The 2014 Bowman Lecture-Bowman's and Bruch's: a tale of two membranes during the laser revolution.

To describe the historical evolution of the role of lasers in effecting therapeutic changes in the four acellular membranes of the eye. Over the past 50 years, iterative developments have been instituted in lasers used for various forms of eye surgery predominately on the basis of data generated in early experiments in the 1960s to determine thresholds for damage and their incorporation in codes of practice for laser safety. The evolutionary steps are described. Excimer laser technology resulted in the generation of the new field of laser refractive surgery with over 40 million individuals now

Complications of radial and transverse keratotomy.

Radial keratotomy for myopia and transverse keratotomy for astigmatism are the most commonly performed refractive surgical procedures. A decade of experience with modern techniques has produced considerable literature on the complications of keratotomy. Vision-threatening complications (bacterial keratitis, traumatic rupture of the globe through weakened keratotomy scars, endophthalmitis, cataract formation from surgical trauma to the lens) are quite rare, occurring in less than 1% of eyes in published series. The most common side effects affect most patients in the first few months after surg…

Effective spherical aberration of the cornea as a quantitative descriptor in corneal topography.

Following excimer laser photorefractive keratectomy and other refractive surgical procedures, complaints of halos, glare, and monocular diplopia are common. These procedures increase the asphericity of the cornea, which may cause the optical distortions. We used ray tracing techniques to estimate the longitudinal monochromatic aberration of the cornea from the measured corneal topography (effective spherical aberration) in 15 normal eyes with varying degrees of astigmatism and in ten eyes after photorefractive keratectomy. Best spherical corrected visual acuity in the astigmatic eyes was highl…

Sustainability in Cataract and Refractive Surgery: Current Challenges and Future Perspectives.

Cataract and refractive surgery are integral to improving vision health and quality of life; however, their growing environmental impact poses significant concerns. These procedures contribute notably to medical waste, excessive energy consumption, and pharmaceutical overuse, amplifying the carbon footprint of health care. Key environmental challenges include the dependence on single-use surgical instruments, high energy demands from laser and phacoemulsification systems, and the waste associated with medication disposal and packaging. Although some sustainable initiatives, such as reusable su

Evaluation of the toric implantable collamer lens for simultaneous treatment of myopia and astigmatism.

Myopic astigmatism is a prevalent condition that can be treated with spectacles, contact lenses, or laser refractive surgery. However, these treatment options have functional limitations at higher levels of refractive error. The toric implantable collamer lens is designed to treat a broad range of refractive error, generally up to -18 diopters with +1 to +6 diopters of astigmatism. Approval for a more limited treatment range of up to 15 diopters of myopia with +1 to +4 diopters of astigmatism is being sought in the US, where this device has not yet received marketing approval. Surgical correct

Comparison of laser and manual removal of corneal epithelium for photorefractive keratectomy.

BACKGROUND: Photorefractive keratectomy relies on precise ablation of cornea stromal tissue to achieve a desired change in refraction. The routine technique for photorefractive keratectomy has been manual debridement of the epithelium prior to performing excimer laser ablation. We investigated whether laser ablation versus manual debridement of the corneal epithelium influences the refractive result. METHODS: A retrospective matched controlled study analyzing the refractive outcome of 46 eyes after excimer laser photorefractive keratectomy was performed. Half of the eyes had the corneal epith

[Therapeutic Options in Keratoconus].

A recent epidemiology study revealed that prevalence of keratoconus was much higher than previously assumed. Therefore, it is no longer deemed an "orphan disease", as it has a relevant socioeconomic impact on the healthcare system. One of the most important risk factors for developing keratoconus is chronic eye rubbing which, apart from other known risk factors such as atopy or Down's syndrome, is the only modifiable factor. Informing the patient and offering behavior modifying therapies seems to be essential. Further therapeutic options regarding improvement of visual function include the wea

Theoretical analysis of wavefront aberration from treatment decentration with oblique incidence after conventional laser refractive surgery.

Analysis of induced wavefront aberration after refractive surgery is important in the design of vision correction and the development of visual correction technology. Based on a mathematical model of the anterior corneal surface, the influence of treatment decentration on induced wavefront aberrations was studied by considering oblique incidence. The results revealed that significant coma was induced from the treatment translation, and it was nearly proportional to the translation or corrected refraction of vision correction. The induced aberrations from the lateral translation correlated with

The wound healing response after laser in situ keratomileusis and photorefractive keratectomy: elusive control of biological variability and effect on custom laser vision correction.

Biological diversity in the wound healing response is thought to be a major factor limiting the predictability of the outcome of refractive surgical procedures such as laser in situ keratomileusis and photorefractive keratectomy. Corneal wound healing is critical to the success of topography-linked or wave front-linked excimer laser ablation to optimize visual performance. This is because of the importance of retaining subtle features of custom ablation and the tendency of epithelial hyperplasia and stromal remodeling to obscure these features following either procedure. The corneal wound heal…
