PURPOSE: Quadriceps weakness is common after anterior cruciate ligament (ACL) reconstruction, resulting in prolonged disability and increased risk for reinjury and osteoarthritis. Functional resistance training (FRT) combines resistance training with task-specific training and may prove beneficial in restoring quadriceps strength. The primary purpose of this study was to determine if a walking-specific FRT program (e.g., resisted walking) improves knee strength in individuals after ACL reconstruction.
METHODS: Thirty participants were randomized into one of three groups: 1) FRT with a customi

OBJECTIVE: Current return-to-sport decisions are primarily based on elapsed time since surgery or injury and strength measures. Given data that show rates of successful return to competitive sport at around 55%, there is strong rationale for adopting tools that will better inform return to sport decisions. The authors' objective was to assess reactive strength as a metric for informing return-to-sport decisions.
DESIGN: Case-control design.
METHODS: Fifteen elite athletes from national sports teams (23 [6.0] y) in the final phase of their return-to-sport protocol following a unilateral knee

INTRODUCTION: Children often present to the emergency department for treatment of abuse-related injuries. ED providers-including emergency nurses-do not consistently screen children for abuse, which may allow abuse to go undetected and increases the risk for reinjury and death. ED providers frequently cite lack of knowledge or confidence in screening for and detecting child abuse. The purpose of this quality improvement project was to implement an evidence-based screening program that included provider education on child physical abuse, a systematic screening protocol, and use of the validated

BACKGROUND: Following anterior cruciate ligament reconstruction (ACLR), patients undergo a battery of performance assessments to determine progression of return to sport activity. Rates of reinjury following ACLR are high, indicating that current assessments may not accurately identify deficits at the time point of return to sport progression.
RESEARCH QUESTION: To assess single-leg postural control in ACLR patients around the time point of return to sport progression and their relationships to subjective function.
STUDY DESIGN: Descriptive Laboratory Study.
METHODS: 198 individuals (108 AC

Muscle strain injuries disrupt the muscle-tendon unit, early rehabilitation is associated with a faster return to sports (RTS), but the time course of tissue healing remains sparsely described. The purpose was to examine tissue regeneration and the effectiveness of early versus delayed rehabilitation onset on functional and structural recovery after strain injuries. A total of 50 recreational athletes with a severe acute strain injury in their thigh or calf muscles were randomized to early or delayed rehabilitation onset. Magnetic resonance imaging (MRI) was obtained initially, 3 and 6 months

OBJECTIVE: The main mechanism of injury to the spine is torsion especially when coupled with compression. In this study, the in vitro torsional stiffness of the lumbar spine segments is compared in flexion and extension positions by cyclic and failure testing.
METHODS: Fifteen lumbar spines were sectioned from fresh cadavers into 15 L2/3 and 15 L45 motion segments. Each vertebral segment was then potted superiorly and inferiorly in polymethylmethacrylate, effectively creating a bone-disk-bone construct. The potted spinal segments were mounted in a mechanical testing system, preloaded in compr

INTRODUCTION: Certain scapular fractures are best treated with open reduction to restore form and function to the glenohumeral joint. The purpose of this study was to review the results of operative treatment of scapular fractures at a level one trauma center.
PATIENTS AND METHODS: Twenty-one cases of operatively treated scapular fractures over a four-year period were identified after a database query. Nine patients had inadequate follow-up and one had a reinjury excluding a total of ten patients from our analysis. This left 11 patients (eight male and three female) with displaced scapular ne

INTRODUCTION: Eccentric exercise, where the contracting muscle is lengthened, produces microscopic damage in muscle fibers, and sensations of stiffness and soreness, the next day. These normally resolve within a week. A more major sports injury is the muscle strain. Because strain injuries are known to occur during eccentric contractions, it is hypothesized that the microscopic damage from eccentric exercise can, at times, progress to a muscle strain. As the amount of microscopic damage depends on the muscle's optimum length for active tension, it is further proposed that optimum length is a m

From 1986-1992, a total of 21 ACL reconstructions were completed in 20 skeletally immature athletes with an average age of 13.7 years (range: 11.8-15.6 years). Fifteen patients underwent operative reconstruction with hamstring tendons with wide open physes and 6 patients with bone-patellar tendon-bone (BPTB). All grafts were anatomically placed through transphyseal bone tunnels in the femur and tibia. Nineteen of 20 patients (20 of 21 reconstructions) returned for follow-up at an average of 34 months (range: 17-89 months). All patients had reached skeletal maturity at follow-up. No patient had

Residual symptoms resulting from recurrent episodes of inversion-type ankle sprains may be attributed to a decreased neuromuscular response of the peroneal or tibialis anterior muscles, thereby increasing the probability for reinjury. The purpose of this study was to examine the electromyographic (EMG) response time of the peroneal and tibialis anterior muscles in response to sudden plantar flexion/inversion stress in the chronically functional unstable and normal ankle. Subjects for this study consisted of 13 athletically active individuals (five males and eight females, mean age = 19.2 +/- 1

Three hundred fifty male recruits were divided into two groups: Walk (N = 170) and Run (N = 180). A physical screen was performed before the commencement of recruit training consisting of a visual assessment of the feet looking for pes planus, pes cavus, and deformities of the toes. Each recruit was also asked if he had sustained any injury in the preceding 2 years. The visual findings and history of prior injury were noted and compared with actual injuries sustained during the 12-week training course. Fifty-three subjects in the Walk group and 54 in the Run group were identified as being at r

The authors have reviewed 44 patients retrospectively who failed arthroscopic partial meniscectomy. The study attempts to define the chances for success and to identify prognostic factors as these patients return for reoperation. Each patient had a repeat arthroscopic examination from 2 to 60 months after partial meniscectomy (average 19 months). They were reviewed an average of 31 months after reoperation (6 to 60 months), and each completed a subjective questionnaire evaluating the efficacy of their repeat arthroscopic surgery. Seventy-one percent of the patients had improvement with reopera

BACKGROUND: Anterior cruciate ligament (ACL) injury is one of the most common sports-related injuries, often resulting in not only mechanical instability but also long-term proprioceptive dysfunction. The ACL is richly innervated with mechanoreceptors that contribute to sensorimotor control. Injury-induced degeneration of these receptors leads to disrupted afferent signaling and maladaptive central nervous system (CNS) responses, which can compromise functional recovery and increase the risk of reinjury.
OBJECTIVE: This review aims to summarize current knowledge on ACL-associated propriocepti

The objective of this study was to compare rates of anterior cruciate ligament (ACL) failure among adolescent patients to evaluate patella alta as a high-risk variable. Demographic and surgical data were retrospectively queried for patients ≤18 years of age with primary ACL reconstruction performed at a single academic center between 2011 and 2016 and minimum of 2-year clinical surveillance. Patellar height indices, including Caton-Deschamps index (CDI) and Insall-Salvati index (ISI), were retrospectively calculated from preoperative imaging to assess the presence of patella alta. Failure was

CONTEXT: After anterior cruciate ligament reconstruction (ACLR), long-term functional deficiencies can occur, with controversial results reported when comparing women and men. Dynamic balance and unilateral hop test performance are considered important indicators for the risk of reinjury of the lower extremity. Although both sexes seem to have a similar risk to experience a second anterior cruciate ligament injury, sex-specific differences of dynamic balance and unilateral hop performance in handball players following ACLR are unknown.
OBJECTIVE: To compare dynamic balance and unilateral hop
