PURPOSE: To report a case of bilateral simultaneous central retinal artery occlusion (CRAO) following head injury in a young 29-year-old man.
METHODS: A 29-year-old man presented with head injury following road traffic accident. Posterior segment evaluation revealed CRAO in both eyes.
RESULTS: The patient was treated for CRAO in the form of immediate ocular massage, paracentesis, intravenous mannitol and transdermal isosorbide dinitrate patch. Despite treatment the vision continued to be no perception of light. Systemic investigations were unremarkable. Color Doppler of carotid arteries show…

This article describes optical coherence tomography (OCT) findings in patients with central retinal artery occlusion by conducting a retrospective review of two cases. Central retinal artery occlusion shows a distinct pattern on OCT images. In the acute phase, OCT images demonstrate the increased reflectivity and thickness of the inner retina and a corresponding decrease of reflectivity in the outer layer of the retina and retinal pigment epithelium/choriocapillaris layer. Follow-up OCT images demonstrate a decrease in the reflectivity and thickness of the inner retinal layers and a correspond…

A 46-year old woman developed simultaneous central retinal artery occlusions (CRAOs) in Wegener granulomatosis (WG). She had presented six years earlier with xerostomia, skin rash, and arthralgias and received a diagnosis of Sjogren syndrome. Anti-neutrophilic cytoplasmic antibody (ANCA) was negative. Months prior to the CRAOs, she had developed hearing loss, proptosis, and scleritis that were not responsive to prednisone 50 mg/d. The CRAOs occurred while she was being treated at this dose level. ANCA was now positive. This is the 12th case of CRAO in WG and the 6th case of bilateral CRAOs rep…

Stimulant abuse is associated with ischemic stroke. This report describes a 43-year-old woman with a central retinal artery occlusion following the use of intranasal methamphetamine. Toxicology testing should be considered in patients presenting with acute visual loss.

PURPOSE: To describe a rare bilateral central retinal artery occlusion in a patient with Churg-Strauss syndrome.
DESIGN: Observational case report.
METHODS: A 68-year-old Japanese man with Churg-Strauss syndrome developed sudden vision loss in both eyes. Medical history included bronchial asthma, elevated eosinophils, and aortic regurgitation.
RESULT: Central retinal artery occlusion was diagnosed by retinal whitening, a cherry-red spot, and delayed arteriovenous transit time on fluorescein angiography. Corticosteroids, cyclophosphamide, and anticoagulant therapy altered the hypereosinophil…

BACKGROUND: Acute central retinal artery occlusion occurs as a sudden interruption of the blood supply to the retina and results in an almost complete loss of vision in the affected eye. There is no generally agreed treatment regimen although a number of therapeutic interventions have been proposed.
OBJECTIVES: The objective of this review is to examine the effects of treatments used for acute non-arteritic central retinal artery occlusion.
SEARCH STRATEGY: We searched the Cochrane Controlled Trials Register - CENTRAL/CCTR, which contains the Cochrane Eyes and Vision Group specialised regist…

An occlusion of the central retinal artery (CRAO) leads to a unilateral acute painless loss of vision. The individual etiology remains unclear in many cases. Potential pathomechanisms are embolism, vasoobliteration and vascular compression. Emboli (calcified, thrombotic, myxomatous, bacterial or cholesterol) are of carotid or cardiac origin. Atherosclerotic plaques, giant-cell arteritis and other types of vasculitis may cause vasoobliteration. A retrobulbar mass (hematoma, neoplasms, retrobulbar injections) may lead to an optic nerve and central retinal artery compression. Funduscopic signs of…

BACKGROUND: We report a case of bilateral simultaneous central retinal artery occlusion (CRAO). The time lag was 21 hours.
CASE: The patient was a 78-year-old woman. She had hypertension, arteriosclerosis, and bilateral thrombosis of the legs. Both eyes could only distinguish light and dark at the first medical examination.
RESULTS: We started steroid and urokinase therapy and used a vasodilator of peripheral vessels. When the patient left the hospital, visual acuity had improved to the ability of counting fingers (0.01 leftward 30 cm on the left).
CONCLUSIONS: Both visual field and color v…

gamma-Aminobutyric acid (GABA) is normally primarily in amacrine cells in the rat retina. Immediately after an ischaemic insult, attained by occlusion of the central retinal artery for 60 min, GABA is then found to be associated with Müller cells. During subsequent reperfusion, the distribution of GABA immunoreactivity gradually reverts from the glial cells back into neuronal elements of the retina. Twenty-four hours after ischaemia, GABA staining is indistinguishable from that seen in control animals. It is suggested that during central retinal artery occlusion, Müller cell energy levels are …

Previous animal models of central retinal artery occlusion using glass or latex embolic material may not simulate human disease. We have developed a rabbit model using human atherosclerotic material, which may be useful in developing embolus specific treatments, including thrombolysis or laser photodisruption. Fresh human atherosclerotic plaque was harvested from atherosclerotic human aorta by mechanical removal and suspension in normal saline. The suspension was agitated vigorously to produce small particles, which were separated into various sizes by filtration through mesh filters with spec…

A 56-year-old man, complaining of sudden visual loss in his left eye, demonstrated cream-colored retinal edema along a macular branch of the central retinal artery, overall delay of fluorescence in angiography and general depression of the central visual field, which were interpreted as the incomplete form of central retinal artery occlusion (CRAO) mimicking cilioretinal or branch retinal artery occlusion. After receiving paracentesis and fibrinolytic agents, the patient recovered his vision gradually, while the ophthalmoscopic findings progressed to show central retinal vein occlusion (CRVO) …

We report a case of bilateral retinal artery occlusion due to mitral valve prolapse. Most patients with retinal ischaemia in whom it is found have not been previously known to have it. Since it is a common condition it would seem essential that it be included in the differential diagnosis of amaurosis fugax and retinal artery occlusion if future ischaemic events are to be prevented. All patients with retinal ischaemia should have a full cardiovascular examination supplemented by echocardiography.

A central retinal artery occlusion occurred in the left eye of a 25-year-old woman with migraine and bilateral intrapapillary drusen. An extensive investigation failed to establish the presence of any systemic disease or vasculopathy. Eight years later she had a central retinal artery occlusion in her right eye. Optic disk drusen and migraine may have combined to produce this patient's bilateral visual loss consequent to retinal vaso-occlusion.
