Machine learning-based forecast of Helmet-CPAP therapy failure in Acute Respiratory Distress Syndrome patients.

BACKGROUND AND OBJECTIVE: Helmet-Continuous Positive Airway Pressure (H-CPAP) is a non-invasive respiratory support that is used for the treatment of Acute Respiratory Distress Syndrome (ARDS), a severe medical condition diagnosed when symptoms like profound hypoxemia, pulmonary opacities on radiography, or unexplained respiratory failure are present. It can be classified as mild, moderate or severe. H-CPAP therapy is recommended as the initial treatment approach for mild ARDS. Even though the efficacy of H-CPAP in managing patients with moderate-to-severe hypoxemia remains unclear, its use ha

Effects of reducing exposure to air pollution on submaximal cardiopulmonary test in patients with heart failure: Analysis of the randomized, double-blind and controlled FILTER-HF trial.

BACKGROUND: Air pollution exposure could mitigate the health benefits of exercise in patients with heart failure (HF). We tested the effects of a respiratory filter on HF patients exposed to air pollution during exercise. METHODS AND RESULTS: Ancillary analysis of the FILTER-HF trial, focused on the exercise outcomes. In a randomized, double-blind, 3-way crossover design, 26 HF patients and 15 control volunteers were exposed to clean air, unfiltered dilute diesel engine exhaust (DE), or filtered DE for 6min during a submaximal cardiopulmonary testing in a controlled-exposure facility. Prospec

[Clinical research progress in non-invasive positive pressure ventilation from 2020 to 2021].

Non-invasive positive pressure ventilation (NPPV), an essential respiratory support method, is widely used in acute/chronic respiratory failure and assisting rehabilitation in patients with chronic obstructive pulmonary disease (COPD). We searched the relevant research articles about NPPV published from 1st October 2020 to 30th September 2021 through Medline. Researches focusing on the clinical application and viral transmission protection during high-flow nasal cannula oxygen and NPPV in COVID-19, were mainly retrospective and of small sample size. It demonstrated that high-flow nasal cannula

Efficacy and safety of a low-flow veno-venous carbon dioxide removal device: results of an experimental study in adult sheep.

INTRODUCTION: Extracorporeal lung assist, an extreme resource in patients with acute respiratory failure (ARF), is expanding its indications since knowledge about ventilator-induced lung injury has increased and protective ventilation has become the standard in ARF. METHODS: A prospective study on seven adult sheep was conducted to quantify carbon dioxide (CO2) removal and evaluate the safety of an extracorporeal membrane gas exchanger placed in a veno-venous pump-driven bypass. Animals were anaesthetised, intubated, ventilated in order to reach hypercapnia, and then connected to the CO2 remo

The Evolving Role of Extracorporeal Carbon Dioxide Removal in Acute Respiratory Failure: A Narrative Review.

Extracorporeal carbon dioxide removal (ECCO₂R) has emerged as a supportive therapy for patients with acute respiratory failure, particularly when refractory hypercapnia and respiratory acidosis persist despite standard management. This narrative review summarizes the physiological rationale, current evidence, and clinical applications of ECCO₂R in modern critical care practice. ECCO₂R removes carbon dioxide directly from the bloodstream, thereby facilitating lung-protective ventilation strategies and potentially reducing ventilator-induced lung injury. The technique has been primarily evaluate

HEPA air filtration for asthma and COPD: lessons from randomized controlled trials.

INTRODUCTION: People with asthma and chronic obstructive pulmonary disease (COPD) may experience worsening respiratory symptoms during high pollution events. High-Efficiency Particulate Air (HEPA) filtration removes fine particles and may reduce indoor exposure and respiratory morbidity. AREAS COVERED: We narratively synthesized randomized controlled trials (RCTs) of portable HEPA filtration in asthma and COPD. PubMed and Scopus were searched from inception to 5 May 2026, reference lists were updated on 10 June 2026. Trials consistently demonstrated substantial reductions in indoor PM2.5, but

Noninvasive ventilation in children: A review for the pediatric anesthesiologist.

