therapy combination. OBJECTIVE: The objective of this study was to investigate the effects of radiofrequency associated with u

Accuracy of integration of multislice computed tomography imaging into three-dimensional electroanatomic mapping for real-time guided radiofrequency ablation of left atrial fibrillation-influence of heart rhythm and radiofrequency lesions.

Circumferential radiofrequency ablation around the orifices of the pulmonary veins is a curative catheter-based therapy of paroxysmal and persistent atrial fibrillation (AF). Three-dimensional cardiac image integration is a promising new technology to visualize the complex left atrial anatomy and neighbouring structures. This study aimed to validate the accuracy of integrating multislice computed tomography (MSCT) into three-dimensional electroanatomic mapping (EAM) to guide radiofrequency catheter ablation (CA) of AF. Forty consecutive patients (34 male, mean age 56 +/- 10 years) with multidr

Radiofrequency ablation of atrial flutter. Efficacy of an anatomically guided approach.

BACKGROUND: Previous reports have shown that radiofrequency ablation can terminate atrial flutter and prevent recurrences. However, different methods have been used, and the current experience remains limited. The objective of the present study was to determine the efficacy of radiofrequency ablation of atrial tissue in patients with atrial flutter using an anatomically guided approach. METHODS AND RESULTS: We treated 22 patients aged 30 to 73 years. Atrial flutter was recurrent for a mean of 5 years despite the administration of multiple antiarrhythmic drugs. Radiofrequency current was direc

[Non-pharmacological treatment of arterial fibrillation].

In selected patients with atrial fibrillation and severe symptoms, non-pharmacological treatment may be an alternative or supplement to medical therapy. Atrioventricular nodal radiofrequency ablation (requires pacemaker implantation), or atrial pacing for sick sinus syndrome, are established treatment modalities. All other non-pharmacological therapies for atrial fibrillation are still experimental. After the "Maze" operation, atrial depolarisation has to follow one specific path determined by surgical scars in the myocardium. This prevents new episodes of atrial fibrillation, but at the cost

Anti-Fibrotic Effects of RF Electric Currents.

Hypertrophic scars and keloids are two different manifestations of excessive dermal fibrosis and are caused by an alteration in the normal wound-healing process. Treatment with radiofrequency (RF)-based therapies has proven to be useful in reducing hypertrophic scars. In this study, the effect of one of these radiofrequency therapies, Capacitive Resistive Electrical Transfer Therapy (CRET) on biomarkers of skin fibrosis was investigated. For this, in cultures of human myofibroblasts treated with CRET therapy or sham-treated, proliferation (XTT Assay), apoptosis (TUNEL Assay), and cell migratio

Radiofrequency Catheter Ablation: How to Manage and Prevent Collateral Damage?

Radiofrequency catheter ablation has become the standard of care for the management of various arrhythmias and, in fact, the first-line therapy for many tachyarrhythmias. It entails creating scar tissue in the heart in regions where abnormal impulses form or propagate to restore normal cardiac conduction. As the heart is a complex organ and is surrounded by and related to many other anatomical structures, it is important to avoid the collateral damage that can happen from radiofrequency (RF) ablation on the endocardium as well as on the epicardium. This review explores methods for mitigating o

Delayed vertebral body collapse after stereotactic radiosurgery and radiofrequency ablation: Case report with histopathologic-MRI correlation.

Stereotactic radiosurgery and percutaneous radiofrequency ablation are emerging therapies for pain palliation and local control of spinal metastases. However, the post-treatment imaging findings are not well characterized and the risk of long-term complications is unknown. We present the case of a 46-year-old woman with delayed vertebral body collapse after stereotactic radiosurgery and radiofrequency ablation of a painful lumbar metastasis. Histopathologic-MRI correlation confirmed osteonecrosis as the underlying etiology and demonstrated that treatment-induced vascular fibrosis and tumor pro

Long-term pacemaker dependency after radiofrequency ablation of the atrioventricular junction.

This prospective study was conducted to determine the percentage of patients with long-term pacemaker dependency after successful radiofrequency ablation of the atrioventricular junction. Abrupt inhibition of the pacemaker was performed 13.5 +/- 8.1 months after ablation in 59 patients. A > or =5-second asystole was considered to indicate pacemaker dependency. Pacemaker dependency was present in 18 patients. Absence of escape rhythm immediately after ablation was strongly associated with a higher incidence of long-term pacemaker dependency. The following variables were not associated with pace

[Some considerations about the treatment of trigeminal neuralgia with intrathecal baclofen.].

A case history of therapy resistent trigeminal neuralgia was presented by Wechsung and Müller. This patient was previously treated without any succes with glycerol injections, "electrocauterisation", a Janetta operation and 2 reexplorations of the posterior fossa. Attempts with oral carbamazepine and baclofen were unsuccesful too. The authors did not try out the combination of carbamazepine and baclofen, which is recommended by Fromm and Terrence. When conservative treatment fails, surgical procedures are indicated. The most succesful surgical procedure is the radiofrequency thermal lesion of

Dropped head syndrome: report of a rare complication after multilevel bilateral cervical radiofrequency neurotomy.

INTRODUCTION: Cervical radiofrequency neurotomy is a safe and relatively low-risk procedure commonly used to treat facet joint-mediated axial neck pain. Severe complications are extremely rare and can be avoided with proper technique and appropriate imaging guidance. This article describes the development and subsequent management of a case of dropped head syndrome after cervical radiofrequency neurotomy. METHODS: A 77-year-old man with cervicalgia, multilevel facet arthropathy, and a known kyphosis in the setting of cervical degenerative disk disease underwent successful conventional radiofr
