Current trends in the diagnosis and management of renal nutcracker syndrome: a review.

Nutcracker syndrome is caused by compression of the left renal vein between the aorta and the superior mesenteric artery where it passes in the fork formed at the bifurcation of these arteries. The phenomenon results in left renal venous hypertension. The syndrome is manifested by left flank and abdominal pain, with or without unilateral haematuria. Other common presentation is as "pelvic congestion syndrome" characterized by symptoms of dysmenorrhea, dyspareunia, post-coital ache, lower abdominal pain, dysuria, pelvic, vulvar, gluteal or thigh varices and emotional disturbances. Likewise comp

Antihypertensive effect of 0.1-Hz blood pressure oscillations to the kidney.

BACKGROUND: Physiological blood pressure (BP) fluctuations with frequencies >0.1 Hz can override renal blood flow autoregulation. The influence of such immediate changes in renal perfusion pressure (RPP) on daily BP regulation, eg, via shear stress-stimulated liberation of renal endothelial NO, however, is unknown. Thus, we studied the effects of such RPP oscillations on renal function and on systemic BP during the onset of renal hypertension. METHODS AND RESULTS: Seven beagles (randomly assigned to each of the following protocols) were chronically instrumented for the measurement of systemic

Flow-dependence of vasodilatory response in renal vascular bed to hemorrhage in dogs.

The vasodilatory response in the renal vascular bed was studied after quick mild hemorrhage under different levels of renal blood flow. In neuraxis intact dogs, the renal vascular resistance increased or decreased within 5 s after hemorrhage and reached a stable level close to the control within 10 s. The responses under neuraxis intact conditions may involve both responses of the renal vascular resistance to changes in renal sympathetic nerve activity caused by the baroreceptor reflex and responses to decrease in renal perfusion pressure after hemorrhage. In order to exclude baroreflex neural

[Hemodynamic effects of positive pressure breathing].

A decrease of cardiac output during artificial ventilation with PEEP has been reported in 30-100% of patients. The drop in cardiac output is caused by (I) a decrease of transmural filling pressures of both ventricles, (II) an increase in afterload of the right ventricle, (III) a change of left ventricular geometry caused by a right to left shift of the interventricular septum in face of an enlarging right ventricle. Afterload enhancement and right to left interdependance are effective on high PEEP levels only (above 15 cm H2O). Pulmonary occlusion pressure represents left ventricular filling p

Obstructive aortic and renal thrombosis in the newborn--spontaneous recovery.

A 13-day-old infant presented with acute renal failure secondary to a large thrombus formation involving the umbilical aorta and both renal arteries. The initial clinical manifestations were hematuria, followed by anuria, severe dehydration, grunting respirations, and cyanosis of her feet. She was treated with intravenous fluids and peritoneal dialysis. At the onset, there was no blood flow into the abdominal aorta or into the kidneys. Heparinization and fibrinolytic therapies were unsuccessful in dissolving the clot. However, the aortic clot recanalized spontaneously a few weeks later, but th

Cyclosporine before Coronary Artery Bypass Grafting Does Not Prevent Postoperative Decreases in Renal Function: A Randomized Clinical Trial.

BACKGROUND: Acute kidney injury is a common complication after cardiac surgery, leading to increased morbidity and mortality. One suggested cause for acute kidney injury is extracorporeal circulation-induced ischemia-reperfusion injury. In animal studies, cyclosporine has been shown to reduce ischemia-reperfusion injury in the kidneys. We hypothesized that administering cyclosporine before extracorporeal circulation could protect the kidneys in patients undergoing cardiac surgery. METHODS: The Cyclosporine to Protect Renal Function in Cardiac Surgery (CiPRICS) study was an investigator-initia

Regulation of Renal and Extrarenal Calcitriol Synthesis and Its Clinical Implications.

There is evidence that calcitriol is the only biologically active vitamin D metabolite. This review summarizes data on the regulation of renal and extrarenal synthesis of calcitriol by nutritional, physiologic, mechanical, genetic, and disease-related factors. Relatively low circulating calcitriol due to low substrate availability, i.e., low circulating 25-hydroxyvitamin D, has been reported in nutritional rickets, osteomalacia, obesity, and preeclampsia. In these situations, vitamin D supplementation can increase circulating calcitriol and, together with calcium, prevent rickets/osteomalacia

The role of anesthesia and catecholamines in the renal response to mild hemorrhage.

The role of the sympathoadrenal system in the renal response to mild hemorrhage was investigated in animals anesthetized with either sodium pentobarbital (Group I) or alpha-chloralose (Group II). Significantly greater decreases in renal blood flow (RBF) and increases in renal vascular resistance (RVR) were seen after hemorrhage in Group II than in Group I. Concomitant with these changes were increases both in circulating catecholamines and plasma renin activities. In Group II, however, the rise in plasma epinephrine was also significantly higher than in Group I. Additional studies were perform

The Effects of Renal Denervation on Renal Hemodynamics and Renal Vasculature in a Porcine Model.

RATIONALE: Recently, the efficacy of renal denervation (RDN) has been debated. It is discussed whether RDN is able to adequately target the renal nerves. OBJECTIVE: We aimed to investigate how effective RDN was by means of functional hemodynamic measurements and nerve damage on histology. METHODS AND RESULTS: We performed hemodynamic measurements in both renal arteries of healthy pigs using a Doppler flow and pressure wire. Subsequently unilateral denervation was performed, followed by repeated bilateral hemodynamic measurements. Pigs were terminated directly after RDN or were followed for 3

Fetal renal artery flow velocity waveforms in the presence of congenital renal tract anomalies.