Preserving adequate respiratory function is essential in the perioperative period. Mechanical ventilation with endotracheal intubation is widely used for this purpose. In select patients, noninvasive ventilation (NIV) may be an alternative to invasive ventilation or may complement respiratory management. NIV is used to provide ventilatory support and increase gas exchange at the alveolar level without the use of an invasive artificial airway such as an endotracheal tube or tracheostomy. NIV includes both continuous positive airway pressure (CPAP) and noninvasive positive pressure ventilation.

Diagnostic and therapeutic endonasal rhinologic procedures generating aerosol during COVID-19 pandemic: a systematized review.

INTRODUCTION: Most rhinologic procedures, particularly endoscopic sinonasal procedures, are liable to produce aerosols. The severe acute respiratory syndrome coronavirus 2 (SARS-Co V-2) transmits via respiratory droplets, but the degree of its spread through airborne routes by aerosol is unclear. OBJECTIVE: The aim of this article is to counsel rhinologists on how to modify their conventional practice during the COVID-19 pandemic by prioritising the need of procedures, identifying aerosol- generating procedures and using precise personal protection equipment for various endonasal procedures.

[Natural latex allergy. Patient management: from clinic to prevention. A review].

The widespread use of latex devices has been followed, in the last 25 years, by an increase in IgE mediated sensitization. The clinical manifestations of latex allergy affect the skin (urticaria and angioneurotic oedema), the lower and the upper respiratory tracts (rhinoconjunctivitis, asthma and glottis oedema), and the cardiovascular system (anaphylaxis). There is also an anaphylactic risk during surgery and invasive diagnostic procedures. Vegetable food cross-reacts with latex so that more than half of the patients show specific IgE against some food. Further than traditional groups at risk

Physiologic effect and safety of the pumpless extracorporeal interventional lung assist system in patients with acute respiratory failure--a pilot study.

Interventional lung assist (iLA) effectively reduces CO(2) tension and permits protective lung ventilation in patients with acute respiratory distress syndrome. However, there is little experience in using iLA in acute respiratory failure from various causes and no experience for small body sizes such as Asian patients. We evaluated the physiologic effect and safety of the iLA device in patients with acute respiratory failure from various causes. We enrolled 11 consecutive patients with severe respiratory failure from various causes. Wire-enforced cannulae (13-15 Fr) were inserted under ultras

Protective performance of medical fabrics used in protective clothing and masks against viral particles smaller than severe acute respiratory syndrome coronavirus 2.

BACKGROUND: The coronavirus disease 2019 (COVID-19) pandemic prompted proactive infection control measures, including improved hand hygiene and mask-wearing; however, viral respiratory infections, such as those caused by rhinoviruses, were not fully controlled by such public health measures during the pandemic. Because rhinovirus, a member of the Picornaviridae family, has a smaller particle size compared with coronaviruses, we investigated the protective performance of personal protective equipment (PPE), including protective clothing and masks, against picornaviruses. METHODS: We analyzed t

Treatment of Severe Hypercapnic Respiratory Failure Caused by SARS-CoV-2 Lung Injury with ECCO2R Using the Hemolung Respiratory Assist System.

Acute respiratory distress syndrome (ARDS) due to COVID-19 leads to a high rate of mortality in the intensive care unit (ICU). A lung-protective mechanical ventilation strategy using low tidal volumes is a cornerstone to management, but uncontrolled hypercapnia is a life-threatening consequence among severe cases. A mechanism to prevent progressive hypercapnia may offset hemodynamic instability among patients who develop hypercapnia. We present the case of a woman in her mid-60's with severe acute hypercapnic respiratory failure secondary to COVID-19 pneumonia who was successfully treated with

Mice lacking GFAP are hypersensitive to traumatic cerebrospinal injury.

Glial fibrillary acidic protein (GFAP) is an intermediate filament protein expressed primarily in astrocytes. We have tested whether GFAP protects against mechanical stress by inducing percussive head injury in GFAP-null mice with a weight drop device. When mice were positioned on a foam bed which allowed head movement at impact, all 14 wild-type mice tested survived, but 12 of 15 GFAP-null mice died within a few minutes. The cause of death appeared to be upper cervical spinal cord injury resulting in respiratory arrest. When the foam bed was replaced by a firm support, both GFAP-null and wild