Colour Doppler flow mapping of the renal arteries and subsequent pulsed Doppler measurement of impedance to flow in these vessels were attempted in 33 fetuses with postnatally confirmed renal pathology. The majority presented with unilateral or bilateral hydronephrosis (n = 21) and bilateral renal agenesis (n = 8). Renal artery blood flow could be visualized in all, except for the eight cases of bilateral renal agenesis. Bilateral flow velocity recordings were collected in six out of 12 cases of bilateral hydronephrosis and in five out of nine cases of unilateral hydronephrosis. The pulsatilit

Renovascular hypertension: information from experiments using conscious dogs.

1. Evidence from experiments in conscious, instrumented dogs shows that hypertension from renal artery stenosis is due to: (i) the stimulus, the mechanical resistance of the stenosis; and (ii) the secondary responses to this, especially angiotensin II (initially) and cardiovascular hypertrophy. 2. The hydraulic resistance of the stenosis is responsible for about 20-25% of the rise in blood pressure. 3. Angiotensin II is initially the most important secondary response to the stenosis. Within days, however, other as yet undetermined factors become dominant in the maintenance of the hypertension.

Hypertension with co-existing renal disease.

It is generally accepted that treatment of systemic hypertension protects the kidney from haemodynamically-mediated injury. However, renal function may deteriorate during traditional antihypertensive therapy. Currently, hypertensive nephrosclerosis accounts for approximately 26% of all end-stage renal disease (ESRD) in the United States. Furthermore, in spite of aggressive treatment of hypertension, there is no evidence of a reduction in the incidence of hypertensive nephrosclerosis as a cause of ESRD. A current hypothesis states that renal protection is dependent on control of systemic hypert

[Intrarenal localisation of renorenal collaterals in the dog. Observations on renal arteries as end-arteries (author's transl)].

One of the two main branches of the left renal artery was slowly occluded in four dogs. Repeated angiography was carried out from 45 to 61 days postoperatively; this made it possible to observe the development of collaterals between the normal and constricted branches of the renal artery, i.e. intrarenal renorenal collaterals. These collaterals developed predominantly from preexisting vessels of the extraglomerular system in the vicinity of the renal pelvis and calyces and, to a lesser extent, from medullary vessels and from forating capsular arteries.

The Haemodialysis Session Effect on the Choroidal Thickness and Retinal and Choroidal Microcirculation-A Literature Review.

Haemodialysis (HD) is currently the most commonly used method of renal replacement therapy. The process of dialysis involves numerous changes that affect many systems, including the eye. The changes occurring in the course of HD may affect the ocular parameters, such as intraocular pressure, central corneal thickness, retinal thickness, retinal nerve fibre layer thickness, and choroidal thickness (CT). The choroid, being one of the most vascularized tissues, is characterized by the highest ratio of blood flow to tissue volume in the entire body, may be particularly susceptible to changes occur

Pathophysiology of renal hemodynamics.

The high blood flow rate/gram of kidney tissue supplies mainly the renal cortex. The net effect of the interaction of the renin-angiotensin system, the kallikrein-kinin system and prostaglandins is to autoregulate renal blood flow within a narrow range. Drugs and neurogenic factors also influence renal hemodynamics. The renal circulation responds to changes in extracellular fluid volume, and in cardiac output. Renal ischemia occurs readily as these parameters decrease and prompt correction of circulatory dynamics can restore renal blood flow and prevent tubular necrosis. With hypovolemia or he…

Biopsy-proven acute tubular necrosis in a child attributed to vancomycin intoxication.

Acute renal failure in children treated with vancomycin typically presents with interstitial nephritis. There is debate as to the extent of direct tubular toxicity attributable to vancomycin, especially in the absence of aminoglycoside treatment. We report a case of acute tubular necrosis (ATN) associated with vancomycin toxicity in an 8-year-old boy where there is no likely alternate explanation for toxic or ischemic injury. Treatment with hemodialysis resulted in the elimination of vancomycin from the circulation and subsequent improvement in renal function.

Seizure-induced rhabdomyolysis accompanied by acute renal failure in a dog.

Secondary complications from extreme and prolonged convulsive seizures occurred in a young dog. Physical exertion resulted in necrosis of a substantial amount of skeletal muscle. The release of large quantities of myoglobin into the circulation caused severe impairment of renal function. Anoxia, apparently associated with the inability to breathe during seizures, resulted in brain damage. It was concluded that, although primary therapy must be directed toward alleviation of intense seizure activity, subsequent screening for renal disease and corollary supportive therapy would be appropriate.

Elevated renal nerve activity after spinal transection: effects on renal function.

Spinal transection approximately doubles renal sympathetic activity (RSA) in rats. These experiments localized spinal pathways inhibiting RSA and determined the effects of transection-elicited renal sympathetic hyperactivity on renal circulation and renal function. Experiments were conducted in chloralose-anesthetized, paralyzed, artificially respired, male Sprague-Dawley rats. RSA was measured from an electrode on the left renal nerve. Renal arterial blood flow (RABF), glomerular filtration rate, urine flow rate, and renal sodium and potassium excretions were also measured. Localized lesions …

Erythrocyte membrane-encapsulated SZF nanocomposites for hyperuricemia therapy.

INTRODUCTION: Hyperuricemia is a prevalent metabolic disorder primarily induced by purine dysregulation. Current therapies face limitations due to systemic side effects and inadequate efficacy, necessitating novel treatment strategies. METHODS: In this study, a biomimetic nanodrug, SZF@PDA‑RM, was synthesized by hybridizing the traditional medicine Shizhifang (SZF) with polydopamine (PDA), followed by erythrocyte membrane coating. Its physicochemical properties and reactive oxygen species (ROS)-responsive release were characterized. In vitro, a hyperuricemia cell model using uric acid (UA)-st
